La humanidad constituye un ente singular. Encarna la paradoja de ser por un lado una gran tribu de primates y al mismo tiempo tener la capacidad de desplegar sorprendentes propiedades emergentes. 

Sin embargo pudo más su deseo insaciable que su inteligencia y no está siendo capaz de evitar un previsible colapso civilizatorio global. Los ajustes necesarios nos obligarían a iniciar un proceso de decrecimiento para el que nadie está preparado, ni siquiera los ecologistas más convencidos o los científicos más conscientes. 

Estamos en una situación de emergencia global ante lo que probablemente sea el mayor reto que como especie hayamos enfrentado. Y no tenemos una buena jugada, las cartas son malas. En situaciones así estamos en manos de la providencia y dependemos de lo inesperado. Es verdad que la capacidad creativa y de adaptación del ser humano es enorme. También que bajo presión somos capaces del mayor heroísmo. Habrá que ver.

Creo que es importante dejar espacio a la esperanza. Vamos a tener que soportar mil discursos catastrofistas que de paso tratarán de vendernos desde productos de consumo a ideas políticas. El miedo, el agobio y la furia alcanzarán cotas nunca vistas dado que las emociones se contagiarán globalmente y las masas se moverán ciegas y terribles.

Dicen los místicos que en el mismo centro de la noche oscura del alma habita una presencia que ilumina. Cuando nos toca atravesar una crisis vital terminamos recurriendo a esa fuente que sacia nuestra sed de sentido, equilibrio y transcendencia. Quizá también ahora sea sensato buscar ahí las respuestas que cada cual precise en lugar de dejarnos llevar por la locura colectiva y la tormenta de ruido de fondo que ya empieza a desatarse ahí afuera. 





Humanity is a unique entity. It embodies the paradox of being a great tribe of primates and at the same time having the ability to unfold amazing emergent properties. 

However, it’s insatiable desire prevailed over its intelligence and is not being able to avoid a predictable global civilisational collapse. The necessary adjustments would force us to initiate a process of decline for which no one is prepared, not even the most convinced ecologists or the most conscientious scientists. 

We are in a situation of global emergency in the face of what is probably the greatest challenge that we as a species have ever faced. And we don't have a good move, the cards are bad. In situations like this we are in the hands of providence and we depend on the unexpected. It is true that the creative and adaptive capacity of human beings is enormous. It is also true that under pressure we are capable of the greatest heroism. We will have to see.

I think it is important to leave room for hope. We are going to have to endure a thousand catastrophic speeches that will try to sell us from consumer products to political ideas in the process. Fear, oppression and anger will reach unprecedented heights as emotions will be globally transmitted and the masses will move blindly and terribly.

The mystics say that in the very centre of the dark night of the soul there is a presence that illuminates. When we have to go through a life crisis we end up turning to that source that satisfies our thirst for meaning, balance and transcendence. Perhaps now it's also clever to look for the answers we each need there instead of letting ourselves be carried away by the collective madness and the storm of background noise that's already beginning to break out there.

Sobre la profilaxis del VIH con pastillas (pPrep)




  • El tratamiento preventivo del VIH con pastillas (pPrep) está disponible desde hace años. El Ministerio de Sanidad aprobó la financiación pública del medicamento hace un mes pero aún no está disponible en la mayoría de las Comunidades. En Cataluña se puede conseguir en muchos hospitales y en Madrid en el centro Sandoval (c/Sandoval 7) a partir de finales de diciembre. 
  • La pastilla se toma una vez al día y tras una semana de uso protege contra la infección al 99% si se toma diaria y con menor porcentaje si no es así. 
  • En España no está disponible en farmacias y quien la necesite debía comprarla por Internet (30-50€ al mes).
  • Es un tratamiento a considerar para personas que estén en riesgo de contactar con el virus y que no quieran usar preservativos, que es la opción en este momento más segura. 
  • Como médico me llama poderosamente la atención:


1. Que aún no esté disponible en farmacias (lo que obliga a comprarlo online con riesgo).
2. Que la mayoría de los profesionales sanitario no tengamos información al respecto.

¿Qué es la Profilaxis Pre-exposición (PrEP)?

