NO NOS CONFIEMOS. DON’T LET’S GET COMPLACENT. 不要自满胜利和冠军头衔会让人迷失




Los triunfos y los títulos ofuscan. Es bueno alegrarse pero no confiarse. En estos tiempos de rapidez y confusión son muchos los partidos que se juegan a la vez y muchas las porterías que son amenazadas. Hoy nos levantamos con la resaca del mundial de fútbol, la alegría de unos y la pena de otros. Algo que lleva pasando desde la caída de Troya, que precisamente pereció por confiada. 


No puedo evitar reflexionar sobre la liga del Colegio de Médicos de Madrid. Una candidatura ganó las elecciones hace año y medio pero el astuto perdedor solicitó una pequeña demora para que los nuevos cargos firmasen. En este tiempo armó un recurso que hundió las aspiraciones de los vencedores. 


Miro hacia oriente, allí no veo mucho fútbol pero sí mucha ingeniería de sistemas. Se están merendando la industria automovilística, baterías, energía solar, bienes de equipo, construcción naval, infraestructura pesada, robótica industrial, telecomunicaciones… y modelos ampliados de lenguaje.


Miro a nuestras instituciones, carreteras, servicios públicos, sistema político, judicial… un imperio resquebrajado.


Miro a mis manos vacías de médico de pueblo. El último mes y medio he compartido consulta con un residente de segundo año en rotación rural. Les contaré lo que hemos hecho para no desfallecer. 


En primer lugar me propuse aportar una nueva perspectiva que nos ayudase a los dos en nuestra labor clínica. La prioridad era ampliar la mirada. Lo contaremos más adelante. Tan solo adelantar que nos ayudamos de Homero, y como no de Odiseo. Él también ganó su mundial para luego perderlo absolutamente todo. Salvo el ingenio. Y eso es lo que le llevó de regreso a la casilla de salida. 


La película de Nolan es una buena ocasión de recordarlo. No podemos permitirnos el lujo de seguir en Europa durmiéndonos en los laureles. Vienen curvas, fuegos y catástrofes. Es hora de empezar a pensar, dialogar y construir. 




DON’T LET’S GET COMPLACENT


Triumphs and titles cloud the judgement. It is good to rejoice but not to grow complacent. In these times of speed and confusion, many matches are being played at once and many goals are under threat. Today we wake up with the hangover from the football World Cup, the joy of some and the sorrow of others. Something that has been happening since the fall of Troy, which precisely perished through complacency.I cannot help reflecting on the election at the Madrid College of Physicians. One slate won the elections a year and a half ago, but the astute loser requested a small delay so that the new office-holders could sign. In that time he prepared an appeal that sank the aspirations of the winners.I look eastwards; there I see little football but a great deal of systems engineering. They are devouring the automotive industry, batteries, solar energy, capital goods, shipbuilding, heavy infrastructure, industrial robotics, telecommunications… and large language models.I look at our institutions, roads, public services, political system, judiciary… a cracked empire.I look at my empty hands as a village doctor. For the last month and a half I have shared my surgery with a second-year resident on a rural rotation. I shall tell you what we have done so as not to despair.First of all, I set out to bring a new perspective that would help both of us in our clinical work. The priority was to broaden our gaze. We shall tell that story later. For now, I shall just mention that we turned for help to Homer, and of course to Odysseus. He too won his world cup only to lose absolutely everything afterwards. Everything except his ingenuity. And that was what took him back to the starting square.Nolan’s film is a good occasion to remember it. We in Europe cannot afford the luxury of continuing to rest on our laurels. Curves, fires and catastrophes are coming. It is time to start thinking, talking and building.



不要自满胜利和冠军头衔会让人迷失。

Triple fragilidad. Triple Frailty. 三重脆弱





1⃣ Un paciente frágil es una persona mayor de 70 años que presenta una disminución de la reserva fisiológica (Barthel mayor de 90). Esta vulnerabilidad lo expone a un algo riesgo de sufrir complicaciones graves, caídas, discapacidad, hospitalización o dependencia ante situaciones de estrés leve. 


