Relato: un duelo diferente. A different mourning. 不同的哀悼




No one had prepared her for that. It was not enough to be a beloved daughter in her family, to be an independent and capable professional, to be loved and desired. She burned in pain and nobody seemed to be sorry for her loss, even more she felt strange about herself for embodying such a great sorrow. But she could not help it. She had never experienced such a tear. She wept until she was dry, and yet she continued to miss terribly the being who had come closest to his soul without even uttering a word. Rest in peace his vitality, his joy, and his barking.








Nadie la había preparado para aquello. No bastó ser una hija querida en su familia, ser una profesional independiente y capaz, ser amada y deseada. Ardía en dolor y nadie parecía acompañarla en el sentimiento, más aún se sentía extraña de sí misma por encarnar una pena tan grande. Pero no lo podía evitar. Jamás había experimentado un desgarro así. Lloró hasta quedar seca y aun así seguía echando terriblemente de menos al ser que más se había acercado a su alma sin ni siquiera pronunciar una palabra. Descansen en paz su vitalidad, alegría y sus ladridos.

没有人为此准备她。在家庭里做个心爱的女儿,做个独立、有能力的专业人士,做个受人爱戴和渴望的女儿是不够的。她痛得浑身发烫,似乎没有人为她的损失感到难过,更使她感到奇怪,她自己竟有这么大的悲伤。但她情不自禁。她从未经历过这样的眼泪。她哭到干涸为止,可是她仍然非常想念那个最接近他灵魂的人,连一句话也没说。安息,他的活力,他的欢乐,和他的吠叫。

La disrupción tecnológica en el mundo de la salud. Technological disruption in health. 卫生技术中断




We all talk about the technological revolution being witness about how it is transforming our environment. First there were the travel agencies, then the taxis , later the tourist apartments rental, o online shopping, online education...


However, one question remains to be answered: how will health disruption affect health systems and their professionals and users?


From my point of view we can know something: the change will be progressive and will come from the successive implementation of technologies that provide advances in different aspects of clinical practice, management and organization of resources. We also know that health systems are huge organisations with high inertia and that health professionals tend to have a conservative profile when it comes to incorporating new technologies. Before we adopt them we need to have scientific evidence that is both useful and beneficial to the patient and society. The problem is that scientific evidence is costly and very slow. The average time for a new molecule to become an accepted drug is about ten years, which is unreasonable for products and services in the digital sphere.


Perhaps it can be said that the disruption of healthcare technology began a few decades ago with the introduction of electronic medical record systems that allowed health services to begin to be digitised. The growing volumes of data favoured the creation of information and management systems with which to get to know what was being done and establish roadmaps towards the organisation's objectives.


 At present, there is a lot of recorded information that is not yielded and a lot of free text that remains qualitative and unusable. What will happen when there are semantic algorithms that can quantify both the information written in history and the information spoken in consultation?


Another advance front will come from the 5G mobile telephony technology that will generalize immersive virtual reality scenarios that will allow health professionals and patients to interact remotely in high quality teleconsultations. It will also be possible to interact with bots or artificial avatars that will allow increasingly purified degrees of conversation.


The advances in wereables will also allow an increasing number of variables to be recorded: temperature, heart rate, blood pressure, arterial oxygen saturation, physical activity, sleep... which, when added to the facial analyses, can make inferences in emotional states, quantification of pain and functional impotence and many other possibilities.


With this amount of information it will be necessary to create personal artificial intelligence units to guard, analyse and manage all the biometric data that each individual produces, connected in turn to those of their responsible health professionals who will look after them. Just as the progressive introduction of autonomous cars will completely modify mobility as we now know it, the implementation of personal health assistants will do the same with the provision of health services.


The revolution will be led by the citizens themselves by having better technological services that will progressively generate more biometric information. They will know how they sleep, how they move and how they should move, they will know their degrees of cardiovascular risk, they will detect early arrhythmias, hypertensions and diabetes, they will have alerts of possible incipient pathology... all of which will produce a foreseeable exponential increase in the need for health services to check or act on the data presented. Against all odds it will not be the chronically and complex patients who end up dismantling the current health systems, it will be the healthy citizens eager to enjoy better health.


The hypothesis presented here attempts to help generate a global reflection in health professionals and at the social level. Given that it is the advances and proposals of the market, led by the increasingly powerful technological companies, that seem to be taking the initiative to develop knowledge societies, what role should professionals play with technical knowledge?, what courses of action would produce more social benefit?, will they coincide with those that produce more personal benefit?

