Ciegos al cambio. Blind to change. 盲目改變





El futuro está velado a los mortales. Hay un abismo negro que impide su visión. La desgracia vence sin embargo cuando esta ceguera invade el presente continuo y los seres humanos pierden la facultad de discriminar y de entender.

Un tiempo de bonanza puede trocar rápidamente en uno de catástrofe cuando la bruma de la indecisión, el bloqueo y la falta de criterio llena con sus paños opacos la intemperie. Y es la mala noticia que tenía que anunciarles: eso de ahí fuera es niebla densa. 

La paradoja es que algunos sí pueden ver a través de esa nube, concretamente los chino parlantes.

La manera más sencilla de explicarlo tiene que ver con los verbos ya que son ellos los responsables de expresar la acción en relación al tiempo. En los idiomas europeos los verbos se conjugan para adaptarse a una amplia gama de tiempos verbales, eso permite que podamos situar la acción en presente, pasado, futuro, hacerla condicional o distinguir si es simple o continua. En chino el verbo solo existe en infinitivo, no se conjuga. El tiempo verbal lo da el contexto. 

Lo que estamos viviendo en nuestro momento histórico es un cambio de contexto. Los valores están cambiando al ritmo veloz y líquido que marcan las ideas y tendencias lo que está amenazando y hundiendo instituciones y estructuras solidas tradicionales. 

Por su lado China ha evolucionado a gran velocidad adaptándose a un mundo complejo y cambiante. Ha sabido mantener sus estructuras sociales básicas y añadir las que le ha parecido bien del exterior. La coexistencia de un sistema político firme con limitación de derechos civiles y una economía basada en capitalismo de estado, han permitido su elevación al puesto de primera potencia mundial. 

Desde Europa seguimos sin entender cómo nuestra posición histórica de supremacía sigue devaluándose mientras la industria se desmantela, los mejores profesionales emigran, la población disminuye y la economía se hunde.   Asistimos con pavor al rápido deterioro del antiguo imperio americano al que considerábamos hermano y miramos con suspicacia la elevación del nuevo imperio chino que ha conseguido inundar nuestros mercados de productos baratos al enorme precio de haber fagocitado incontables puestos de trabajo y dejado al mínimo la estructura industrial. 

De nada nos sirven nuestro superpoderes gramaticales si por mucho que podamos conjugar nuestros verbos nos movemos por contextos oscuros que no aportan ninguna claridad. 

La única respuesta de calado que ha dado Estados Unidos es la transformación del capitalismo tradicional en un capitalismo de gigantes, permitiendo la emergencia de megacorporaciones monopolísticas que concentran una gran masa de poder. Estos gigantes tratan a los trabajadores como perros de paja y destruyen todo el entramado de pequeñas y medianas empresas de su sector y de los aledaños. Al final crecen ofreciendo competitividad al precio de eliminar derechos sociales y destruir empleo y entramado social. 

El ciudadano europeo vive pues en la mitad de un espejismo de supermercados llenos y gran bonanza de bienes y servicios mientras las clases medias adelgazan y crecen las colas de la asistencia social. 

La confusión, los gritos y las quejas sirven de poco. El ruido de fondo que crean medios de comunicación y redes sociales no favorece ni el silencio, ni la reflexión, ni la toma de conciencia. Queda algún intelectual pero su voz no llega lejos en medio de tanta algarabía, máxime cuando las crisis sociales se suceden y el miedo se extiende cada vez más. 

Lo más revolucionario en este momento es algo tan simple como abrir los ojos. Ver que todo está cambiando según la antigua ley que permite que el más poderoso haga valer su voluntad. Entender que necesitaremos mucha creatividad para adaptarnos. Confiar en que podremos aportar el valor que los tiempos exigen. 





Blind to change

The future is veiled from mortals. There is a black abyss that prevents their vision. But misfortune overcomes when this blindness invades the present continuous and human beings lose the faculty to discriminate and understand.

A time of bonanza can quickly turn into a time of catastrophe when the mist of indecision, blockage and lack of judgment fills the open air with its opaque cloths. And this is the bad news that I had to announce to you: this is a dense fog outside. 

The paradox is that some can see through that cloud, specifically the Chinese speakers.

