Who will lead the health care?

Many of us have been wondering for a long time if we will ever see someone who will adequately lead the health sistema. And the tricky thing is to get to know what the hell means in a proper way. For some, it's keeping costs down and maintaining the freedom to choose a private service for whomever they want. For others, it is that the service should be forever universal and free. The consensus is similar a Gordian knot. 

Therefore, there will be no leaders to suit everyone. Nor will we be able to reach an agreement if we define them as defenders of the common good.

I expect nothing from the management structures. I have been on the NHS boat for many years now and I have heard only good words and seen very few gestures. Nor do I expect anything from professional organisations, which are dense, dusty and unfriendly to change and innovation. 

I hope that if there is a value proposition, it will come from the health professionals, as we have seen in this pandemic in which health managers and organisations have lagged behind the care providers by weeks. 

The problem is that with the current level of overload, professionals are focused on a single priority: survival. And this, I am afraid, does not benefit anyone, not society, which is aware that its health care facilities are burnt out and on the verge of exhaustion, not the managers who see that it is impossible to make changes, not the politicians who, despite announcing new hospitals, see the system crumble. 

What strength is left for a family doctor when at the end of her day she leaves exhausted because she has seen her patients, those of her colleague who is on leave without a substitute, those without an appointment, the home visitsand the telephone calls? What energy is left for the ICU nurse with the units full? What encouragement do I give to those who support the emergencies and see how more and more people are coming?

Cannon fodder. It's sad to realize that we all are. We're gonna need to hit rock bottom to make the decision to step up. Swarming societies may not help, but that does not mean that all doors will be closed to us. Everyone can make a revolution at the edges of their own inner world. That is where the future lies. If we manage to make contact with ourselves and see that we need very little to survive and that the potential for human growth that is open to us is almost infinite, we may have a chance. 






¿Quién liderará la sanidad?

Muchos llevamos tiempo preguntándonos si llegaremos a ver a alguien que lidere de una manera adecuada la sanidad. Y lo complicado es llegar a saber qué diablos es eso de una manera adecuada. Para unos es que los costes se mantengan contenidos y mantener la libertad de elegir un servicio privado para el que quiera. Para otros que el servicio sea por siempre universal y gratuito. El consenso no es que sea difícil sino que ha devenido en nudo gordiano. 

Por tanto no habrá líderes a gusto de todos. Tampoco si los definimos como defensores del bien común, termino para el que de nuevo no nos conseguiremos poner de acuerdo.

No espero nada de las estructuras de gestión. Son ya muchos años en el barco y solo he escuchado buenas palabras y visto muy pocos gestos. Tampoco de las organizaciones profesionales, densas, polvorientas y poco amigas al cambio y la innovación. 

Abrigo la esperanza de que si hay alguna propuesta de valor esta saldrá de los profesionales como hemos visto en esta pandemia en la que las gerencias y organizaciones sanitarias han ido semanas por detrás que los asistenciales. 

El problema es que con el nivel actual de sobrecarga los profesionales se centran en una única prioridad: sobrevivir. Y esto me temo no beneficia a nadie, ni a la sociedad que consiente que sus sanitarios estén quemados y al borde de la extenuación, ni a los gestores que ven que es imposible hacer cambios, ni a los políticos que pese a anunciar nuevos hospitales ven como el sistema se desmorona. 

¿Qué fuerza le queda a una médica de familia cuando al terminar su jornada sale agotada por haber visto sus pacientes, los de la compañera que está de baja sin suplente, los sin cita, los domicilios y las llamadas telefónicas? ¿Qué energía le queda a la enfermera de UCI con las unidades llenas? ¿Qué animo a los que sostienen las urgencias y ven como cada vez acude más gente?

Carne de cañón. Es triste darse cuenta de que todos lo somos. Vamos a necesitar tocar fondo para tomar la decisión de dar un paso al frente. Las sociedades convertidas en enjambre quizá no ayuden pero no por ello se nos cerrarán todas las puertas. Cada cual puede llevar a cabo una revolución en los límites de su mundo interno. Será ahí donde nos juguemos el futuro. Si conseguimos tomar contacto con nosotros mismos y comprobar que para subsistir hace falta bien poco y que las potencialidades de crecimiento humano que se nos abren son casi infinitas, tal vez tengamos alguna posibilidad.