It would seem clever for health professionals to play an active role in the design and development of the new tools and the new digital architectures that will be built. The conversation with computer engineers, developers and designers should include healthcare professionals, patients and managers. In order to be able to create social value, it is intelligent for us to add to the strength of the market itself a plus of collective intelligence based on the best possible scientific evidence and on values such as justice and social solidarity. It is true that everything in health is going to change, but it is also possible to work together so that this change would be oriented towards an scenario that benefits us most as a society.

Todos hablamos de la revolución tecnológica y somos testigos de cómo está transformando nuestro entorno. Primero fueron las agencias de viaje, luego los taxis y el alquiler de pisos turísticos, las compras online, la educación a distancia...

Sin embargo una pregunta queda por responder: ¿cómo afectará la disrupción sanitaria a los sistemas sanitarios y a sus profesionales y usuarios?

Desde mi punto de vista algo podemos saber, por ejemplo que el cambio será progresivo y vendrá de la mano de la sucesiva implementación de tecnologías que provean avances en distintos aspecto de la práctica clínica, la gestión y la organización de recursos. También sabemos que los sistemas sanitarios son enormes organizaciones con grades inercias y que los profesionales sanitarios solemos tener un perfil conservador a la hora de incorporar nuevas tecnologías. Antes de adoptarlas necesitamos disponer de  pruebas científicas que sean útiles a la par que beneficiosas para el paciente y la sociedad. El problema reside en que la evidencia científica es costosa y muy lenta. El tiempo medio para que una nueva molécula se constituya en fármaco aceptado es de diez años, plazo inasumible para los productos y servicios de la esfera digital.

Quizá se pueda decir que la disrupción tecnológica sanitaria comenzó hace unas décadas con la instauración de los sistemas de historia clínica electrónica que permitieron empezar a digitalizar la prestación de servicios. Los crecientes volúmenes de datos favorecieron la creación de sistemas de información y de gestión con los que conseguir saber qué se estaba haciendo y establecer hojas de rutas hacia objetivos de la organización.

 En el momento actual hay mucha información registrada a la que no se saca un rendimiento y otra mucha en texto libre que sigue siendo cualitativa y no es susceptible de utilizar. ¿Qué pasará cuando haya algoritmos semánticos que puedan cuantificar tanto la información escrita en la historia como la hablada en consulta?

Otro frente de avance vendrá de la mano de la tecnología 5G de telefonía movil que generalizarán los escenarios de realidad virtual inmersiva que permitirán interactuar a profesionales sanitarios y pacientes a distancia en teleconsultas de alta calidad. Así mismo será posible interactuar con bots o avatares artificiales que permitirán grados cada vez más depurados de conversación.

Los avances en wereables permitirán a su vez registrar un número creciente de variables: temperatura, ritmo cardiaco, tensión arterial, saturación arterial de oxígeno, actividad física, sueño... que cuando añadan los análisis faciales podrán hacer inferencias en estados emocionales, cuantificación de dolor e impotencia funcional y otras muchas posibilidades.

Con tal cantidad de información será necesario crear unidades de inteligencia artificial personales que custodien, analicen y gestionen todos los datos biométricos que cada individuo produzca conectadas a su vez a las de sus profesionales de salud responsables que velarán por ellas. Así como la progresiva instauración de los coches autónomos modificará completamente la movilidad tal y como ahora la conocemos, la implementación de asistentes personales de salud harán lo mismo con la prestación de servicios sanitarios.

La revolución será liderada por los propios ciudadanos al disponer de mejores servicios tecnológicos que progresivamente irán generando más información biométrica. Sabrán cómo duermen, cómo se mueven y cómo deberían moverse, sabrán sus grados de riesgo cardiovascular, detectarán de forma precoz arritmias, hipertensiones y diabetes, tendrán alertas de posible patología incipiente... todo lo cual producirá un previsible aumento exponencial de la necesidad de servicios sanitarios que chequeen, comprueben o actúen ante los datos presentados. Contra todo pronóstico no serán los enfermos crónicos y complejos los que terminen desmontando los actuales sistemas sanitarios, serán los ciudadanos sanos deseosos de disfrutar de una mejor salud.