2⃣ Una Atención Primaria frágil es aquella que tras décadas de desgaste presupuestario, gestión insuficiente y plantillas mermadas presenta una disminución de la capacidad funcional. Esta vulnerabilidad la expone a un alto riesgo de sufrir pérdida progresiva de efectivos, aumento de burnout, fallos de seguridad e incapacidad de asumir situaciones que aumenten la demanda. 


3⃣ Un médico de familia frágil es un profesional con dilatada trayectoria profesional que presenta signos de cansancio, hartazgo y enfado ante el sistema. Esta vulnerabilidad lo expone a un alto riesgo de desmotivación, desesperanza, enfermedad mental, adicciones y burnout ante situaciones que hagan rebosar su situación de vaso de agua lleno. 




Tengo a mi cargo unos 150-200 pacientes frágiles. Trabajo en un equipo rural frágil con dos médicas de baja larga, una pediatra sobrecargada, una directora que no da a basto y una gerencia con baja capacidad de gestión. La enfermera que comparte mis pacientes cambió de destino hace un mes y no es posible sustituirla por un problema administrativo que la gerencia no es capaz de arreglar. 


Soy un médico frágil. Trato de cumplir mi cometido pero no siempre lo consigo. Además cometo errores. Hoy me llaman del colegio de mi hija diciéndome que está con una fuerte migraña, imaginen el agobio. No funcionaba la centralita telefónica, aún me quedaban por ver varios pacientes y llamar a más de otros diez con mi teléfono personal (doy gracias de que al menos funcionaba el ordenador). Hablé con la hija de una anciana a la que no veo desde hace meses. Me informa que ha estado ingresada por una complicación grave por una infradosificación de un medicamento. Compruebo la historia y efectivamente estaba mal prescrito. Tendré que revisarlo a fondo pero el fallo parece mio. Me quería morir. Sé que esto puede pasar incluso en las mejores circunstancias, pero no se imaginan lo que duele cuando eres tú el que se equivoca. 


Reconocer que mis pacientes frágiles están frágilmente atendidos también es doloroso para mi. Pero es lo que hay. Y seguiré tratando de hacer todo lo posible para mejorarlo. Entre otras cosas compartiendo estas líneas que escribo a media noche con la intención de redimirme un poco y encomendarme a la paciencia de los que me conocen y, pese a todos mis fallos, siguen confiando en mi.


Esta semana los médicos de España estamos de huelga por disconformidad con un estatuto marco que impone condiciones de trabajo peores que al resto de trabajadores. Una huelga que apoyo pero que no puedo permitirme secundar en esta ocasión por que trabajo en un centro frágil con mucho paciente frágil. Además yo también soy frágil y me equivoco. Si alguien más desaparece del equipo el riesgo de colapso o complicación es altísimo. Y así llevamos viviendo en Atención Primaria mucho tiempo, al borde del colapso. Y nos equivocamos, no llegamos a todo. No es posible. 


Y con cada error, con cada situación que no logramos mejorar, vamos dejando trozos de nosotros por el camino. Hoy me comentaba un colega castellano que está de baja porque no puede más. Le entiendo bien, al menos me permitió regalarle un pequeño poema. Al menos nos queda la poesía. 






Triple Frailty

A frail patient is a person over 70 years of age who presents a decrease in physiological reserve (Barthel index greater than 90). This vulnerability exposes them to a high risk of suffering serious complications, falls, disability, hospitalisation, or dependency when faced with mild stress.

Frail Primary Care is that which, after decades of budgetary attrition, insufficient management, and depleted staffing levels, presents a decrease in functional capacity. This vulnerability exposes it to a high risk of progressive loss of personnel, increased burnout, safety failures, and an inability to cope with situations that increase demand.

A frail GP is a professional with an extensive career who shows signs of exhaustion, fed-upness, and anger towards the system. This vulnerability exposes them to a high risk of demotivation, hopelessness, mental illness, addiction, and burnout when faced with situations that cause their already overflowing cup to run over.

I am responsible for around 150–200 frail patients. I work in a frail rural team with two doctors on long-term sick leave, an overworked paediatrician, a practice manager who cannot cope, and a management board with poor administrative capacity. The nurse who shares my patients moved to a different post a month ago, and it is impossible to replace her due to an administrative issue that management is incapable of fixing.