  • Pre = antes (previa)
  • Exposición = tener contacto con el VIH
  • Profilaxis = tratamiento para prevenir que ocurra una infección
La profilaxis Pre-exposición (PrEP, siglas en inglés) es una estrategia de prevención de la infección con el VIH donde los individuos VIH negativos toman medicamentos anti-VIH antes de entrar en contacto con el VIH para reducir su riesgo de infectarse. Los medicamentos previenen que el VIH establezca una infección dentro del cuerpo.
PrEP ha demostrado reducir el riesgo de infección con el VIH a través del contacto sexual en los  hombres gay y bisexuales, mujeres transgénero y en hombres y mujeres heterosexuales, así como en personas que se inyectan drogas.
No protege contra otras infecciones de transmisión sexual (ITS) ni tampoco previene el embarazo. No es una cura para el VIH.

¿Para quién funciona la PrEP?

La PrEP ha demostrado reducir el riesgo de infección con el VIH en muchos estudios. El estudio iPrEx demostró que la PrEP reduce el riesgo de infección con el VIH entre los hombres gay y bisexuales y en las mujeres transgénero. Dos grandes estudios, Partners PrEP y TDF2, demostraron que PrEP también reduce el riesgo de infección con el VIH entre los hombres y mujeres heterosexuales. El estudio Bangkok Tenofovir demostró que la PrEP funciona para las personas que se inyectan drogas.




Efectividad

El uso de Truvada como PrEP provee una reducción del riesgo de infectarse del 96%  al 99% en las personas VIH negativas que toman las píldoras todos los días de acuerdo con  las indicaciones. Si se saltea una dosis diaria, el nivel de protección contra el VIH puede disminuir. Sólo funciona si toma el medicamento. Las personas que usan la PrEP correcta y consistentemente tienen niveles más altos de protección contra el VIH.
De acuerdo con el análisis de los datos del estudio iPrEx se descubrió que la PrEP es efectiva para:
  • Las personas que toman 7 píldoras de PrEP por semana, su nivel estimado de protección es del 99%
  • Las personas que toman 4 píldoras de PrEP por semana, su nivel estimado de protección es del 96%
  • Las personas que toman 2 píldoras de PrEP por semana, su nivel estimado de protección es del 76%
No existe suficiente data disponible para proveer una guía temporal específica que no sea de uso diario, por lo que la FDA recomienda que la PrEP se use diariamente para alcanzar el nivel más alto de protección.
Un par de cosas que deben tenerse en cuenta:
  • Al comenzar la PrEP, lleva al menos siete días para alcanzar altos niveles de protección contra el VIH.
  • Al suspender la PrEP, las personas deben continuar utilizando la PrEP durante cuatro semanas después de una exposición significativa.
  • La PrEP no protege contra otras infecciones de transmisión sexual (ITS) ni previene el embarazo. No es una cura para el VIH.




Efectos secundarios

Truvada como PrEP es generalmente seguro y bien tolerado. La mayoría de las personas que usan la PrEP reportan no experimentar efectos secundarios, pero se reportaron algunos efectos secundarios en los estudios clínicos. Los participantes del estudio iPrEx reportaron efectos secundarios que se encuadran en cuatro categorías principales (aquí ordenadas de más a menos comunes):
  1. Náusea: El 9% de quienes recibieron Truvada reportaron náuseas en el primer mes, en comparación con el 5% de los que recibieron placebo. Después del primer mes, no hubo diferencia en las tasas reportadas de náuseas entre los que recibieron Truvada y quienes recibieron placebo.
  2. Dolores de cabeza: El  4.5% de las personas que recibieron Truvada reportaron dolores de cabeza, en comparación con el 3.3% de quienes recibieron placebo.
  3. Pérdida de peso: El 2.2% de las personas que recibieron Truvada reportaron una pérdida de peso no intencional del 5%, en comparación con el 1.1% de los que recibieron placebo.
  4. Pequeños aumentos de la creatinina sérica: se sabe que Truvada causa pequeños aumentos en la creatinina sérica, una molécula de ocurrencia natural que es filtrada en los riñones. En este estudio, el 0.3% de quienes recibieron Truvada experimentaron aumentos moderados en la creatinina sérica que persistieron hasta el siguiente análisis. Los niveles de creatinina volvieron a bajar una vez que estos participantes abandonaron la PrEP. Cuatro de los cinco participantes retomaron la PrEP sin que volviera a aumentarles el nivel de creatinina. Los investigadores monitorearon la función renal durante la duración del estudio y no encontraron problemas serios.
Para la mayoría de las personas, estos efectos secundarios desaparecieron por si solos después de las primeras semanas de tomar Truvada, o cuando se suspendieron los medicamentos.