La hipótesis presentada trata de servir de ayuda para generar en un primer momento una reflexión global en los profesionales de la salud y en un segundo a nivel social. Dado que son los avances y propuestas del mercado, liderado por las cada vez más poderosas compañías tecnológicas, las que parece que están llevando la iniciativa de desarrollo de las sociedades del conocimiento, ¿qué papel deberían jugar los profesionales con conocimiento técnico?, ¿qué cursos de acción producirían más beneficio social?, ¿coincidirán con los que produzcan más beneficio personal?

Parecería sensato que los profesionales de la salud tuvieran un rol activo en el diseño y desarrollo de las nuevas herramientas y en las nuevas arquitecturas digitales que se irán construyendo. La conversación con ingenieros informáticos, desarrolladores y diseñadores debería incluir a sanitarios, pacientes y gestores. Para poder crear valor social es inteligente que a la propia fuerza del mercado le añadamos un plus de inteligencia colectiva basada en la mejor evidencia científica posible y en valores como la justicia y la solidaridad social. Es verdad que todo en salud va a cambiar pero también lo es que es posible trabajar juntos para que dicho cambio se oriente hacia el escenario que como sociedad más nos beneficie.

我们都在谈论技术革命是如何改变我们环境的见证。首先是旅行社,然后是出租车,后来是旅游公寓出租,网上购物,网上教育……


然而,还有一个问题有待回答:健康破坏将如何影响卫生系统及其专业人员和用户?


从我的观点来看,我们可以知道一些事情:这种变化将是渐进的,并且将来自于技术的连续实施,这些技术在临床实践、资源管理和组织资源的不同方面提供了进展。我们还知道,卫生系统是具有高惯性的大型组织,并且卫生专业人员在纳入新技术时往往具有保守的姿态。在我们采用它们之前,我们需要有科学证据,这些证据既有益又有益于患者和社会。问题在于科学证据昂贵而且非常缓慢。新分子成为公认药物的平均时间约为十年,这对于数字领域的产品和服务是不合理的。

这场革命将由公民自己领导,通过提供更好的技术服务,逐步产生更多的生物特征信息。他们会知道他们如何睡觉,如何移动以及应该如何移动,他们会知道他们的心血管危险程度,他们会发现早期心律失常,高血压和糖尿病,他们会有可能的早期病理学警报……所有这些都将产生可预见的指数增长,需要卫生服务来检查或对所呈现的数据采取行动。尽管存在种种可能性,但最终摧毁现有卫生系统的不是慢性和复杂的患者,而是渴望享有更好健康的健康公民。


这里提出的假设试图帮助卫生专业人员和社会层面产生全球性反映。鉴于由日益强大的技术公司领导的市场的进步和建议似乎正在主动发展知识社会,专业人员应该在技术知识方面发挥什么作用?什么样的行动课程会产生更多的社会效益?他们会与那些产生更多个人利益的人重合吗?

对于卫生专业人员来说,在设计和开发将要建立的新工具和新的数字体系结构方面发挥积极作用似乎是明智的。与计算机工程师、开发人员和设计人员的对话应该包括医疗专业人员、患者和管理人员。为了能够创造社会价值,我们明智的做法是,根据尽可能好的科学证据和诸如正义和社会团结的价值观,为市场本身的力量加上集体智慧。的确,一切健康的事物都会改变,但也有可能一起努力,使这种改变朝着正确的方向发展。

Mi médico es un Bot. My doctor is a Bot.






The great sanitary disruption is just around the corner. Specifically in China which is becoming the new world leader in innovation by leaps and bounds.

New health models are being created with non-human services in which people will interact with bots fed by artificial intelligence that will be applied to:


1. Administrative and appointment services.
2. Nursing services.
3. Medical services.


The market in China is enormous with large masses of rural population or in cities without sufficient health services. Providing automated solutions and ehealth products is therefore an obvious and very profitable need.


Money has started to move. Everything else will follow.

La gran disrupción sanitaria está a la vuelta de la esquina. Concretamente en China que se está convirtiendo en el nuevo líder mundial en innovación a pasos agigantados.

Se están creando modelos sanitarios con servicios no humanos en los que se interacionará con bots alimentados por inteligencia artificial que se aplicarán a:


1. Servicios administrativos y de cita.
2. Servicios de enfermería.
3. Servicios médicos.

El mercado en China es enorme existiendo grandes masas de población rural o en ciudades sin suficientes servicios sanitarios. Proveer soluciones y productos automatizados es pues una necesidad evidente y muy rentable.