The simplest way to explain it has to do with verbs, since they are responsible for expressing the action in relation to time. In European languages, verbs are conjugated to adapt to a wide range of verb tenses, which allows us to situate the action in the present, past, future, make it conditional or distinguish whether it is simple or continuous. In Chinese the verb only exists in the infinitive, it is not conjugated. The verb tense is given by the context. 

What we are experiencing in our historical moment is a change of context. Values are changing at the rapid and liquid pace set by ideas and trends, which is threatening and sinking traditional institutions and solid structures. 

For its part, China has evolved at great speed, adapting to a complex and changing world. It has known how to maintain its basic social structures and add those that it has found good from the outside. The coexistence of a strong political system with limited civil rights and an economy based on state capitalism has allowed it to rise to the position of world power. 

From Europe we still do not understand how our historical position of supremacy continues to devalue as industry is dismantled, the best professionals emigrate, the population declines and the economy collapses.   We witness with dread the rapid deterioration of the old American empire that we considered our brother, and we look with suspicion at the rise of the new Chinese empire that has managed to flood our markets with cheap products at the enormous price of having phagocytized countless jobs and left the industrial structure at a minimum. 

Our super grammatical powers are of no use to us if, however much we can conjugate our verbs, we move around in dark contexts that bring no clarity. 

The only significant response that the United States has given is the transformation of traditional capitalism into a capitalism of giants, allowing the emergence of monopolistic mega-corporations that concentrate a large mass of power. These giants treat the workers like “straw dogs” and destroy the whole network of small and medium-sized enterprises in their sector and those around them. In the end they grow by offering competitiveness at the price of eliminating social rights and destroying jobs and the social fabric. 

The European citizen lives then in the middle of a mirage of full supermarkets and great bonanza of goods and services while the middle classes are thinning and the queues of social assistance are growing. 

The confusion, the shouting and the complaints are of little use. The background noise created by the media and social networks does not encourage silence, reflection or awareness. There are still some intellectuals, but their voices do not reach far in the midst of so much noise, especially when social crises follow one another and fear spreads more and more. 

The most revolutionary thing at this moment is something as simple as opening one's eyes. To see that everything is changing according to the old law that allows the most powerful to enforce their will. Understanding that we will need a lot of creativity to adapt. To trust that we will be able to provide the value that the times demand.

Navegar la incertidumbre. Navigating uncertainty. 應對不確定性

La verdadera pandemia se llama incertidumbre y tiene que ver con la falta de seguridad ante la posibilidad de enfermar, de insuficiente confianza en las instituciones y responsables y de poca certeza ante un futuro impredecible que provoca gran inquietud. 

Es una situación de crisis social similar a las de otras épocas con la peculiaridad de que en esta ocasión es global y audiovisual, lo que aumenta la sincronía y el contagio emocional. Muchos de los que la vivimos la experimentamos como algo novedoso, intenso e inédito, lo que aumenta más la percepción de gravedad y agobio.

Las circunstancias de las pandemias epidémicas suelen hacer que estas sean muy difícilmente controlables. Normalmente se requiere mucho tiempo para que las aguas vuelvan a su cauce. Esto es muy difícil de asumir para una sociedad infantilizada acostumbrada a conseguir lo que desea con inmediatez. En esta ocasión el problema no se va a poder arreglar únicamente poniendo dinero encima de la mesa. Nos las estamos viendo con un fenómeno natural y a la naturaleza le importan un bledo nuestras finanzas.

Si observamos el nivel de gobernanza de los distintos territorios veremos que pese a las consabidas excepciones se ha generalizado el malestar ante una evidente falta de liderazgo. Lo habitual es que los gestores políticos señalen a otros como culpables de la situación y nadie asuma responsabilidad alguna ni actúe en consecuencia. Esto suele enfadar a la ciudadanía que termina irritada tanto si se toman medidas, que suelen ser impopulares, como si no se toman, lo que se percibe como dejadez. 

Los cursos de acción que puede tomar el político son principalmente coercitivos: limitación de derechos, movilidad, escolarización... Y muy limitados a la hora de promover la responsabilidad del ciudadano y las conductas cívicas, que lógicamente dependen de cada cual. Tampoco hay gran capacidad de trasvasar recursos de una institución a otra como por ejemplo reforzar la sobrecargada sanidad con recursos de protección civil, ayuntamientos u otros trabajadores cuando estos estuvieron confinados. 

