La Atención Primaria madrileña sufre una situación de limitación de recursos y sobrecarga crónica de desenlace incierto. La presión asistencial es cada vez mayor en numerosos equipos que ven con desesperación como las ausencias de personal aumentan sin cesar en un horizonte sin refuerzos ni líneas de mejora creíbles mientras los pacientes esperan cada día más a ser atendidos. Los responsables de gestión reciben a su vez presiones del político de turno para reducir listas de espera y mejorar la accesibilidad pero es imposible deshacer el embrollo sin mojarse con aumentos de presupuesto o reformas estructurales que nadie se atreve a acometer. La última ocurrencia de la gerencia de Atención Primaria madrileña es crear “agendas de rebosamiento” en las que un médico asume ese día todos los pacientes “sin cita” que acudan al centro de salud. Lo que se traduce en el “más madera” de aquella película de los hermanos Marx en la que alimentaban la máquina de vapor del tren con la madera de los vagones del mismo. El problema es que nos estamos quedando sin tablas, ya no queda nada que quemar. Aumentar el número de pacientes vistos disminuyendo tiempos de visita y calidad de asistencia, destruyendo la longitudinalidad y maltratando a los profesionales tal vez sea una opción a corto plazo, pero no es mantenible. 


No entraré en la cuestión de si es necesario subir o bajar impuestos dada la alta carga demagógica del aserto y el elevado contenido emocional que suele acompañar a quien defiende una u otra opción. Más bien animaré la reflexión sobre de qué forma la sociedad puede ayudar a que su sanidad pública no colapse, los gestores y políticos a plantear líneas de actuación reales y los profesionales a mantener el tipo en lo posible. La ciudadanía es quizá la única que tiene la sartén por el mango, sin saberlo. Primero necesita conocer el verdadero diagnóstico del caso: shock hipovolémico, es decir insuficiencia circulatoria por pérdida de fluidos vitales (económicos y de recursos humanos). Segundo admitir el pronóstico: el caso tiene muy mala pinta. Tercero asumir el tratamiento y su coste: medidas de cuidados intensivos que requieren reformas severas e inversión millonaria. En caso de no asumir la factura la sanidad terminará convertida en zombi, una organización ni muerta ni viva que seguirá funcionando con cada vez menos vida en las venas. Los telediarios deberían recordarnos a diario que en 5 años un 25% de los médicos de familia de este país se jubilarán. Y no habrá renovación, los más jóvenes cada vez eligen destinos en otros países o en puestos no tan sobrecargados. Si seguimos sin hacer nada lo que seguro habrá que asumir es que cuando llegue la hora de la ancianidad o la enfermedad todos lo pasaremos peor, los que tengan menos recursos más.

Rather, I will encourage reflection on how society can help to ensure that its public health system does not collapse, managers and politicians to propose real lines of action and professionals to maintain the status quo as far as possible. The public is perhaps the only one who has the upper hand, without knowing it. First, they need to know the true diagnosis of the case: hypovolemic shock, i.e. circulatory failure due to loss of vital fluids (economic and human resources). Second, admit the prognosis: the case looks very bad. Thirdly, to assume the treatment and its cost: intensive care measures that require severe reforms and an investment of millions. If the bill is not met, the health system will end up as a zombie, an organisation that is neither dead nor alive and will continue to function with less and less life in its veins. And there will be no renewal, the younger ones are increasingly choosing postings in other countries or in jobs that are not so overburdened. If we continue to do nothing, what we will surely have to accept is that when the time comes for old age or illness, we will all have a harder time, and those with fewer resources will have a harder time.

Reflexiones sobre el fracaso en el mundo sanitario. Reflections on failure in the healthcare world. 对医疗界失败的反思




 Publico hoy en la página de la Gestió importa un artículo sobre el fracaso. Con la que está cayendo pienso que puede ser de ayuda tomar conciencia de las posibles sombras para valorar y agradecer las luces. En un mundo como el nuestro tan reacio a asumir errores y fracasos perdemos la posibilidad de aprender de ellos y ayudarnos para corregir rumbos o enmendar heridas. Espero les resulte de utilidad. 




Tener una residente en la consulta implica más trabajo, pero tiene ventajas. Una de ellas es estimular la reflexión y la toma de conciencia del tutor. Se parece un poco al hecho de encender la luz un día de nubes, todo parece verse mejor en la habitación. La pregunta que me surgió es cómo afrontar la docencia del manejo del fracaso en un mundo como el sanitario que no parece tolerarlo y lo invisibiliza siempre que puede. 

 

Reflexionaba esta semana sobre el concepto de fracaso que va un poco más allá del de error médico que nos es más habitual. Crear una cultura de mejora continua implica de manera imprescindible trabajar con el error puntual pero cuando hablamos de fracaso reconocemos algo más: el malogro de un proyecto personal o profesional cuya línea de tiempo no alcanza objetivos o termina sin éxito. Tanto los errores como los fracasos duelen y suscitan sentimientos adversos. Hieren la autoestima y afectan el estado de ánimo. En ocasiones pueden alterar la paz mental y afectar el desempeño laboral. Estas consecuencias no se suelen hablar y cada cual se apaña como puede dentro de un ambiente de trabajo que no ayuda a compartir estas cuestiones. 