Si pudiéramos retroceder en el tiempo, ¿sería posible sanar las sociedades e imperios que colapsaron? Con nuestro conocimiento ¿podríamos aportar alguna medicina que evitara la muerte social? 

Todos los seres vivos mueren, las organizaciones e instituciones humanas también. De momento no tenemos forma de detener lo que se ha venido llamando el ciclo de la vida. Lo que sí es posible es aprender del pasado y de la historia, aunque como especie nos suele costar. Es sabido que el ser humano tropieza varias veces con la misma piedra, lo que pone en evidencia nuestra tendencia al olvido y la dificultad para reconocer errores y aprender de ellos. 

En nuestra época podemos decir que la sociedad está aquejada de graves males. El más peligroso es la dificultad para tomar conciencia de que su modo de vida basado en el crecimiento ilimitado amenaza su supervivencia a corto plazo. El siguiente la enorme inercia que tendrá que vencer para acometer cambios una vez que se dé cuenta. 

De alguna u otra forma, la historia nos dice que en muy pocos casos es posible evitar que el Titanic esquive el iceberg. Por eso las épocas de colapso se caracterizan por la inestabilidad, el miedo y la incertidumbre, lo que suele aumentar la percepción de malestar o enfermedad. Nos sentimos enfermos pese a que los indicadores de salud digan lo contrario. Acudimos a los servicios sanitarios ante la más mínima inconveniencia, olvidamos nuestra capacidad de autocuidado y autosanación.

El problema con las enfermedades serias es que suelen requerir medicinas amargas. Plantear un decrecimiento económico global, cambiar el actual sistema basado en combustibles fósiles por otro más ecológico y desescalar el ritmo de vida no va a ser agradable para nadie. Lo cierto es que la alternativa es muchísimo peor. Si seguimos sin hacer ajustes el genocidio a nivel planetario será mayúsculo a corto plazo, nada que ver con la actual pandemia.

Nos va a tocar informarnos, estudiar, reflexionar y dialogar. Para tomar verdadera conciencia de nuestra verdadera posición y fragilidad tendremos que atrevernos a salir de nuestra zona de certeza y atravesar nuestra coraza ideológica para explorar alternativas. No será sencillo en una coyuntura que no facilita ninguna de estas actitudes. Pero si algo hemos aprendido con la crisis del Coronavirus es que podemos adaptarnos y colaborar. Somos capaces de grandes cosas y eso nos debe animar y dar esperanza. Lo vamos a necesitar. 








Healing sick societies.


If we could go back in time, would it be possible to heal the societies and empires that collapsed? With our knowledge, could we provide any medicine that would prevent social death? 



All living beings die, human organizations and institutions die. At the moment we have no way of stopping what has been called the cycle of life. What is possible is to learn from the past and from history, although as a species it is often difficult for us. It is well known that human beings stumble upon the same stone several times, which highlights our tendency to forget and the difficulty of recognizing mistakes and learning from them. 



In our time we can say that society is afflicted by serious ills. The most dangerous is the difficulty in becoming aware that its way of life based on unlimited growth threatens its short-term survival. The problem with serious illnesses is that they often require bitter medicines. Proposing a global economic decline, changing the current system based on fossil fuels for a more ecological one and de-escalating the pace of life is not going to be pleasant for anyone. The truth is that the alternative is much worse. If we continue to fail to make adjustments, genocide on a planetary level will be major in the short term, nothing to do with the current pandemic.




We will have to inform ourselves, study, reflect and dialogue. To become truly aware of our true position and fragility we will have to dare to step outside our zone of certainty and pierce our ideological shell to explore alternatives. It will not be easy in a situation that does not facilitate any of these attitudes. But if we have learned anything from the Coronavirus crisis it is that we can adapt and collaborate. We are capable of great things and that should encourage us and give us hope. We are going to need it.

Queremos ser dioses. We wanna be gods. 我們想成為神。





Más poder, más posibilidades, más deprisa, más objetos, más consumo.