El dinero ha empezado a moverse. Todo lo demás vendrá detrás.

Digitalización de la medicina. Digitization of medicine









One of the questions I am asked the most when I explain the changes that the Internet and the current technological revolution are producing in the health world is: Is it worth it for me?


That is why I continue working to try to explain what the Tsunami of changes that our society is undergoing and that health professionals do not fully understand given the complexity of current health systems, hyper-regulated and little given to structural innovation.

Una de las preguntas que más me hacen cuando expongo los cambios que Internet y la revolución tecnológica actual están produciendo en el mundo sanitario es: ¿me merece la pena?

Por eso sigo trabajando para tratar de explicar en qué consiste el Tsunami de cambios que nuestra sociedad experimenta y que los profesionales de la salud no terminamos de entender dada la complejidad de los actuales sistemas sanitarios, hiperregulados y poco dados a la innovación estructural.

¿Es seguro tomar nolotil? Is it safe to use nolotil painkiller?







The Spanish Ministry of Health has launched a precautionary warning with the use of Nolotil (Metamizole) at the behest of the British National Health Service which detected ten cases of serious reactions in English tourists who took this medicine in Spain.



1. Nolotil (Magnesium Metamizole) is a painkiller widely used in Spain and Latin America. It has good analgesic power.

2. In several countries it is prohibited due to the risk of serious blood reactions (loss of white blood cells). These reactions are rare.

3. It seems that in the Spanish population the serious reactions are less frequent.



Is it safe to take nolotil?



We will answer that like any medicine:

1. It should be used with caution, the shortest possible time at the lowest possible dose and if possible under the supervision of your doctor.

2. It is reasonable to avoid it in northern Europe people and in the elderly or frail people in whom it is suspected to be dangerous.






El Ministerio de Sanidad español ha lanzado un aviso de precaución con el uso del Nolotil (Metamizol) a instancias del Servicio Nacional de Salud británico que detectó diez casos de reacciones graves en turistas ingleses que tomaron ese medicamento en España.



1. El Nolotil (Metamizol Magnésico) es un fármaco contra el dolor muy utilizado en España y latinoamérica. Tiene buena potencia analgésica.

2. En varios países está prohibido por el riesgo de reacciones sanguíneas graves (pérdida de glóbulos blancos). Estas reacciones son poco frecuentes.

3. Parece que en población española las reacciones graves son menos frecuentes.



Ante estos datos ¿es seguro tomar nolotil?

Responderemos que como todo medicamento:

1. Ha de ser usado con precaución, el mínimo tiempo posible a la menor dosis posible y si es posible bajo supervisión de su médico.

2. Es razonable evitarlo en personas del norte de Europa y en ancianos o personas frágiles en los que se sospeche pueda ser peligroso.

Una década de médico en redes sociales. A decade as a doctor on social networks






 When I decided to start writing a blog as a practicing doctor in April 2008, a huge impulse to get out of the practice pushed me. I was aware then, as I am now, that it is no longer possible to maintain the same professional role as if nothing had changed. Jumping into the public arena that the Internet has opened is not an option in a society that has left behind its industrial base to settle into something as liquid as knowledge. By the mere fact of using mobile phones, search engines, apps and social networks we are already participating in this new place that is both market. If we choose to do it in a passive way this way the network will treat us. If, on the other hand, we risk producing content of any kind, the network will multiply it and spread it in a way that we will never be able to imitate. In both cases the price is high: our privacy.

In all these years I have had the option of sharing countless ideas, reflections, scientific evidence, narratives and poems... that have opened the door to conversations, people, groups and possibilities. With a few exceptions (giving a talk or conference), I have not earned any money with it, but I have to admit that the return on investment has come about in ways that are usually unsuspected. I have been able to talk to many journalists, collaborate in various media, get to know ideas, forums and remarkable people. I also participated in courses, talks and workshops in which I shared concerns, visions and possibilities. In the end, I was able to confirm once again that adage that says "there are many worlds but they are in this one". 