 

Sobre el fracaso personal

 

Dentro de mis fracasos profesionales detecté un buen puñado al hacer una retrospectiva, permítanme compartirles unos cuantos. Los cuatro años de desierto laboral post-MIR en los que tuve que trabajar de todo menos de médico de familia por no encontrar un contrato digno. El circuito de pacientes sin cita que hicimos en Lavapiés en consenso con administrativos y enfermeras que duró dos semanas. El fracaso de la conciliación laboral con cuatro hijos al conseguir un turno de mañana a los 49 años. El fracaso como director de centro de salud cuando dimití por desavenencias con la dirección. El fracaso de mis proyectos de innovación (blog para pacientes, redes sociales, vídeos y salud…). Mi fracaso reivindicativo de la situación de la Atención Primaria…

 

Y ahí tenemos a los brillantes médicos MIR enfrentándose a diario con fracasos que no preveían. El posible fracaso en las urgencias, donde al principio son totalmente incapaces y después a penas se mantienen a flote. El posible fracaso con algunas rotaciones, técnicas o aprendizajes. El posible fracaso en las relaciones laborales. El posible fracaso en su esfera personal y relacional… Pero nadie dice nada de esa cara B dado que lo que se publica en las redes sociales tiene más que ver con la versión luminosa de cada cual en la que no cabe la sombra. 

 

A nivel de organización pasa lo mismo, nos cuesta hablar del fracaso. Es verdad que somos muy buenos a la hora de quejarnos o señalar fallos y errores ajenos. Y si son de categorías profesionales distintas, otras especialidades, el mundo de la gestión sanitaria en general o la política sanitaria en particular, somos verdaderos expertos. Pero reconocer el fracaso de nuestra unidad funcional, nuestra categoría profesional o especialidad es excepcional dado que esto nos mete en la ecuación y nos señala y a nadie le gusta salir en determinadas fotos. 

 

Admitir un fracaso implica un juicio. Es necesario dictaminar que un proyecto se ha malogrado. Si hablamos de un fracaso personal esto solo será posible si lo reconocemos. Para ello hace falta un ejercicio de humildad que reconozca lo que hay. No es necesario pasarnos de punitivos o de condescendientes, tan solo admitir que como humanos fallamos y fracasamos continuamente como parte de nuestra naturaleza. Y luego tener el valor de admitirlo ante los demás, algo que no suele ser sencillo pero que tiene un potente efecto de catarsis y liberación. 

 

Sobre el fracaso del sistema de salud

 

Si echamos una mirada amplia a nuestro sistema sanitario no tardaremos en ver fracasos por todas partes. Algunos pequeños, otros más grandes. Ha fracasado una Atención Primaria cada vez más ahogada, fracasan las ardientes urgencias de hospital, fracasa la atención a la salud mental, fracasa el sistema cuando sobremedica, fracasa la gestión sanitaria cuando se maltrata al personal, fracasa la sociedad cuando abusa del sistema. 

 

Pero no nos podemos quedar en la penumbra, si bien es necesario admitirla, reconocerla y airearla. Creo que es sano asumir las luces y las sombras que hay en nuestras vidas y en las personas e instituciones que nos rodean. Sombras que como ya percibió Carl Jung compartimos dentro de ese gran inconsciente colectivo donde se albergan sueños y pesadillas comunes. Juan Gérvas describe a los médicos de familia como perdedores de largo recorrido en una expresión que tiene que ver con lo que estamos hablando y que contiene una semilla de perseverancia y esperanza. No pasa nada si vamos perdiendo cosas por el camino si sabemos a dónde queremos ir y qué valores defender. De él tomo la cita que inicia este texto y la sumo a esta pequeña reflexión que creo importante pensando en cada uno de nosotros y en las nuevas generaciones de profesionales sanitarios que vienen y se están encontrando una sanidad flamígera con cada vez más profesionales seniors heridos y quemados. Que tengan la referencia de compañeros con largas carreras profesionales y grandes historias de fracasos a sus espaldas que sigan sonriendo y manteniéndose al pie del cañón es más importante que nunca. Para todos.






Reflections on failure in the healthcare world


Today I am publishing an article on failure on the Gestió importa website. With the current crisis, I think it might be helpful to be aware of the possible shadows in order to value and be grateful for the lights. In a world like ours, so reluctant to accept mistakes and failures, we lose the possibility of learning from them and helping us to correct our course or mend our wounds. Having a resident in the practice means more work, but it has advantages. One of them is to stimulate the reflection and awareness of the tutor. It is a bit like turning on the light on a cloudy day, everything seems to look better in the room. The question that arose in my mind is how to deal with the teaching of failure management in a world such as healthcare, which does not seem to tolerate it and makes it invisible whenever it can. 