Queremos convertirnos en seres mejorados, amplificados, modificados, tuneados. Usar todo lo que tengamos al alcance para ello, intoxicarnos de tecnología, de información, de petróleo, de ruido. Llegar más lejos, más alto, más rápido. Conseguir dejar atrás al viejo homo sapiens que no era nada más que una versión prescindible, una versión venida a menos desde aquella caída que le sacó a patadas de un olvidado paraíso y lo sumió en el pecado y en la desesperación.

Ahora todo va a cambiar gracias a las promesas de excelsos gurús que nos venden el cielo a precio de costo y nos llenan la vida de móviles de última generación, tecnología 5G, redes sociales donde nuestra capacidad de relación se multiplica y una realidad virtual hecha de series, grupos de WhatsApp y juegos inmersivos. Tener una identidad multiplicada mola casi tanto como una capacidad de desplazamiento alimentada con gasolina. Todos somos productores y consumidores de contenido por lo que se consigue la cuadratura del círculo, hemos devenido en consumidores y producto al mismo tiempo. Lo terrible es que no nos contaron el verdadero precio de toda esta fiesta: convertirnos en esclavos y arrasar el planeta. Además lo hicieron bien porque casi nadie se dio cuenta pese a que nuestras cadenas eran evidentes y pese a que el planeta no dejaba de mandar señales claras. ¿Cómo íbamos a reconocer el engaño y volver a lo de antes? ¿Quién sería el valiente de renunciar a su coche y a su teléfono móvil?

Tampoco nos acordamos de Aquiles, Atalanta, Edipo, Ícaro, Jasón. Hemos olvidado las advertencias de Hector, Paris, Pandora y Prometeo, las historias de Sísifo, Tiresias, Atlas y Casandra. En sus vidas experimentaron la desmesura y pagaron por ello, porque la hibris griega es castigada por los dioses con pena capital. En nuestro tiempo además parecen reinar la impaciencia, la impiedad, la imprudencia y la vanidad, lo que empeora con mucho la situación y condicionan un colapso civilizatorio irremisible.

Algunos lo han visto pero sus voces no serán oídas. Los potentes auriculares que la sociedad lleva puestos lo impiden; además estamos a otras cosas.

We want to become improved, amplified, modified beings. We shall use everything we have within our reach to do so, to become intoxicated with technology, with information, with oil, with noise. To go further, higher, faster. To leave behind the old Homo Sapiens who was nothing more than an expendable version, a version that has been in decline since that old fall that kicked him out of a forgotten paradise and plunged him into sin and despair.

How could we recognize the deception and go back to what we were doing before? Who would be the brave one to give up his car and his mobile phone?

Nor do we remember Achilles, Atalanta, Oedipus, Icarus, Jason. We have forgotten the warnings of Hector, Paris, Pandora and Prometheus, the stories of Sisyphus, Tiresias, Atlas and Cassandra. In their lives they experienced the excess and paid for it, because the Greek hibris is punished by the gods with capital punishment. In our time, moreover, impatience, impiety, recklessness and vanity seem to reign, which makes the situation much worse and conditions an irreversible civilisational collapse.

¿Qué podemos esperar? What can we expect? 我們能期待什麼?





¿Qué podemos esperar?

A medida que pasan los años nos vamos llenando de distopias. Pareciera que puestos a imaginar el futuro solo nos salieran pesadillas. Lo vemos en las artes, en los telediarios, en series y películas y por supuesto en Internet. Esos sueños negros contaminan los nuestros y al final acabamos todos asumiendo que nos dirigimos hacia ellos. 

No quedan voces que hablen de esperanza. No quedan testimonios ni testigos que nos anuncien una buena noticia. Vemos las oscuras nubes del desastre climático acercarse y todo el mundo habla, teoriza y se agita pero nadie hace nada. No hay a quien imitar. Las figuras públicas más progresistas o ecologistas o intelectuales, siguen llevando el máximo tren de vida que pueden permitirse. Muchos de los que no lo somos también. Se habla bastante de decrecimiento pero aquí lo único que decrece es el número de trabajadores que por otro lado tienden a una mayor explotación y sobrecarga en una sociedad cada vez más asimétrica y polarizada. Somos testigos de cómo la sociedad ha sucumbido ante un sencillo virus que parece anunciar los terribles Tsunamis económico-social y climático que vienen detrás.