I am a frail doctor. I try to fulfil my duty, but I do not always succeed. Furthermore, I make mistakes. Today I received a call from my daughter’s school telling me she has a severe migraine; you can imagine the anxiety. The telephone switchboard wasn't working, I still had several patients left to see, and I had to call more than ten others using my personal mobile (thankfully, at least the computer was working). I spoke to the daughter of an elderly woman whom I haven't seen for months. She informed me that her mother had been hospitalised with a serious complication due to an underdose of a medication. I checked the medical records and, indeed, it had been incorrectly prescribed. I will have to review it thoroughly, but the fault appears to be mine. I wanted the ground to swallow me up. I know this can happen even under the best of circumstances, but you cannot imagine how much it hurts when you are the one who makes the mistake.

Admitting that my frail patients are receiving frail care is also painful for me. But it is what it is. And I will keep trying to do everything possible to improve it. Among other things, by sharing these lines that I am writing at midnight, with the intention of redeeming myself a little and relying on the patience of those who know me and, despite all my flaws, continue to trust me.

This week, doctors in Spain are on strike due to disagreement with a framework statute that imposes worse working conditions than those of other workers. A strike that I support, but which I cannot afford to join on this occasion because I work in a frail centre with many frail patients. Moreover, I too am frail, and I make mistakes. If anyone else disappears from the team, the risk of collapse or complication is extremely high. And this is how we have been living in Primary Care for a long time, on the verge of collapse. And we make mistakes; we cannot keep on top of everything. It is not possible.

And with every mistake, with every situation we fail to improve, we leave pieces of ourselves along the way. Today a colleague from Castile told me he is on sick leave because he cannot take it anymore. I understand him well; at least he allowed me to gift him a short poem. At least we have poetry left.





三重脆弱 (Triple Frailty)

一位脆弱患者是指年龄在70岁以上、生理储备功能有所下降(巴氏量表Barthel指数大于90分)的老年人。这种脆弱性使他们处于极高的风险之中,一旦面对轻微的压力,就极易引发严重的并发症、跌倒、残疾、住院或产生依赖性。

一个脆弱的基层医疗系统是指历经数数十年的预算削减、管理不善以及人员流失后,功能容量出现下降的医疗机构。这种脆弱性使其面临极高的风险:人员持续流失、职业倦怠(burnout)加剧、安全漏洞频现,且无力应对任何需求增加的突发状况。

一位脆弱的家庭医生则是指一位有着丰富执业经验,但在体制面前表现出疲惫、厌倦和愤怒的专业人士。这种脆弱性让他们处于极高的风险之中:面对任何让本已倒满的水杯溢出来的压力事件时,他们极易陷入丧失动力、绝望、心理疾病、成瘾和职业倦怠的深渊。

我负责管理大约150到200名脆弱患者。我身处一个脆弱的农村医疗团队中:两名医生长期请病假,一位儿科医生超负荷工作,一位院长(负责人)分身乏术,而上级管理部门的管理能力又十分低下。一个月前,与我共同分担患者管理工作的护士调任到了其他岗位,但由于管理层无法解决的一个行政问题,至今都无法对该岗位进行人员接替。

我是一名脆弱的医生。我努力去履行我的职责,但并不总能做到。而且,我也会犯错。今天我接到女儿学校打来的电话,说她偏头痛发作得厉害,你可以想象我当时有多焦虑。当时卫生院的电话总机坏了,我还有好几位患者没看,而且还要用我自己的私人手机给另外十多个人打电话(谢天谢地,至少电脑还能用)。

我与一位已经好几个月没见面的老妇人的女儿通了话。她告诉我,她母亲因为药物剂量不足导致了严重的并发症,刚刚住过院。我核对了病历,发现确实开错药了。我必须得彻底复查一遍,但这个错误看起来是我的。当时我真想找个地缝钻进去。我知道哪怕在最好的情况下这种事也可能会发生,但你无法想象,当你成为犯错的那个人时,心里会有多痛。