La medicina emergente. Emerging Medicine. 新興醫學




En esta época caracterizada por la transición de sociedades sólidas a líquidas todo parece confuso. El mundo sanitario sufre convulsiones ante el aumento de la complejidad de pacientes cada vez más mayores y con más problemas de salud por un lado y la gran afluencia de jóvenes y personas de edad media con problemas de salud menores o de adaptación a un medio social y laboral hostil.

Para los profesionales de la salud cada día que pasa se nos hace un poquito más cuesta arriba dar la talla. La sobrecarga habitual sigue aumentando y no encontramos suficiente tiempo ni para escuchar al paciente como se merece ni para reflexionar los casos que nos llegan. ¿Solucionará esto la tecnología como algunos profetas nos anuncian?

La respuesta no es fácil. Aún no disponemos de soluciones disruptivas, a lo más parches o promesas pero nada que ofrezca respuesta a los retos que enfrentamos.

Ante el sufrimiento humano parece necesario un acompañamiento con rostro humano. Lo que no está tan claro es hasta qué punto esto debe asumirlo el sistema sanitario. Por otro lado la valoración de la complejidad en las esferas biológica, psicológica, social y existencial de la persona es algo que probablemente exceda la capacidad de una máquina por muy desarrollada que esta sea.

Seguramente veamos desarrollos de inteligencia artificial adaptados a problemas de salud menores que sean susceptibles de ser manejados con autocuidados. Al igual que hoy todo el mundo tiene un ordenador personal en su teléfono llegará el día en que tengamos un asesor personal que nos ayude con la gestión de pequeños problemas de salud o con procesos de patología crónica que exijan supervisión y control.

Por otro lado no parece descabellado pensar en que cada profesional sanitario sea reforzado a su vez por un asistente personal que ofrezca ayuda tanto para el diagnóstico como para el tratamiento y la estrategia de cuidados de cada paciente.

Lo que no tengo tan claro es si los profesionales aguantarán la presión. Toda tecnología implica un coste personal. Si no se protege al profesional calibrando la presión asistencial que recibe y la dificultad y complejidad de lo que trate es fácil que este acabe quemado o saturado. Lo estamos viendo y cada vez va a más.

Cuando uno tiene una enfermedad grave o potencialmente peligrosa, cuando uno está próximo a la muerte o ha sufrido un accidente importante necesita ayuda. En esos casos queremos tener cerca a nuestros seres queridos y un equipo sanitario lo más excelente posible. Nadie quiere verse rodeado solo de robots aunque cada vez tengamos más alrededor.



Emerging Medicine

In this era characterized by the transition from solid to liquid societies, everything seems confusing. The health world suffers convulsions due to the increasing complexity of increasingly older patients with more health problems on the one hand and the large amount of young people and middle-aged people with minor health problems or bad adaptation to a hostile social and working environment.

For health professionals every day that passes to work properly it's harder. The usual overload continues to increase and we do not find enough time either to listen to the patient as he deserves or to reflect enough on the cases that come to us. Will technology solve this as some prophets tell us?

The answer is not simple. We still do not have disruptive solutions, at most patches or promises but nothing that offers an answer to the challenges we face.

If we talk about human suffering, it seems necessary to have an accompaniment with a human face. What is not so clear is the extent to which the health system should take this on board. On the other hand, the assessment of complexity in the biological, psychological, social and existential spheres of the person is something that probably exceeds the capacity of a machine, however developed it may be.

Surely we will see developments of artificial intelligence adapted to minor health problems that can be handled with self-care. Just as today everyone has a personal computer on their phone, the day will come when we will have a personal advisor to help us with the management of small health problems or with chronic pathology processes that require supervision and control.

On the other hand, it does not seem outlandish to think that each health professional should be reinforced in turn by a personal electronic assistant who offers help both for the diagnosis and for the treatment and care strategy of each patient.

What is not so clear to me is whether the professionals will withstand the pressure. All technology has a personal cost. If you don't protect the professional by calibrating the care pressure he or she receives and the difficulty and complexity of what he or she treats, it is easy for the professional to end up burned or saturated. We are seeing it and it is getting more and more.