En las calles el enfado y la crispación aumentan. Se echan culpas al gobierno, a los políticos y a las correspondientes teorías conspiranoicas. Se hacen caceroladas, manifestaciones y demás protestas que aumentan el ruido de fondo de nuestros malogrados sueños. No pinta bien la cosa, nada bien. Y eso nos debería hacer pensar ¿qué podemos esperar? Porque si no ponemos algo de nuestra parte nos encontraremos con una profecía autocumplida. De alguna forma daremos por supuesto el desastre y empujaremos a la sociedad hacia él. 

Viene bien recordar que esperanza tiene que ver con deseo, en el fondo es un anhelo de futuro, una elección, una querencia de un determinado bien o circunstancia. Por eso en el infierno de Dante se la deja fuera, lo peor que en esa época se podía imaginar era un futuro sin salida. Me temo que hoy esto sigue vigente.

También será necesario llamar a las cosas por su nombre. Por fin parece que hemos reconocido que nos enfrentamos a una situación global de colapso social, económico, climático y humano. Un escenario de enorme complejidad que afecta a todo el planeta. Y en las manos llevamos instituciones, herramientas, armazones teóricos y filosóficos de la época industrial. No hemos construido un nuevo cimiento que soporte el gran peso de nuestra avanzada tecnología que permite un nivel de expolio y degradación medioambiental sin precedentes. Ese entramado debería tener mimbres éticos y morales, filosóficos y existenciales. Tendría que ver con los valores, con el bien y el mal, con el sentido de la vida y con la trascendencia. Pero la modernidad eliminó sin miramientos las religiones, que han quedado relegadas en pequeñas minorías, y ha arrinconado el pensamiento y la filosofía en beneficio de una sociedad del espectáculo en la que somos actores y público a la vez.

¿Cómo rescatar la esperanza? Quizá tengamos que desempolvar las tradiciones y los libros que nos hablaban de ella y la larga lista de personas que dieron la vida por defenderla. Una lista que llega a nuestros días pese a que no interese poner el foco en ella. Una lista que forman mujeres y hombres de todos los rincones del planeta que aportaron su pequeña luz y testimonio para que otros veamos. No es necesario inventar nada, basta detenerse un instante y darnos cuenta de que siendo la humanidad tan diminuta en un universo tan enorme, no nos es posible ser referentes de nosotros mismos, hay que buscarlo fuera de nosotros. Y al hacerlo reconocer que somos pura levedad y por lo tanto del todo prescindibles. Tomar consciencia del milagro de la vida debería ser un potente revulsivo que nos impulse hacia modos de vida compasivos que nos hagan un poco más humanos y respetuosos con los demás y con nosotros mismos.

La esperanza en su simplicidad es enormemente contagiosa, basta una gota para disolver un océano de sinsentido y de maldad. Por eso es razonable anunciar que no todo está perdido, tal vez baste con que una diminuta voz se alce y nos diga al oído que aún estamos a tiempo.







What can we expect?

As the years go by, we are filled with dystopias. It seems that when we imagine the future, we only get nightmares. We see it in the arts, in the news, in series and films and of course on the Internet. These black dreams contaminate our own and at last we all end up assuming that we are heading towards them. 

There are no voices left that speak of hope. There are no testimonies or witnesses left to announce good news. We see the dark clouds of climate disaster approaching and everyone is talking, theorizing and shaking but no one is doing anything. There is no one to imitate. The coolest public figures, environmentalists or intellectuals, continue to lead the maximum train of life they can afford. Many of us try to do so. There is a lot of talk about economic downturn but here the only thing that is decreasing is the number of workers who, on the other hand, tend to be more exploited and overburdened in a society that is increasingly asymmetrical and polarized. We are witnesses to how society has succumbed to a simple virus that seems to announce the terrible economic, social and climatic tsunamis that are coming.