La complejidad de la situación obliga a rescatar a nivel personal y comunitario dos importantes valores como son la creatividad y la flexibilidad. Será necesarios hacer cambios y ajustes en nuestra conducta personal y familiar a la hora de relacionarnos con los espacios públicos y con los demás. Variar costumbres, evitando algunas y rescatando o reforzando otras. Aprender modos sociales seguros y adaptarnos a tener menos interacción social presencial. 

A nivel comunitario será fundamental potenciar las redes de cuidado y protección social así como la sanitaria. Toda asociación de personas formal o informal puede  aportar valor, desde ONGs, parroquias/Cáritas, asociaciones de vecinos, clubes deportivos, grupos culturales, etc... poniendo el foco en los más vulnerables, excluidos, mayores y enfermos. La coordinación de estos agentes con los Servicios Sociales de zona sería muy deseable, lo que implicará mejorar los puentes de comunicación.

Será necesario dar respuestas a la formación académica de los niños que tal vez no pueda ser del todo escolarizada y deba manejarse familiarmente o en pequeños grupos de clases particulares. También a la asistencia sanitaria que claramente no podrá recaer únicamente en los recursos limitados del barrio o pueblo que tal vez puedan verse reforzados por algún tipo de voluntariado, o medios del ayuntamiento, protección civil u otros recursos. Y sobre todo tomando conciencia de que lo correcto es solicitar asistencia sanitaria únicamente cuando sea imprescindible. 

Todos estos cambios requerirán la implicación de grandes mayorías, lo que no será fácil sin la participación de los líderes naturales de los distintos ámbitos y territorios. Un liderazgo basado en el ejemplo y el servicio. De poco nos van a servir ahora la queja y la protesta cuando estamos navegando dentro de la tormenta. Es necesario cambiar la actitud y recordar que precisamente ahora es cuando más necesidad tiene la sociedad de sus servidores públicos sean estos limpiadores, profesores, agentes de seguridad, sanitarios, funcionarios, gestores o políticos. Incluyendo también a todos los que desde el ámbito privado pueden aportar valor social, poniendo temporalmente este por encima del beneficio económico.

Aportar servicio y cuidado a los demás es el camino más seguro para disminuir la incertidumbre de este tiempo. Cada cual deberá hacer una reflexión personal que le ayude a situarse en este contexto para que en lugar de permanecer inmóviles dentro de un confinamiento impuesto nos atrevamos a convertirnos en agentes que velan y trabajan por el bien de otros. 

Es fundamental aceptar que de este embrollo no nos va a sacar el estado ni ningún salvador externo, saldremos todos cuando se cumpla el tiempo que la naturaleza imponga. Lo que sí está en nuestra mano es el cómo. Lo podremos hacer incendiados en miedo o en ira ante el naufragio o remangados y con las manos metidas en faena mientras achicamos el agua de las muchas grietas que tiene el casco de nuestras sociedades. 




Navigating uncertainty


The real pandemic is called uncertainty, and it has to do with a lack of security in the face of the possibility of falling ill, insufficient trust in institutions and officials, and little certainty in the face of an unpredictable future that causes great concern. 

It is a situation of social crisis similar to those of other times with the peculiarity that on this occasion it is global and audiovisual, which increases synchrony and emotional contagion. Many of us are feeling it as something new, intense and unprecedented, which further increases the perception of gravity and distress.

The circumstances of epidemic pandemics often make them very difficult to control. It usually takes a long time for the waters to return to their course. We are dealing with a natural phenomenon and nature doesn't give a damn about our finances.

It is common for political managers to point to others as the culprits of the situation and no one takes responsibility or acts accordingly. This often angers citizens who end up being irritated whether action is taken, which is often unpopular, or not taken, which is perceived as neglect. 

The courses of action that the politician can take are mainly coercive: limitation of rights, mobility, schooling And very limited when it comes to promoting citizen responsibility and civic behaviour, which logically depend on each individual. Nor is there much capacity to transfer resources from one institution to another, such as reinforcing overburdened health care with resources from civil protection, town councils or other workers when these were confined. 

The complexity of the situation means that two important values, creativity and flexibility, have to be rescued at personal and community level. It will be necessary to make changes and adjustments in our personal and family behaviour when relating to public spaces and others. Varying customs, avoiding some and rescuing or reinforcing others. Learn safe social modes and adapt to having less face-to-face social interaction. 