Was it worth it? The answer would imply a value judgment, and I don't really want to go to trial. What I can say is that the scales ended up equal. In one side there are many hours of reflection, writing, creativity and work. Of effort, constancy, attention and study. In the other to have been able to meet and learn from valuable people, to have written books, produced documentaries and videos, collaborated in interesting innovative projects, to promote scientific conferences without external sponsors... In short, I believe that I achieved what I originally intended, to get out of the consultation. And in doing so I found countless health professionals of different profiles and a society increasingly concerned about their health and their difficulties in dealing with the difficulty and disease.

Networks have changed a lot in this decade. It is more and more difficult to participate in conversations with people outside our circles of acquaintances or our ideas or interests. Almost all of us tend to relate to relatives and friends of our rope, with similar characteristics, pairs and ideology. In my assessment we have lost a lot with this tendency. 

Tension, trolls and insults have increased. When I had to suffer it, I admit that I was amazed to see how easily people who I considered serious and formed threw stones easily at facts that they judged terrible without having the data in hand. And many social networks are designed like the Roman circus in which it was enough to raise your thumb or lower it to decide the fate of those who fought in the coso for their lives. The networks end up defining us, can bring out the best in us and also the worst.

Health professionals will never be able to compete with youtubers, celebrities, postín journalists and tertullians. Our social leadership functionality is based on the great capacity for service that we deploy rather than on the visibility of our opinions on health. Very few manage to do activities of diffusion, scientific divulgation or health education  in radio, written press, television or Internet. Having participated in some of them I know well the work and effort behind them, so I admire and value each time that a colleague comes to the fore defending a health message that deserves it.

It is clear to me that health disclosure is a teamwork that is built with the bricks that each individual slowly contributes to a structure that he cannot see but that he intuits is greater than himself. A work that comes from afar and that will not be exhausted in us.



Of those hundreds of spanish health professionals who ten years ago constituted the health blogosphere, there were few of us left. The vast majority got tired, changed their interests or simply stopped talking. Many blogs stopped sharing content and many profiles on Twitter and other networks were silenced forever. At the same time, there were countless voyeurs who, without participating, followed the evolution of the former, a role that is fundamental in any self-respecting virtual social network. And perhaps the challenge is still here, in the ability to increase communication by making those who remain silent at their tables dare to go out and dance in the midst of the dance floor. The result has been an incomplete generational change because although there are new actors producing content, the level of conversations has not managed to emulate that of those pioneers who considered equals the physiotherapists, nurses, psychologists or doctors who spoke of health along with them. Losing the added value that the network brought with these conversations, it is common that many people value negatively a source that instead of quality information pours more and more background noise.

I do not consider myself an example for anyone but a lucky person. I can say without embarrassment that you have at your disposal  thousands of pages of a blog that is still alive, hundreds of videos, several collaborative projects, the documentary Six Minutes and the book Diary of a barefoot doctor. All my public presentations are accessible via the Internet and the articles I have published in scientific journals or other blogs as well. It's not that all this is very valuable, I'm just throwing away the little merit of having shared it. What you want to do with it is no longer up to me, but as a doctor I couldn't help but choose a path that tried to seek the social good both inside and outside the clinical practice. I intuit that if a significant number of health professional ever dared to emulate it, there would be a significant change. For now we will have to wait.









Cuando decidí comenzar a escribir en abril de 2008 un blog como médico en ejercicio un enorme impulso a salir de la consulta me empujaba. Era consciente entonces, como lo soy ahora, de que no es posible seguir manteniendo el mismo rol profesional como sin nada hubiera cambiado. Saltar a la arena pública que Internet ha abierto no es una opción en una sociedad que ha dejado atrás su base industrial para asentarse en algo tan líquido como el conocimiento. Por el mero hecho de usar móviles, buscadores, aplicaciones y redes sociales ya estamos participando en esta nueva plaza que es a la vez mercado. Si elegimos hacerlo de forma pasiva así nos tratará la red. Si por el contrario nos arriesgamos a producir contenido de cualquier tipo la red lo multiplicará y difundirá de una forma que nosotros no seremos capaces de imitar jamás. En ambos casos el precio es alto: nuestra privacidad.

En todos estos años he tenido la opción de compartir incontables ideas, reflexiones, evidencias científicas, narraciones y poesías... que me han abierto la puerta a conversaciones, personas, grupos y posibilidades. Salvo excepciones (dar alguna charla o conferencia) no he ganado dinero con ello pero he de reconocer que el retorno de la inversión se ha presentado por cauces habitualmente insospechados. He podido hablar con muchos periodistas, colaborar en medios diversos, conocer ideas, foros y personas notables. También participar en cursos, charlas y talleres en los que he compartido inquietudes, visiones y posibilidades. Al final he vuelto a constatar aquel adagio que reza "hay muchos mundos pero están en este". 