When you have a serious or potentially dangerous illness, when you are close to death or have had a major accident you need help. In those cases we want to have our loved ones close by and a health team  better as possible near. No one wants to be surrounded by robots alone, even though we are getting more and more around.





新興醫學



在這個從實體社會過渡到流動社會的時代,一切似乎令人困惑。一方面,由於年齡越來越大,健康問題更多的患者變得越來越複雜,而大量的年輕人和中年人的健康問題較輕,或者對敵對的社會和工作環境的適應能力較弱,健康世界因此而遭受驚厥。

Los armarios. The closets. 壁櫥




Todos tenemos miedo. Desde aquel llanto primigenio del bebé se acumulan inevitablemente. Por eso necesitamos los armarios, esos lugares sólidos y oscuros donde los metemos. Da igual que sean de gruesas maderas nobles como los de nuestros abuelos o de aglomerado barato con diseño sueco, al final sirven para lo mismo, quitar de nuestra vista lo que nos atemoriza. 

Es un proceso esencial para la salud mental de todas las personas y un mecanismo de defensa que habitualmente cumple con diligencia su función. Evita una considerable carga de sufrimiento y permite que funcionemos bien a nivel social. El problema, como todo, radica en la proporcionalidad. Cuando usamos el armario para cualquier cuestión o para asuntos cuyo tamaño excede su capacidad nos metemos irremediablemente en pendientes resbaladizas. 

Los primeros en poner el armario encima de la mesa fueron las personas que lograban salir del mismo haciendo pública una opción sexual que previamente habían escondido. Gracias a ellas nos es posible tomar conciencia de una realidad que nos afecta a todos más de lo que nos gustaría reconocer. 

Nos escondemos de los demás pero también, y esto es mucho peor, de nosotros mismos. Ponemos fuera de la vista lo que nos provoca miedo y con ello muchas veces auténticos tesoros como nuestra naturaleza salvaje, los deseos, lo que nos da sentido o la capacidad creativa. 

Como médico tengo la obligación de tener en cuenta estos armarios cuando alguien llega sufriendo a la consulta. Habitualmente sangra por alguno de ellos.  Y no es sencillo hacerlo explícito por cuanto hablamos de algo que por definición está escondido para el que lo padece. Me suelen enseñar sin problemas la gotera, el azulejo roto o la avería aparente pero uno, que ya lleva años en esto, sabe que en muchas ocasiones lo que duele está fuera de la vista. 

Para acercarse a los armarios ajenos es requisito imprescindible hacerlo antes con los propios. No es obligatorio abrirlos todos ni sanearlos por completo. Basta con aproximarse y recordar lo que metimos dentro. Efectivamente nuestra carga interior se aligerará y nuestro nivel de sufrimiento de fondo disminuirá si somos capaces de abrir sus puertas y airearlos. Pero esta cuestión dependerá del grado de evolución humana que cada cual posea. Mientras más rígida sea la propia identidad más cerrados y profundos suelen ser pues más densa es la sombra personal. Por el contrario aquellos con menos ego suelen proyectar sombras más tenues y necesitan por lo tanto armarios más livianos. 

Es increíble lo que la gente mete en ellos, y más increíble aún lo grandes que suelen ser las sombras que estas cosas terminan proyectando. Uno cuando niño puede colgar alguna pequeña vergüenza, una culpa, un error no asumido, y terminar con una enorme limitación vital que le condicione sus relaciones o su forma de vida. 

Por otro lado algunos desaprensivos descubrieron que es posible aventar el miedo que contienen  si se sabe soplar de la forma adecuada, tal y como hace quien quiere encender una fogata. Para vender productos, ideas políticas o modificar voluntades de cualquier tipo suele ser muy útil recurrir a estas técnicas. Y como lo que tenemos en los armarios suelen ser cosas parecidas, no resulta difícil manipular grandes masas de gente con los mensajes adecuados. 

El alto nivel de entretenimiento y ruido de fondo social que hemos creado actúan como narcóticos añadidos al ritmo de vida acelerado. Hoy es mucho más fácil, y generalmente más agradable, conectarse a una pantalla que a uno mismo. No está de moda abrir armarios por más que actualmente no sea tan difícil reconocer una homosexualidad. Hacerlo con ciertas enfermedades o problemas nos sigue resultando un mundo y qué decir de nuestras culpas, errores y miserias. No verán por ahí muchos que vayan reconociendo lo abandonados que tienen a sus padres, lo mal que se llevan con sus hijos o proclamando sus infidelidades de pareja o de cualquier tipo.