 

This week I was reflecting on the concept of failure, which goes a little further than the more commonplace concept of medical error. Creating a culture of continuous improvement implies working with the occasional error, but when we talk about failure we recognise something else: the failure of a personal or professional project whose timeline does not achieve its objectives or ends unsuccessfully. Both mistakes and failures hurt and arouse adverse feelings. They hurt self-esteem and affect mood. They can sometimes disturb peace of mind and affect job performance. These consequences are often not talked about and everyone copes as best they can in a work environment that is not conducive to sharing these issues. 

 

On personal failure

 

Among my professional failures I detected a handful in retrospect, let me share a few with you. The four years of post-MIR labour desert in which I had to work as anything but a family doctor because I could not find a decent contract. The circuit of patients without appointments that we made in Lavapiés in consensus with administrative staff and nurses that lasted two weeks. The failure to reconcile work and family life with four children by getting a morning shift at the age of 49. The failure as director of a health centre when I resigned due to disagreements with the management. The failure of my innovation projects (blog for patients, social networks, videos and health...). My failure to make demands about the situation of primary care...

 

And there we have the brilliant MIR doctors facing daily failures that they did not foresee. The possible failure in the emergency department, where at first they are totally incapable and then they can barely stay afloat. The possible failure with some rotations, techniques or training. Possible failure in working relationships. The possible failure in their personal and relational sphere... But nobody says anything about this B-side, given that what is published on social networks has more to do with the luminous version of each person in which there is no room for the shadow. 

 

At the organisational level it's the same thing, we find it difficult to talk about failure. It is true that we are very good at complaining or pointing out other people's failures and mistakes. And if they are of different professional categories, other specialties, the world of health management in general or health policy in particular, we are real experts. But acknowledging the failure of our functional unit, our professional category or speciality is exceptional because it brings us into the equation and points the finger at us, and nobody likes to be in certain photos. 

 

Admitting failure implies a judgement. It is necessary to rule that a project has failed. If we are talking about a personal failure, this will only be possible if we acknowledge it. This requires an exercise in humility that recognises what is there. It is not necessary to be overly punitive or condescending, just to admit that as humans we fail and fail continuously as part of our nature. And then have the courage to admit it to others, which is often not easy but has a powerful cathartic and liberating effect. 

 

On the failure of the health system

 

If we take a broad look at our health care system, it is not long before we see failures everywhere. Some small, some larger. Primary care has failed when it is increasingly suffocated, hospital emergency departments have failed, mental health care has failed, the system has failed when it overmedicates, health management has failed when it mistreats staff, society has failed when it abuses the system. 

But we cannot remain in the shadows, although it is necessary to admit them, to recognise them and to air them. I believe it is healthy to assume the lights and shadows in our lives and in the people and institutions that surround us. Shadows that, as Carl Jung already perceived, we share within that great collective unconscious where common dreams and nightmares are harboured. Juan Gérvas describes family doctors as long term losers in an expression that has to do with what we are talking about and which contains a seed of perseverance and hope. It is all right if we lose things along the way if we know where we want to go and what values we want to defend. From him I take the quote that begins this text and I add it to this small reflection that I think is important for each of us and for the new generations of health professionals who are coming and are finding a flaming healthcare system with more and more wounded and burnt senior professionals. It is more important than ever that they have the reference of colleagues with long professional careers and great stories of failure behind them, who continue to smile and stay on the job. Dieciséis vueltas al mundo. Sixteen times around the world. 绕行世界16次




Mientras su madre leía un libro del espacio a nuestra hija en la cocina oigo desde el salón que hablan del tamaño terrestre, me preguntan si conozco el diámetro, respondo que se lo pregunten a Siri. El asistente les da el dato y tras un breve cálculo mental oigo "pues con los dos últimos coches he dado la vuelta al mundo dieciséis veces". Mi mujer es médica de familia, para ir a trabajar conduce 112 km al día, 560 a la semana, 2240 al mes, 24640 al año. Muchos años de suplencias en pueblos remotos, refuerzos lejanos, interinidades a distancia y plaza fija a más de 56 km de casa. Pienso que no voy muy a la zaga, podría poner sobre la mesa otras tantas vueltas y ya hablaríamos de 32 sin contar las que por viajes de ocio acumulamos. 

Si hiciéramos el cálculo de pacientes atendidos también saldría una cifra loca, toda vez que un generalista en España tiene presiones asistenciales y cargas de trabajo superiores a los países que nos rodean. 

Pero, más allá de los números me pregunto ¿servirá esto para algo? y viendo cómo las instituciones parecen derretirse y el malestar social aumentar no puedo evitar sentir escalofríos. 

Nos acercamos a un momento en el que tendremos que dejar de girar como locos y volver a caminar. Un momento en que quizá no será fácil encontrar médicos de familia. No adelantemos el futuro, hoy me conformo con mirar a la luna y parar un momento. 





Sixteen times around the world