¿Mereció la pena? La respuesta implicaría un juicio de valor y no me apetece mucho someterme a juicio. Lo que sí puedo decir es que los platillos de la balanza acabaron igualados. En uno hay muchas horas de reflexión, escritura, creatividad y trabajo. De esfuerzo, constancia, atención y estudio. En el otro haber podido conocer y aprender de personas valiosas, haber escrito libros, producido documentales y vídeos, colaborado en interesantes proyectos innovadores, impulsar jornadas científicas sin patrocinadores externos... En definitiva creo que conseguí lo que en un principio me propuse, salir de la consulta. Y al hacerlo me encontré con incontables profesionales de la salud de diferente perfil y con una sociedad cada vez más preocupada por su salud y sus dificultades para manejar la dificultad y la enfermedad.

Las redes han cambiado mucho en esta década. Cada vez es más difícil participar en conversaciones con personas ajenas a nuestros círculos de conocidos o a nuestras ideas o intereses. Casi todos tendemos a relacionarnos con familiares y amigos de nuestra cuerda, con características parejas e ideología parecida. En mi valoración hemos perdido mucho con esta tendencia. 

Ha aumentado la crispación, los trols y las injurias. Cuando me ha tocado sufrirlo reconozco que me causó estupor ver con qué facilidad personas que consideraba serias y formadas lanzaban piedras sin pudor ante hechos que juzgaban terribles sin tener los datos en la mano. Y es que muchas redes sociales están diseñadas como el circo romano en el que bastaba levantar el pulgar o bajarlo para decidir la suerte de los que en el coso luchaban por su vida. Las redes acaban definiéndonos pueden sacar lo mejor de nosotros y también lo peor.

Los profesionales de la salud jamás podremos competir con youtubers, famosos, periodistas de postín y tertulianos. Nuestra funcionalidad de liderazgo social se basa en la gran capacidad de servicio que desplegamos más que en la visibilidad de nuestras opiniones en salud. Muy pocos consiguen hacer actividades de difusión, divulgación científica o educación para la salud en radio, prensa escrita, televisión o Internet. Al haber participado en algunas de ellas sé bien el trabajo y esfuerzo que hay detrás, por eso admiro y valoro cada ocasión en la que algún compañero o compañera salen a la palestra defendiendo un mensaje de salud que lo merece.

Tengo claro que es un trabajo en equipo que se construye con los ladrillos que cada individuo aporta lentamente a una estructura que no puede ver pero que intuye mayor que él mismo. Un trabajo que viene de lejos y que no se agotará en nosotros.

De aquella centena de profesionales de la salud que hace diez años constituimos la blogosfera sanitaria quedamos pocos. La gran mayoría se cansó, cambió sus intereses o simplemente dejó de conversar. Muchos blogs dejaron de compartir contenido y muchos perfiles de Twitter y otras redes enmudecieron para siempre. A su vez lo hicieron incontables mirones que sin participar seguían las evoluciones de los primeros, rol que es fundamental en toda red social virtual que se precie. Y es que quizá el reto siga estando aquí, en la capacidad de incrementar la comunicación haciendo que los que se mantienen callados en sus mesas se atrevan a salir a bailar a la pista. La resultante ha sido un cambio generacional incompleto pues aunque hay nuevos actores produciendo contenido, el nivel de conversaciones no ha conseguido emular el de aquellos pioneros que consideraban iguales a los fisioterapeutas, enfermeras, psicólogos o médicas que hablaban de salud junto con que ellos. Al perder el valor añadido que aportaba la red con estas conversaciones es común que muchos valoren negativamente una fuente que en lugar de información de calidad cada vez vierte más ruido de fondo.

No me considero un ejemplo para nadie pero sí una persona afortunada. Puedo decir sin avergonzarme que tienen a su disposición las miles de páginas de un blog que sigue vivo, cientos de vídeos, varios proyectos colaborativos, el documental seis minutos y el libro Diario de un médico descalzo. Todas mis presentaciones públicas son accesibles por Internet y los artículos que he publicado en revistas científicas o en otros blogs también. No es que todo esto sea muy valioso, tan solo me arrojo el pequeño mérito de haberlo compartido. Lo que quieran hacer con ello ya no depende de mi, pero como médico no he podido evitar elegir un camino que tratase de buscar el bien social tanto dentro como fuera de la consulta. Intuyo que si alguna vez un número significativo de sanitarios se atreviese a emularlo se produciría un cambio significativo. De momento tendremos que esperar.