Antes se podían abrir los armarios con cierta seguridad en los confesionarios, pero hace tiempo que dejaron de usarse. Ahora la gente que se lo puede permitir paga a un psicólogo o un psiquiatra que además suelen recomendar diversas drogas para hacerlos más tolerables. Pese a todos los avances seguimos en las mismas. Lo que queremos es que no duelan pero de abrirlos mejor no hablar. Dejen nuestra mierda donde está, que si la removemos  el olor será insufrible. Lo que pasa es que cuando rebosan los retretes suele ser inevitable que las aguas negras inunden el salón. 






The closets.

We are all afraid.  From that first cry of the baby we inevitably accumulate fear. That's why we need the closets, those solid, dark places where we put them. It doesn't matter if they are made of thick noble woods like those of our grandparents or of cheap agglomerate with a Swedish design, in the end they serve to do the same thing, to remove from our sight what frightens us. 

It is an essential process for help the mental health of all people and a defense mechanism that usually diligently fulfills its function. It avoids a considerable burden of suffering and allows us to behave properly on a social level. The problem, like everything else, lies in proportionality. When we use the closet for any issue or for matters whose size exceeds its capacity we hopelessly get into slippery slopes. 

The first to put the closet on the table were the people who managed to get out of it by making public a sexual option they had previously hidden. Thanks to them it is possible for us to become aware of a reality that affects us all more than we would like to recognize. 

We hide from others but also, and this is much worse, from ourselves. We put out of sight what causes us fear and with it many times authentic treasures such as our wild nature, desires, what gives us meaning or creative ability. 

As a doctor I have the obligation to take these closets into account when someone comes to the consultation suffering. He usually bleeds from one of them.  And it's not easy to make it explicit because we're talking about something that by definition is hidden from the sufferer. They usually show me easily the leak, the broken tile or the apparent fault but my longstanding experiency says that on many occasions what hurts is out of sight. 

In order to approach other people's wardrobes, it is essential to do it first with your own. It is not obligatory to open them all or to clean them completely. It is enough to approach and remember what we put inside. Indeed, our inner burden will be lightened and our level of deep suffering will diminish if we are able to open their doors and ventilate them. But this question will depend on the degree of human evolution that each one possesses. The more rigid one's own identity, the more closed and deep they tend to be, because the personal shadow is denser. On the contrary, those with less ego tend to cast fainter shadows and therefore need lighter cabinets. 

It is incredible what people put in them, and even more incredible how big the shadows that these things end up casting are. One as a child can hang some small shame, a guilt, a mistake not assumed, and end up with an enormous vital limitation that conditions his relationships or his way of life. 

On the other hand, some unscrupulous people discovered that it is possible to increase the fear they contain if you know how to blow the right way, just like someone who wants to light a bonfire does. To sell products, political ideas or modify wills of any kind is usually very useful to use these techniques. And since what we have in the closets are usually similar things, it is not difficult to manipulate large masses of people with the right messages. 

The high level of entertainment and social background noise that we have created act as narcotics added to the accelerated pace of life. Today it's much easier, and generally more enjoyable, to connect to a screen than to oneself. It's not fashionable to open closets even though it's not so difficult to recognize homosexuality today. Doing it with certain diseases or problems is still difficult for us and much harder if we consider our faults, mistakes and miseries. You won't see many who openly recognize how badly they take care for their parents, how bad they get along with their children or proclaiming their infidelities as a couple or of any kind.

Before, cupboards could be opened with some security in confessionals, but they have long since ceased to be used. Now people who can afford it pay a psychologist or a psychiatrist who also often recommend various drugs to make them more tolerable. In spite of all the advances we continue in the same position. What we want is that they don't hurt but don’t say anything about open them. Society is saying: “Leave our shit where it is, if we remove it the smell will be insufferable”. What happens is that when the toilets overflow it is usually inevitable that the black waters flood the room.