At the community level it will be fundamental to strengthen the networks of care and social protection as well as health. Any formal or informal association of people can provide value, from NGOs, parishes/charitable organizations, neighborhood associations, sports clubs, cultural groups, etc... focusing on the most vulnerable, excluded, elderly and sick. The coordination of these agents with the Social Services of the area would be very desirable, which will imply improving new bridges of communication.

It will be necessary to respond to the academic training of children which may not be entirely schoolable and must be handled by families or in small groups of private or voluntary classes. Also to health care, which clearly cannot fall solely on the limited resources of the neighbourhood or village, which can perhaps be reinforced by some kind of voluntary work, or means from the town hall, civil protection or other resources. And above all, being aware that the right thing to do is to ask for health care only when it is essential. 

All these changes will require the involvement of large majorities, which will not be easy without the participation of the natural leaders of the different areas and territories. A leadership based on example and service. Complaint and protest will be of little use to us now when we are sailing into the storm. We must change our attitude and remember that it is precisely now that society most needs its public servants, whether they are cleaners, teachers, security agents, health workers, civil servants, managers or politicians. Providing service and care to others is the surest way to reduce the uncertainty of this time. Each one should make a personal reflection that helps him/her to situate him/herself in this context so that instead of remaining immobile within an imposed confinement we dare to become agents that watch over and work for the good of others. 

It is fundamental to accept that neither the state nor any external saviour will get us out of this mess; we will all get out when the time that nature imposes is over. What we can do is how. We can do it in fear or in anger at the shipwreck or with our sleeves rolled up and our hands in the water as we shrink the water from the many cracks in the hull of our societies.

Decálogo de supervivencia para profesionales sanitarios de Atención Primaria en emergencia sanitaria. 


Entramos en Emergencia Sanitaria, eso implica fuerte marejada con aumento de presión asistencial clínica y burocrática en entorno epidémico con plantillas en muchos casos mermadas y agotadas. 

No es esperable recibir ni refuerzos ni ayuda significativa de la superioridad lo que implica la necesidad de autogestionarse y coordinarse lo mejor posible dentro del equipo y con la comunidad. La flexibilidad y la creatividad serán imprescindibles para poder adaptarnos a las circunstancias cambiantes.

Es verdad que la incertidumbre no hace fácil el trabajo para nadie pero también lo es que nuestra formación y el conocimiento de nuestras comunidades nos hacen referencia y servicio esencial para estas.

En momentos de crisis viene bien recordar que añadir un toque de esperanza y buen humor hacen la vida más fácil y que cosas sencillas como una sonrisa o un gesto amable tienen mucha fuerza. 




1. Lo peor de la epidemia ha pasado. Paradójicamente son esperables rebrotes con más bajas laborales tanto de población como de sanitarios por la mayor capacidad actual de hacer pruebas diagnósticas. Si tu equipo y tu comunidad entra de nuevo en emergencia sanitaria va a ser fundamental volver a adaptar el servicio, como ya se ha hecho antes, preveyendo contar con menos sanitarios.  

2. No estás solo, trabajas en equipo. Apóyate en tus compañeros, comunidad y resto de niveles asistenciales para dar la mejor respuesta dentro de lo posible pero no más. 

3. Si te sientes sobrepasado tienes el derecho y la obligación de decirlo y visibilizarlo. Desahogarte con tu círculo personal no está reñido con pedir ayuda a tus compañeros de trabajo y responsables sanitarios.

4. Cuídate. Eso implica trabajo seguro a un ritmo que no te sea extenuante y respetar horario siempre que sea posible. Estamos en una carrera de fondo, los descansos son fundamentales y durante los mismos es básico desconectar. Dosifica las malas noticias y la presencia en redes sociales.

5. Cuando sea necesario hay que decir que NO. A gestores que solicitan burocracia o cursos de acción no prioritarios y a pacientes que solicitan cuestiones no esenciales. También a uno mismo, no te exijas más de lo prudente. 

6. No será posible atender todas las peticiones de servicio que nos hagan, será imprescindible priorizar atendiendo a criterios clínicos y éticos. Primero los pacientes y luego la burocracia.

7. Mientras sea posible hay que atender a todo paciente que llega por la puerta discriminando si su petición es administrativa o clínica y demorable o no.

8. Por mucho que uno lo desee nadie tiene superpoderes. Es inviable atender de forma continua listas de pacientes mayores de 50 al día. Si tu lista de pacientes es muy superior a lo normal, o a la de otros compañeros, y no puedes con ella pide ayuda. 