In the streets, anger and tension are increasing. The government, politicians and corresponding conspiracy theories are blamed. There are pots and pans, demonstrations and other protests that increase the background noise of our failed dreams. It doesn't look good, not good at all. And that should make us think, what can we expect? Because if we don't do something about it, we'll find ourselves with a self-fulfilling prophecy. Somehow we'll take disaster for granted and push society towards it. 

It is good to remember that hope has to do with desire, at heart it is a longing for the future, a choice, a desire for a certain good or circumstance. That is why in Dante's inferno it is left out, the worst thing that could be imagined at that time was a future without a way out. I am afraid that today this is still true.

It will also be necessary to call things by their name. At last we seem to have recognised that we are facing a global situation of social, economic, climatic and human collapse. A scenario of enormous complexity that affects the entire planet. And in our hands we carry institutions, tools, theoretical and philosophical frameworks of the industrial age. We have not built a new foundation to support the great weight of our advanced technology that allows an unprecedented level of plunder and environmental degradation. This framework should have ethical and moral, philosophical and existential foundations. It would have to do with values, with good and evil, with the meaning of life and with transcendence. But modernity has ruthlessly eliminated religions, which have been relegated to small minorities, and has cornered thought and philosophy in favour of a society of the spectacle in which we are both actors and public.

How can we rescue hope? Perhaps we need to dust off the traditions and books that told us about it and the long list of people who gave their lives to defend it. A list that reaches our days even though it is not interesting to put the focus on it. Tómense un momento y consideren qué es lo más importante para ustedes. Bueno, tal vez necesiten su tiempo. Un servidor precisó años y es un tema que me toca revisar de continuo.

Don't believe anything.

Burocracia y Covid-19 en Madrid





Transcribo un tuit de mi compañero Jose María Molero por su interés:


El procedimiento de @SaludMadrid de detección precoz y diagnóstico de nuevos casos de infección #COVIDー19 en AP, ante un caso confirmado parece ciencia ficción pero no lo es:


  1. Hay que crear 2 episodios en la historia clínica electrónica,
  2. Cumplimentar 3 protocolos,
  3. Cambiar el código CIE 10 para la gestión de la Incapacidad Transitoria
  4. Hacer test PCR en menos de 24 horas,
  5. Comunicar el resultado,
  6. Hacer seguimiento telefónico del caso,
  7. Identificar contactos sospechosos,
  8. Tramitar las bajas de los contactos si son trabajadores,
  9. Realizar PCR a los contactos si tienen síntomas
  10. y si es positiva actuar con ellos como nuevos casos confirmados (volver al paso 1)...


Además de seguir con la consulta, urgencias, domicilios...



Como sean muchos vamos dados... de momento no hemos recibido en Atención Primaria de Madrid ningún refuerzo conocido, ni médicos rastreadores ni nada... sigo confiando en que vengan pero...

El rescate de la normalidad

Todos queremos que nos devuelvan nuestra vida normal, o por lo menos lo que de normal podía tener. Nos dicen que no va a ser posible, que nos darán una nueva y eso me hace recelar. Parece que lo que viene será una versión devaluada con menos ingresos y más restricciones. 

Nos tocará arrimar el hombro para conseguir que la vida sea vida y la dosis de miedo y crispación social se reduzca a niveles tolerables. Como médico me atrevo a proponer algunas sugerencias.

Estar vivo no es normal. Si uno mira un instante el universo se dará cuenta de que la norma es precisamente lo contrario. El problema que tenemos en nuestra sociedad es que nos sentimos eternos e invulnerables algo que, por muy impopular que sea, debemos asumir como falso. Hemos convertido en un tabú la levedad humana, su impermanencia. Por eso quitamos la muerte de la vista y escondemos todo lo posible la enfermedad, la pérdida y las catástrofes personales. Es verdad que algunos las airean, pero lo habitual es avergonzarse de ellas y beber su hiel en solitario. 

Al ser pues la vida un regalo extraordinario lo lógico sería agradecerla y vivirla con plena conciencia. Esto lo saben bien los que han resucitado de una enfermedad grave o han sobrevivido a un accidente. Se dan cuenta de lo frágil que es permanecer vivos y lo poco que hace falta para sonreír y recorrer el día. Los demás parecemos haberlo olvidado y nos desplazamos a toda velocidad por existencias agobiadas que dedicamos a hacer demasiadas cosas, consumiendo todo lo que podemos y terminando consumidos. Son los engaños del progreso y el crecimiento económico, la rueda de la jaula donde el pequeño roedor da vueltas sin parar. 