Relato: intento de suicido. Story: Suicide attempt. 故事:自殺企圖









Traía en la mano dos informes nuevos. Uno de urgencias donde tuvo una mala experiencia tras su duodécimo intento de suicidio, otro de la psiquiatra que le volvía a cambiar el tratamiento. La escuché sin interrumpirla durante quince minutos. Se puso nerviosa tras unas horas de ausencia de su pareja y no pudo reprimir el impulso de tomarse todas las pastillas que encontró por casa e inyectarse un vial de insulina. No bastó, nunca basta. Su profunda soledad no halla remedio con esos actos que buscan alcanzar una atención, un afecto, una comprensión que no llega a alcanzar. Y es que en este mundo no es posible. Lo que ocurre es que en algunos casos ese vacío se puede tolerar y en otros no. Por eso la dije que no la juzgaba, ¿cómo condenar algo que yo mismo en esa circunstancia haría el primero?

Lo complicado para mi es conseguir que una persona completamente abducida por el torbellino del síndrome de una vida de mierda sea capaz de darse cuenta de que el infierno que la rodea no es real. Ser capaz de recordar que toda tormenta es impermanente, que toda soledad o abandono son espejismos. Una abeja se puede sentir sola en el panal, pero su sentimiento no es más que una hoja que cae del árbol en otoño, algo que se llevará el viento. Los seres humanos tenemos la capacidad de magnificar y absolutizar ese viaje de la rama al suelo, pero pese a nuestra tenacidad, todos los otoños pasan por más que hagamos incontables esfuerzos para evitarlo.

También compartí mi sentimiento de dolor dejando claro que su gesto me afecta, si ella muere yo de alguna forma muero con ella, como lo haremos todos los que la conocemos. Nuestros actos incluyen de una u otra forma a los demás, por eso considerar al resto de seres vivos ajenos a nosotros nos ciega por completo. La naturaleza nos dota de una identidad que nos hace sentirnos únicos y separados, ahí radican las raíces de muchos sufrimientos. 

Comprobé que el tratamiento antidepresivo actual estaba actualizado en la historia clínica electrónica. Eché de menos poder recomendar una buena dosis de tambor, de baile, de canto chamánico a la luz de una buena hoguera bajo un manto de estrellas. Pero eso los médicos modernos no lo podemos hacer. Como tampoco hacer brotar el sentido de una vida que parece perderlo por momentos. Contemplo esa rigurosa impotencia y la convierto en narración apelando a una magia antigua que han manejado los sanadores de todos los tiempos. Si algo puede ser contado, puede ser transformado. De esta forma encarno en primera persona uno de los remedios que más prescribo: ayudarnos del verbo para poner en palabras nuestras sombras. 




Story: Suicide Attempt

She had two new reports in her hand. One from the emergency room where she had a bad experience after his twelfth suicide attempt, another from the psychiatrist who was changing her treatment again. I listened to her without interrupting for fifteen minutes. She became nervous after a few hours of absence from her partner and could not suppress the urge to take all the pills she found at home and inject herself with a vial of insulin. It was not enough, it was never enough. Her deep loneliness is not remedied by those acts that seek to achieve an attention, an affection, a comprehension that does not reach. It is not possible in this world. What happens is that in some cases this emptiness can be tolerated and in others it cannot. That's why I told her that I didn't judge her, how can I condemn something that I myself would do in that circumstance?

The complicated thing for me is to get a person completely abducted by the whirlwind of a shitty life syndrome to be able to realize that the hell that surrounds her is not real. To be able to remember that every storm is impermanent, that every loneliness or abandonment is mirage. A bee can feel alone in the honeycomb, but its feeling is nothing more than a leaf that falls from the tree in autumn, something that will be blown away by the wind. Human beings have the ability to magnify and absolutize that journey from the branch to the ground, but despite our tenacity, every fall goes through away though we make countless efforts to prevent it.

I also shared my feeling of pain by making it clear that her gesture affects me, if she dies I die with her in any way, as all of us who know her will. Our actions include others in one way or another, so considering other living beings alien to us completely blinds us. Nature endows us with an identity that makes us feel unique and separate, there lie the roots of many sufferings. 

I found that the current antidepressant treatment was updated in the electronic medical record. I missed being able to recommend a good dose of drumming, dancing, shamanic singing in the light of a good bonfire under a blanket of stars. But modern doctors can't do that. Nor can we make the meaning of  life sprout in a person that seems to lose it. I contemplate this rigorous impotence and turn it into a narration appealing to an ancient magic that has been handled by the healers of all times. If something can be told, it can be transformed. In this way I incarnate in first person one of the remedies that I most prescribe: to use language to put our shadows into words.