9. En estado de emergencia sanitaria es fundamental mantener abierta la comunicación con la comunidad (ayuntamiento, farmacias comunitarias, servicios sociales, residencias o colectividades, colegios, asociaciones, policía local...). Esa responsabilidad puede delegarse en alguien del equipo o mejor ser asumida por todos. 

10. En condiciones extraordinarias, si las ausencias de personal no permiten un servicio normal cada director de centro de salud debería tener la potestad de modificar el horario de asistencia presencial (cerrando en horario de tarde por ejemplo) o si la situación es insostenible transfiriendo el servicio presencial o total a otro centro cercano. Es necesario recordar que la responsabilidad de dimensionar correctamente la plantilla y reforzarla si corresponde es de la Consejería de Sanidad.







5 Propuestas prácticas para gestores sanitarios


1. Reducir la burocracia (declaración jurada de enfermedad para procesos menores de 14 días, no necesidad de imprimir partes de confirmación, prorroga de renovación de medicación hasta superar emergencia sanitaria...).

2. Favorecer teleasistencia. Uso de correo electrónico con pacientes y Whatsapp (usando teléfonos móviles del centro de salud, pidiendo consentimiento verbal al paciente, introduciendo teléfono del paciente en el móvil, sin nombre, compartiendo imagen, vídeoconferencia, radiografías…). 
Potenciar líneas telefónicas y número de administrativos que atienden teléfono. 
Permitir teletrabajo (acceso remoto al propio ordenador de consulta para los profesionales que no puedan hacer asistencia directa por enfermedad o factores de riesgo así como para los que tengan que hacer cuarentena domiciliaria estando asintomáticos y quieran trabajar voluntariamente).

3. Seguridad jurídica.

Administrativos. Permitir que puedan preguntar si la consulta es administrativa (partes de baja, recetas...) o no, y si es demorable o no. Preguntar  qué motivo de consulta desea el paciente que le figure al profesional sanitario en su ordenador. 

Enfermería. Permitir el triaje enfermero a la puerta del centro. Permitir gestión de la demanda de problemas de salud sencillos tratables con autocuidados y medicación OTC. 

Médicos. Permitir teleasistencia. Permitir priorizar asistencia antes que burocracia.

4. Realización de PCR por circuitos externos a la plantilla de Atención Primaria (carpas, acceso de pacientes en coche, envío telemático de resultado al paciente).

5. Plan de contingencia ante el posible aumento de infecciones en sanitarios y aumento de bajas laborales. 







Decalogue of survival for health professionals of Primary Care in Covid19 emergency. 


We are entering a Healthcare Emergency, which implies a strong swell with increased clinical and bureaucratic care pressure in an epidemic environment, with reduced and exhausted templates in many cases. 

It is not expected to receive either reinforcements or significant help from the superiority which implies the need to self-manage and coordinate as best as possible within the team and with the community. Flexibility and creativity will be essential in order to adapt to changing circumstances.

It is true that uncertainty does not make the work easy for anyone, but it is also true that our formation and the knowledge of our communities make us a reference and an essential service for them.

In times of crisis it is good to remember that adding a touch of hope and good humour makes life easier and that simple things like a smile or a kind gesture have a lot of strength. 


1. The worst of the epidemic is over. Paradoxically, we can expect a resurgence of the epidemic, with more people and health workers infected  because of the greater capacity to carry out diagnostic tests. If your team and your community enter into a health emergency again, it will be essential to adapt the service, as has been done before, knowing more professionals will be on medical  léase.  

2. You are not alone, you work as a team. Support your colleagues, community and other levels of care to give the best possible response, but no more. 

If you feel overwhelmed, you have the right and the obligation to say so and make it visible. It is not difficult to get rid of your personal circle by asking for help from your colleagues and health managers.

4. Take care of yourself. This means working safely at a pace that is not strenuous and keeping to your schedule whenever possible. We are in a long-distance race, breaks are essential and during these breaks it is essential to disconnect. Dose the bad news and the presence in social networks.

5. Whenever necessary, say NO. To managers who request bureaucracy or non-priority courses of action and to patients who request non-essential issues. Also to oneself, do not demand more of yourself than is prudent. 

6. It will not be possible to attend to all the service requests made to us, it will be essential to prioritise according to clinical and ethical criteria. First the patients and then the bureaucracy.