Salir de una rueda, y mucho menos de una jaula, nunca fue sencillo. Pero es posible cuando uno se da cuenta de que ni la una ni la otra son reales. Lo real es otra cosa, la fuerza del sol, la generosidad de la tierra, la belleza de un paisaje, la solidez de un abrazo a alguien cercano. Momentos que normalmente no se pueden comprar ni vender y están ahí mismo esperando que nos demos cuenta. 

Para alcanzar pues esa normalidad nos será necesario pensar un poco lo que de verdad significa la vida para nosotros. Y de esta forma hacer un repaso de lo que consideramos importante, algo que suele estar bastante mediatizado por una sociedad de mercado que se dedica a vendernos cosas desde que sale el sol hasta el ocaso. 

El siguiente paso es salir a la calle, dejar por un momento la seguridad de nuestra casa y abrirnos al espectáculo que hay fuera. Para ello necesitaremos una buena dosis de prudencia. Seguimos en estado de epidemia y tendremos que protegernos y proteger a los demás, fundamentalmente a los más débiles, y esto no debería ser mediatizado por eslóganes políticos ni intereses económicos o de otra índole. Se trata de tener la suficiente humanidad como para ir más allá de lo que nuestro ombligo nos dicte. La buena noticia es que es sencillo, bastará usar mascarillas en los espacios públicos, con especial cuidado en el transporte y los lugares más frecuentados. También necesitaremos guardar cierta distancia unos con otros, seguir estornudando en la flexura del codo o en un pañuelo y evitar en lo posible las aglomeraciones. 

Será inevitable que nos reunamos y regresemos a los bares y lugares públicos. Hagámoslo con alegría y con prudencia, de la mejor forma posible, valorando el posible riesgo de cada situación. No es lo mismo un encuentro casual en la calle con dos metros de distancia que una pequeña fiesta en un apartamento que reúna a diez amigos. A lo mejor nos toca hacer las primeras fiestas con menos invitados o plantearlas en algún lugar al aire libre. 

Otro factor a comentar es la toxicidad de los medios de comunicación y las redes sociales en el marco de la actual coyuntura de miedo y crispación. Todos estamos nerviosos, los políticos más si cabe, y el espectáculo mediático y en Internet es terrorífico. Cada cual se alinea con sus amigos afines y carga sin piedad contra quien piensa distinto. Las palabras escucha, diálogo y compasión han sido secuestradas. Por ello solicito cierto ayuno mediático. Usen las dosis mínimas que se puedan permitir, y créanme si les digo que evitarán una peligrosa intoxicación que empieza lentamente pero termina pudriendo el ánimo con agresividad, cabreo y desazón. Tal vez sea interesante dejar de escribir improperios en Twitter y rescatar ese cuaderno donde alguna vez escribimos algo o quizá arriesgarnos a mandar una carta a esa persona que la espera. Rescatar el poder sanador de la palabra escrita puede hacer mucho bien a mucha gente. 

Habrá que volver a poner en valor la comunicación de calidad y el contacto físico que tanto necesitamos. Habrá que aprender de nuevo a abrazar, besar, tomar de la mano. Rescatar el tesoro de no sabernos solos, de poder interesarnos por otros y tratar de ayudar, de tender puentes, de cuidar, de hacer algo por otros. 

La sociedad está herida de miedo y frustración, indigesta por haber perdido libertades y haberse acercado a la incómoda consciencia de su fragilidad. Para curar estas heridas nos harán falta potentes medicinas como la consciencia, la compasión y la delicadeza que nos llevarán a tomar decisiones y acometer cambios necesarios. Dejar de hacer lo que no nos aporte vida y atrevernos a soltar aquello que nos encadene sin aportarnos verdadero valor. 

Tengo la profunda sensación de que saldremos de esta y con esa pequeña gota de esperanza me gustaría terminar. Las esperas nunca fueron agradables pero cuando se sabe que el sol está al otro lado de la tormenta suele ser más fácil aguantar el chubasco. 