承认我的脆弱患者们只得到了“脆弱的照顾”,这对我来说同样痛苦。但现实就是如此。我将继续竭尽全力去改善它。其中一个做法,就是分享我在午夜写下的这些文字,试图让自己得到一点救赎,并寄希望于那些了解我的人能多一些耐心——谢谢他们尽管我浑身是错,却依然信任我。

本周,西班牙的医生们正在举行罢工,因为大家对一项强加了比其他行业更差工作条件的框架法令表示不满。这场罢工我虽然支持,但这次我却无法参与,因为我工作的地方是一个脆弱的中心,有着太多脆弱的患者。而且,我自己也是脆弱的,我也会犯错。如果团队里再少一个人,系统崩溃或出现并发症的风险就会极高。长期以来,我们基层医疗就是这样生活在崩溃的边缘。我们也会犯错,我们无法顾及到每一件事。这根本不可能。

每一次犯错,每一个我们未能改善的境况,都让我们在沿途中丢掉一部分的自己。今天一位来自卡斯蒂利亚的同行告诉我,他因为实在撑不下去而请假了。我很理解他,至少,我还能送他一首小诗。

至少,我们还有诗歌。

Un encuentro regio. A royal encounter. 一次王室邂逅

Me crucé esta semana con el rey. En el puente de Segovia con la calle Mayor. Mientras esperaba a cruzar la calle Bailén cortaron el tráfico y pasó la comitiva de seguridad con el coche blindado de la banderita en el que viajaba su majestad a una importante reunión con embajadores en el palacio real. La asimetría de la situación era evidente pero un servidor ganaba en una cosa: grados de libertad. Disfrutaba de un día libre que me permitía ir y hacer lo que me viniera en gana sin ataduras de agenda ni problemas de estado. 


Ese ejercicio de libertad no es fácil de implementar. Libertad no es ir a tomar cervezas como postulan algunas, es algo más. Es un proceso de elección de opciones y sus correspondientes acciones derivadas. Confundir libertad con libertad de consumo es un engaño más. 


Lo cierto es que no es fácil abrirnos a más grados de libertad. Solemos protegernos por rutinas que nos ahorran el trabajo de pensar por cuanto la repetición nos exime de ponderar opciones. La ponderación desgasta. Lo habitual, como dice Jung, es que en lugar de fatigarnos pensando terminemos juzgando. 


Un pequeño experimento que me gusta hacer al salir a la calle sin rumbo definido es observar qué camino termino realizando. Con frecuencia terminamos repitiendo los mismos. Pero en esas rutinas se esconde la posibilidad de hacer pequeños cambios, tal vez ahí resida nuestra verdadera y pequeña capacidad de libertad. O quizá al hablar o al escribir pase lo mismo, nuestro fuero interno nos suele llevar por caminos trillados que, a veces, se abren a alguna posibilidad distinta. 


Llevo un año sin escribir. Un año sabático. Un disgusto hizo que perdiera el placer de compartir ideas, pensamientos y palabras. Afortunadamente he seguido caminando en silencio y voy recuperando la posibilidad de contar, de volver a hablar. He aprendido que es revolucionario guardar silencio en un mundo de gritos y ruido de fondo. Pero también que cuando uno calla otros ocupan su lugar, y en las plazas digitales lo hacen robots. 


Estamos abocados a un mundo cada vez más ruidoso, cada vez más manipulado por algoritmos, cada vez menos libre. Perdemos grados de libertad cuando caemos en las redes de la prisa, la multitarea, la extenuación, la manipulación… cuando consiguen que nos callemos y que agachemos la cabeza. 


Cometo errores, soy falible y también caigo al suelo. Pero tengo claro que merece la pena trabajar en provecho del bien común y compartir ideas en un tiempo que impone las suyas. Seguiremos intentándolo. 








A royal encounter

I ran into the King this week. At the junction of the Segovia bridge and Calle Mayor. While I was waiting to cross Calle Bailén, they stopped the traffic and the security convoy went past, with the armoured car bearing the little flag in which His Majesty was travelling to an important meeting with ambassadors at the Royal Palace. The asymmetry of the situation was obvious, but I had the edge in one respect: degrees of freedom. I was enjoying a day off that allowed me to go wherever I pleased and do whatever I fancied, with no constraints of schedule and no affairs of state to worry about.