7. As long as it is possible, we must attend to every patient who arrives through the door, discriminating whether their request is administrative or clinical and whether it can be delayed or not.

8. No matter how much one wants to, no one has super powers. It is unfeasible to attend to lists of patients over 50 a day on a continuous basis. If your patient list is much higher than normal, or that of other colleagues, and you can't handle it, ask for help. 

9. In a state of health emergency, it is essential to maintain open communication with the community (town hall, community pharmacies, social services, residences or communities, schools, associations, local police...). This responsibility can be delegated to someone in the team or better assumed by all. 

10. Under extraordinary conditions, if staff absences do not allow for a normal service, each health centre director should have the power to modify the schedule of attendance (closing in the afternoon, for example) or if the situation is unsustainable, transferring the attendance or total service to another nearby centre. It is necessary to remember that the responsibility for correctly sizing the staff and reinforcing it if necessary belongs to the Regional Ministry of Health.


Practical proposals for health managers


1. Reduce bureaucracy (affidavit of illness for processes of less than 14 days, no need to print confirmation reports, extension of medication renewal until the health emergency is overcome...).

2. Encourage remote assistance. Use of email with patients and Whatsapp (using mobile phones in the health centre, asking for the patient's verbal consent, inserting the patient's phone in the mobile, without name, sharing image, video conference, x-rays...). 
To promote telephone lines and the number of administrative staff who attend to the telephone. 

3. Legal security.

Administrative staff. Allow them to ask if the consultation is administrative (discharge reports, prescriptions...) or not, and if it is delayable or not. Ask what reason to consult the patient wants to write down on the health professional agenda. 

Nursing. Allow the nurse triage at the door of the centre. Allow demand management of simple health problems that can be treated with self-care and OTC medication. 

Physicians. Allow for remote assistance. Allow prioritizing care over bureaucracy.

4. Carrying out of PCR by external circuits to the Primary Care staff (tents, patient access by car, telematic sending of results to the patient).

5. Contingency plan in the event of a possible increase in the number of infections in health care facilities and an increase in the number of sick leaves.

Cambiar de actitud, cambiar de narrativa. Change of attitude, change of narrative. 態度改變,敘述改變。

Hace pocos días recibí un zasca en Twitter por usar una metáfora que daba lugar a una mala interpretación. Por mucho que queramos el lenguaje no es totalmente controlable, dado que toda comunicación tiene una parte que depende del emisor y otra del receptor. Ese metamensaje nos suele meter en líos excepto en matemáticas donde no ha lugar. 

En cualquier caso la situación me ayudó a reflexionar sobre mis metáforas y narrativas que últimamente tienen un cariz de derrumbe. Es verdad que este año está siendo aciago para los profesionales sanitarios al enfrentarnos a una situación nueva que ha venido acompañada de intensas emociones. 

Primero fueron la sorpresa y el miedo. Durante meses nos enfrentamos a una enfermedad infecciosa sin medidas de protección, sin pruebas diagnósticas y sin tratamiento. Luego vino el enfado tras mucho tiempo de gestión errática y falta de liderazgo sanitario. Finalmente el cansancio y la desesperanza al comprobar que volvemos a la casilla de salida habiendo aparentemente aprendido poco. 

Por estas razones he usado en muchas ocasiones la imagen del barco hundiéndose, narrativa que llevo compartiendo años junto a otros muchos compañeros. En Atención Primaria llevamos décadas sufriendo limitación severa de la inversión y una gestión bastante discutible que ha ido empeorando año tras año las condiciones del servicio. Da la sensación de que un tejido tan dañado termina por romper las pocas costuras que le quedan cuando uno es testigo de barbaridades como las que está pasando este año en el centro de salud de el Álamo, en Madrid donde se ha permitido que dos o tres médicas atiendan siete consultas a la vez. 

Hoy me levanté con una nueva imagen que tal vez pueda sustituir a la anterior. La de un barco de vela de dos palos navegando con fuerte marejada. Y tal vez nos pueda ser más operativa. Es verdad que la organización de un equipo de Atención Primaria se asemeja en parte a una tripulación con distintos roles, también que tanto el barco tiene sus problemas estructurales y la marinería es algo corta. Pero la tormenta es la que hay, la peor que hayamos visto en mucho tiempo, entramos en mar gruesa. 