Sigo dándole vueltas al regusto que me ha quedado en el paladar después de leer la novela Alegría de Manuel Vilas. Como conflicto de intereses he de decir que es uno de mis autores favoritos y que su forma de narrar es una de las más originales del panorama literario contemporáneo. No tengo ninguna duda de que esta obra será otro éxito internacional como la anterior, y lo hará por méritos propios, además de buena literatura su narrativa es sanadora. Y lo es por varios motivos. El primero por ser un testimonio autobiográfico de que la vida es dura, en ocasiones terrible, y aun así tenemos la capacidad de seguir adelante. El síndrome de una vida de mierda existe, y cuando nos alcanza nos sumerge en el infierno más profundo. Y ese momento nos llega a todos los mortales, las crisis vitales son inherentes a la vida. El sufrimiento puede surgir por múltiples frentes entre los que el autor elige dos: sus padres muertos y sus hijos vivos. Desde esta doble perspectiva de relaciones verticales y biológicas nos ofrece su experiencia de muerte y resurrección dado que los muertos que una vez amamos no desaparecen nunca y los vivos que amamos y se alejan tampoco. La novela toca tangencialmente las relaciones horizontales de amistad y pareja sin perder el foco en lo anterior que es lo que el autor nos quiere contar. Al final todo es una búsqueda de sentido y es por eso por lo que esta obra produce esperanza en quien la lee. Es posible convivir con los propios fantasmas y seguir buscando la alegría. 

Otro mérito de la novela es la forma de expresar la relación del autor con su sufrimiento al que decide llamar Arnold. Vilas camina por su mundo interno poniendo un nombre nuevo a todos aquellos que son importantes para él y su fantasma personal no podía faltar. Esto me parece clarividente en un mundo que nos invita a encerrar en profundas mazmorras todo lo que nos disgusta. Los demonios personales solo se pueden vencer cuando tenemos el valor de sacarlos a la luz como nos llevan diciendo los psicólogos desde tiempos pretéritos. Convertir el sufrimiento personal en narrativa sigue siendo una de las medicinas más potentes que conozco y este libro es un ejemplo. Incluso cuando no tenemos nadie que nos escuche es posible plasmar en un papel aquello que nos aterra o atormenta. Y aunque no tengamos el talento del autor, ese paso de valor nos ayudará a que en la sombra amenazante de nuestra crisis vital brille la frágil cerilla de quien pone en palabras su oscuridad.

La narrativa es una manera maravillosa de recrear mundos y permitirnos escuchar y aprender de distintos autores. Cumple una función de entretenimiento y en ocasiones de inspiración, enseñanza y regocijo. Esta obra ayuda a mirar sin miedo las relaciones paternofiliales, a recordar lo importantes que son, a animarnos a cuidarlas. En un mundo convulso y despistado es bueno que alguien nos invite a rescatar lo importante y a poner la vista en algo tan bello como la alegría.





The literature of Manuel Vilas as medicine


I keep thinking about my aftertaste after reading the novel Alegría by Manuel Vilas. As a conflict of interests I have to say that he is one of my favourite authors and that his writting is one of the most original on the contemporary literary scene. I have no doubt that this work will be another international success like the previous one, and will do so on its own merits, as well as good literature its narrative is healing. And it is for several reasons. The first is because it is an autobiographical testimony that life is hard, sometimes terrible, and yet we have the ability to move forward. The syndrome of a shitty life exists, and when it reaches us it plunges us into the deepest hell. And that moment comes to all mortals, life crises are inherent to life. Suffering can arise on multiple fronts from which the author chooses two: his dead parents and his living sons. From this double perspective of vertical and biological relations, he offers us his experience of death and resurrection, given that the dead that we once loved never disappear and the living that we love and move away either. The novel touches tangentially the horizontal relations of friendship and couple without losing the focus on what the author wants to tell us. In the end everything is a search for meaning and that is why this work produces hope in those who read it. It is possible to live with one's own ghosts and to continue searching for joy.