Towards normality


We all want our normal life back, or at least the closest thing to what it was. They tell us it's not going to be possible, that they'll give us a new one, and that makes me suspicious. It seems that what is coming is a devalued version with less income and more restrictions. 

It will be our turn to pitch in to make life a living one, and the dose of fear and social tension will be reduced to tolerable levels. As a doctor I dare to propose some suggestions.

Being alive is not normal. If one looks at the universe for a moment, one will realize that the norm is precisely the opposite. The problem we have in our society is that we feel eternal and invulnerable to something that, however unpopular it may be, we must assume to be false. We have made human lightness, its impermanence, a taboo. That is why we remove death from sight and hide illness, loss and personal catastrophes as much as possible. It is true that some show them in public, but the usual thing is to be ashamed of them and drink their gall alone. 

Since life is an extraordinary gift, it is logical to be grateful for it and to live it with full awareness. This is well known to those who have risen from a serious illness or have survived an accident. They realize how fragile it is to remain alive and how little it takes to smile and go through the day. The rest of us seem to have forgotten and are rushing through overburdened existences, doing too much, consuming all that we can and ending up consumed. They are the tricks of progress and economic growth, the wheel of the cage where the little rodent spins around endlessly. 

Getting out of a wheel, let alone a cage, was never easy. But it is possible when one realizes that neither one nor the other are real. What is real is something else, the strength of the sun, the generosity of the earth, the beauty of a landscape, the solidity of a hug to someone close. Moments that can't normally be bought or sold and are right there waiting for us to notice. 

In order to reach this normality, it will be necessary for us to think a little about what life really means to us. And in this way we can review what we consider important, something that is usually quite mediatized by a market society that is dedicated to selling us stuff from sunrise to sunset. 

The next step is to go out into the street, leave the security of our home for a moment and open ourselves up to the spectacle outside. To do this we will need a good dose of prudence. We are still in a state of epidemic and we will have to protect ourselves and others, fundamentally the weakest, and this should not be mediated by political slogans or economic or other interests. It is about having enough humanity to go beyond what our selfish dictates. The good news is that it is simple, it will be enough to wear masks in public spaces, with special care in transport and the most frequented places. We will also need to keep some distance from each other, continue sneezing into the bend of the elbow or into a handkerchief and avoid crowding as much as possible. 

It will be inevitable that we will meet and return to the pubs and public places. Let us do so with joy and prudence, in the best possible way, assessing the possible risk of each situation. A casual meeting in the street two metres away is not the same as a small party in an apartment with ten friends. Maybe we have to organize the first parties with fewer guests or have them somewhere in the open air. 

Another factor to comment on is the toxicity of the media and social networks in the current climate of fear and tension. We are all nervous, politicians even more so, and the media and Internet spectacle is terrifying. Everyone is aligned with their like-minded friends and ruthlessly charges those who think differently. Words as listening, dialogue and compassion have been hijacked. That is why I call for a certain media fasting. Use the minimum doses you can afford, and believe me when I tell you that you will avoid a dangerous intoxication that starts slowly but ends up rotting your mood with aggression, anger and uneasiness. It might be interesting to stop writing expletives on Twitter and rescue that notebook where we once wrote something, or perhaps risk sending a letter to that person who is waiting a word from us. Rescuing the healing power of the written word can do a lot of good for many people. 

We will have to re-evaluate the quality of communication and physical contact that we so desperately need. We will have to relearn how to hug, kiss, and hold hands. We will have to rescue the treasure of be living in society, of being able to take an interest in others and try to help, the posibility of buildIng bridges among us, to care, to do something for others. 

Society is wounded by fear, frustration and lost freedoms and approached the uncomfortable awareness of its fragility. To heal these wounds we will need powerful medicines such as awareness, compassion and gentleness that will lead us to make decisions and undertake necessary changes. To stop doing what does not bring us life and to dare to let go of that which chains us without bringing us true value. 

I have a deep feeling that we will get out of this and with that small drop of hope I would like to finish. Waiting has never been pleasant, but when you know that the sun is on the other side of the storm, it is usually easier to endure the rain.