Por eso la queja y la denuncia sirven de poco cuando las olas son del tamaño de una casa. No queda más remedio que hacer todo lo que esté en nuestra mano y, como bien saben los marinos, rezar quien pueda. Para ello la palabra clave es PRIORIZAR. Cada barco de esta armada sanitaria tendrá que hacer lo propio sin demorarse a esperar instrucciones del alto almirantazgo que llegan mal y tarde. 





Necesitamos priorizar:


  1. El cambio de actitud y narrativas.
  2. La seguridad de los pacientes.
  3. La seguridad del personal. 
  4. La atención de los más vulnerables o graves.
  5. La atención de la patología urgente severa.



Y todo lo demás deberá ser secundario. Todo no lo podemos abarcar, máxime en un escenario de verano con las plantillas al 50%, muchos profesionales agotados y una perspectiva de empeoramiento de la situación epidemiológica. 

Por otra parte no creo que haya que abandonar:


  1. La progresiva toma de conciencia de nuestra situación.
  2. La comunicación y coordinación entre profesionales y sociedad.
  3. La visibilización y denuncia de la situación ante nuestros responsables y ante la sociedad.
  4. La vía legal, cuando se crucen líneas rojas.
  5. La vía sindical, con acciones de alguna contundencia.


Ayer vivimos un día de fuertes tormentas en mi zona. Las recibí con alegría y creo que me han ayudado a ponerme en mi sitio. Creo que es bueno, sin querer nos descolocamos o permitimos que la vida nos descoloque. Hoy, con un día fresco con la tierra aún mojada, parece que la esperanza es una posibilidad, pero que no nos pille con los brazos cruzados. 






Change of attitude, change of narrative.



A few days ago I got a buzz on Twitter for using a metaphor that led to misinterpretation. As much as we want language, it is not totally controllable, since all communication has a part that depends on the sender and another part on the receiver. That meta-message usually gets us into trouble except in math where there is no place to missinterpretation. 

In any case, the situation helped me to reflect on my metaphors and narratives, which lately have a collapsing aspect. It is true that this year is proving to be a dark time for health professionals as we face a new situation that has been accompanied by intense emotions. 

First there was surprise and fear. For months we faced an infectious disease without protective measures, without diagnostic tests and without treatment. Then came anger after a long time of erratic management and lack of health leadership. Finally, there was the tiredness and hopelessness of finding ourselves back at square one having apparently learned little. 

For these reasons I have used on many occasions the image of the sinking ship, a narrative that I have shared for years with many other colleagues. In Primary Health Care we have been suffering for decades from severe limitations on investment and quite questionable management that has worsened the conditions of service year after year. It seems that such damaged tissue ends up breaking the few seams it has left when one witnesses atrocities such as those taking place this year at the Alamo health centre in Madrid where two or three doctors have been allowed to attend seven consultations at a time. 

Today I woke up with a new image that could perhaps replace the previous one. That of a two-masted sailing boat sailing in a strong swell. And maybe it can be more operational for us. It's true that the organisation of a primary care team resembles in part a crew with different roles, but it's also true that the boat has its structural problems and the seamanship is a bit short. But the storm is there, the worst we've seen in a long time, we're going into heavy seas. 

That's why complaining and denouncing is of little use when the waves are the size of a house. We have no choice but to do everything we can and, as the sailors know, to pray. The key word here is PRIORITY. Every ship in this sanitary navy will have to do the same without delaying to wait for instructions from the high admiralty that arrive badly and late. 


We need to prioritize:

  1. The change of attitude and narratives.
  2. Patient safety.
  3. The safety of the staff. 
  4. The care of the most vulnerable or seriously ill.
  5. The attention of the severe urgent pathology.


And everything else should be secondary. We cannot cover everything, especially in a summer scenario with a 50% staff, many exhausted professionals and a perspective of worsening epidemiological situation. 


On the other hand, I do not think we should give up:


  1. The progressive awareness of our situation.
  2. Communication and coordination between professionals and society.
  3. Making the situation visible and denouncing it to our leaders and to society.
  4. The legal way, when red lines are crossed.
  5. The union way, with actions of some forcefulness.


Yesterday we had strong summer storms in my area. I received them with joy and I believe that they have helped me to put myself in my place. We need to tune up ourselves from time to time. Today, with a cool day and the ground still wet, it seems that hope is a possibility, but not to be caught with our arms crossed.