That exercise of freedom is not easy to put into practice. Freedom is not going for a few beers, as some would have it; it is something more. It is a process of choosing between options and the corresponding actions that follow from them. Confusing freedom with freedom as a consumer is just another kind of deceit.

The truth is that it is not easy to open ourselves up to more degrees of freedom. We usually protect ourselves through routines that save us the work of thinking, because repetition frees us from having to weigh up options. Weighing things up is draining. As Jung says, what usually happens is that instead of tiring ourselves out by thinking, we end up judging.

A small experiment I like to do when I go out without a set destination is to observe which route I end up taking. We very often repeat the same ones. Yet within those routines lies the possibility of making small changes; perhaps our true, modest capacity for freedom resides there. Or perhaps, when we speak or write, the same thing happens: our inner world tends to lead us down well‑trodden paths which, every so often, open up to some different possibility.

I have not written for a year. A sabbatical year. An upset made me lose the pleasure of sharing ideas, thoughts and words. Fortunately, I have carried on walking in silence and I am gradually recovering the ability to tell, to speak again. I have learnt that keeping silent in a world of shouting and background noise is a revolutionary act. But I have also learnt that when one falls silent, others take one’s place, and in the digital squares those others are robots.

We are heading towards a world that is ever noisier, ever more manipulated by algorithms, ever less free. We lose degrees of freedom when we get caught in the nets of haste, multitasking, exhaustion, manipulation… when they manage to make us keep quiet and bow our heads.

I make mistakes, I am fallible and I too fall flat on my face. But I am certain it is worthwhile to work for the common good and to share ideas in a time that imposes its own. We shall keep trying.


一次王室邂逅

这周我偶遇了国王。
在塞哥维亚桥与马约尔大街的交叉口。
当我正等着穿过巴列恩街时,交通被拦下,一支安保车队驶过,里面有挂着小国旗的防弹车,陛下正坐在车里,前往王宫参加一场与大使们的重要会议。
这种情境下的不对称显而易见,不过有一点是我占了上风:自由度。
我正享受着一个休息日,可以随心所欲地去任何地方、做任何事情,没有日程安排的束缚,也没有国事烦扰。

这种自由的实践并不容易。
自由并不是像有些人说的那样只是去喝几杯啤酒,而是更深一层的东西。
它是一个在不同选项之间作出选择,并承担随之而来的行动的过程。
把自由混同于消费自由,只不过是另一种欺骗。

事实上,提升我们的自由度并不容易。
我们往往用日常惯例把自己保护起来,这些惯例帮我们省去了思考的力气,因为重复会让我们免于反复权衡选择。
权衡是很消耗精力的。
正如荣格所说,我们通常不会让自己因思考而疲惫不堪,而是干脆转向评判。

每当我没有预设目的地就出门时,我都喜欢做一个小实验:观察自己最后走的是哪条路。
我们往往会重复走同样的路线。
然而,正是在这些惯例中,隐藏着做出微小改变的可能,也许我们的那一点点真正自由的能力就蕴藏于此。
或者,在说话和写作时也是如此,我们的内心世界往往引导我们走上那些早已踏平的小路,而这些小路有时会通向某种新的可能。

我已经一年没有写作了。
一个休假年。
一场不愉快让我失去了分享想法、思绪和文字的乐趣。
所幸的是,我一直在默默前行,如今慢慢重新找回了讲述、重新开口说话的可能。
我明白了,在一个充斥着呐喊和背景噪音的世界里保持沉默本身就是一种革命性的举动。
但我也明白,当一个人沉默时,别人就会占据他的位置,而在数字广场上,那些“别人”是机器人。

我们正走向一个愈发喧嚣、愈发被算法操控、愈发缺乏自由的世界。
当我们落入匆忙、多任务处理、精疲力竭和被操纵的罗网之中,当他们让我们噤声、让我们低头时,我们就失去了自由度。

我会犯错,我并不完美,我也会跌倒。
但我很清楚,为了公共利益而努力、在一个把自己的观念强加于人的时代分享想法,是值得的。
我们会继续努力下去。