¿Qué redes sociales recomendamos a nuestros pacientes? What social networks do we recommend to our patients?




Social prescribing is not yet a widespread health practice. Health professionals tend to focus on the body for which we recommend medications or improvements in lifestyle. Few health professionals work with a real biopsychosocial approach that integrates the psychological and social dimensions of the person and consequently includes them in the conversation, both in the anamnesis phase and in the recommendations phase. However, our society suffers a very serious epidemic of loneliness that affects many more people than we think. There are countless children alone, migrating from one extracurricular activity to another, many lonely old people anchored to a chair and intoxicated by television, many people whose main occupation is domestic chores who feel intolerably lonely and countless professionals who carry out their work with a feeling of isolation and helplessness. It is true that we are surrounded by a deafening background noise that gives us the illusion of being in the company of crowds, but it is a false mirage, when one approaches the noise in search of company will find absolutely no one.

Social prescription deals with opening the door of resources in the community where the person who asks for help lives. Advising and encouraging them to come out of their state of loneliness, difficulty or isolation and find other people with whom to communicate and interact. And we're not talking about advising Facebook, Instabram, Twitter or otherwise, that we already use too much. It's about remembering that the social network by autonomasia is the real one, the one formed by flesh and blood people who are our neighbors, relatives, friends and acquaintances.

Perhaps it would be good for us to remember the value of those slow and analogical everytime networks: the park, the walk, the coffee shop... as doctor Bernardino Oliva reminds us becouse besides being a seasoned doctor he is a good connoisseur of life.

It is true that health professionals must provide their patients with the right amount of information, resources, Internet links and reading recommendations. But it is also important to find out how they go about loneliness and human support in order to strengthen social seams when life offers tears and holes amidsts of illnesses or life crises.










La prescripción social aun no es una práctica sanitaria extendida. Los profesionales de la salud solemos centrarnos en el cuerpo para el que aconsejamos medicamentos o mejoras en los hábitos de vida. Son pocos los sanitarios que trabajan con un enfoque biopsicosocial real que integre la dimensiones psicológica y social de la persona y en consecuencia las incluya en la conversación, tanto en la fase de anamnesis como en la de recomendaciones. Sin embargo nuestra sociedad sufre una gravísima epidemia de soledad que afecta a muchas más personas de las que pensamos. Hay incontables niños solos, migrando de una actividad extraescolar a otra, muchos ancianos solos anclados a una silla e intoxicados de televisión, muchas personas cuya ocupación principal son los quehaceres domésticos que se sienten intolerablemente solas e incontables profesionales que desarrollan su labor con sensación de aislamiento e indefensión. Es cierto que estamos rodeados de un ruido de fondo ensordecedor que nos aporta la ilusión de estar en compañía de multitudes, pero es un espejismo falso, cuando uno se acerca al ruido en busca de compañía no encontrará absolutamente a nadie.

La prescripción social consiste en abrir a la persona que solicita ayuda la puerta de recursos en la comunidad donde reside. Aconsejar y animar para que salga de su estado de soledad, dificultad o aislamiento y encuentre a otras personas con las que comunicarse e interactuar. Y no hablamos de aconsejar que el personal frecuente Facebook, Instabram, Twitter o demás, eso ya lo hacemos en exceso. Se trata de recordar que la red social por autonomasia es la real, la que forman las personas de carne y hueso que son nuestros vecinos, familiares, amistades y conocidos.

Tal vez nos venga bien recordar el valor de esas redes lentas y analógicas de toda la vida: el parque, el paseo, el café... como bien nos recuerda Bernardino Oliva que además de ser un médico avezado es un buen conocedor de los mimbres de la vida.

Es verdad que los profesionales de la salud deben proveer a sus pacientes una cantidad de información adecuada a cada cual, proporcionar recursos, enlaces a Internet y recomendaciones de lectura. Pero también es importante averiguar como van de soledades y apoyos humanos para poder fortalecer las costuras sociales cuando la vida ofrezca desgarrones y boquetes de la mano de enfermedades o crisis vitales variadas.