Another merit of the novel is the way of expressing the author's relationship with his suffering which he decides to call Arnold. Vilas walks through his inner world giving a new name to all those who are important to him and his personal ghost could not be missing. This seems to me clairvoyant in a world that invites us to shut up in deep dungeons everything that we dislike. Personal demons can only be defeated when we have the courage to bring them to light, as psychologists have been telling us since ancient times. Turning personal suffering into narrative continues to be one of the most potent medicines I know and this book is an example. Even when we have no one to listen to us, it is possible to put on paper what terrifies or torments us. And even if we don't have the author's talent, this step of courage will help us to shine in the threatening shadow of our vital crisis the fragile match of the one who puts his darkness into words.



Narrative is a wonderful way of recreating worlds and allowing us to listen to and learn from different authors. It fulfils a function of entertainment and at times of inspiration, teaching and rejoicing. This work helps us to look fearlessly at paternal-child relationships, to remember how important they are, to encourage us to take care of them. In a convulsed and absent-minded world, it is good that someone invites us to rescue what is important and to set our sights on something as beautiful as joy.

Alegría, de Manuel Vilas. Una lección de vida.



Nunca ha sido tan necesaria la literatura como hoy. En esta época de gritos y sinsentido, de prisas y consumo, necesitamos narraciones que nos ayuden a entender qué está pasando. Manuel Vilas lleva años ayudándonos, más si cabe en sus últimos libros, Ordesa y Alegría, en los que consigue articular una forma de expresión que consigue tocar el corazón de los lectores. Lo hace compartiendo su carne y su sangre, de esa forma que solo es posible a los resucitados. Porque él resucitó de entre los muertos al traspasar su profundo hundimiento. Vilas nos cuenta sus evoluciones por hoteles y ciudades como Dante lo hizo en su dia de la mano de Virgilio. Literature has never been so necessary as it is today. In this age of shouting and nonsense, of haste and consumption, we need narratives that help us understand what is happening. Manuel Vilas has been helping us for years, even more so in his latest books, Ordesa and Alegría, in which he manages to articulate a form of expression that touches the hearts of readers. He does so by sharing his flesh and blood, in such a way that it is only possible for those who have been resurrected. Because he rose from the dead by piercing their deep sinking. As they emerge from hell, the reborn acquire a new radiance, a radiance that attracts all those who still walk in darkness. Estos 50 consejos se derivan de 50 años de aprendizaje de mis propios pacientes, que siempre fueron también mis mejores profesores.

1. Hay muchos tratamientos diferentes, no hay uno que se adapte a todos.

2. Muchos tratamientos tienen una base sólida de estudios científicos que los apoyan, pero algunos no la tienen. Siempre que sea posible, se prefieren los tratamientos basados en la evidencia.

3. Los tratamientos psiquiátricos tienen un nivel de eficacia similar a los tratamientos en el resto de la medicina.

4. Los resultados son muy variables e inicialmente impredecibles. La mayoría de las personas experimentan al menos algún beneficio del tratamiento psiquiátrico, algunas personas se recuperan completamente, una minoría no tiene respuesta y unas pocas sufren daños.

The best tips for patients with psychiatric problems



These 50 tips are derived from 50 years of learning from my own patients, who were always also my best teachers.

1. There are lots of different treatments—no one size fits all.
2. Many treatments have a strong base of supporting evidence, but some do not. Whenever possible, evidenced-based treatments are preferred.
3. Psychiatric treatments are about as effective (but also as ineffective. as treatments in the rest of medicine).
4. Outcomes are very variable and initially unpredictable. Most people experience at least some benefit from psychiatric treatment, some people have full recovery, a minority have no response, and a few are harmed.
5. The success of treatment depends on the person; the nature of the problem; the accuracy of diagnosis; the type of treatment; clinician skill and flexibility; patient effort; the strength of the therapeutic relationship; and the presence or absence of good luck.
6. The choice of optimal treatment depends on the person; the nature of the problem; its severity; patient preference; clinician training; availability; and the results of serial systematic trials to see what works best.
7. “No Treatment” may also be the treatment of choice—especially for people with normal, expectable problems of everyday living and also for those who have had no or bad responses to previous treatments.
8. For mild symptoms, watchful waiting, the healing powers of time, reduction of stress, and support from friends and family may be all that is needed.
9. In contrast, severe/persistent symptoms require immediate attention—the longer you wait, the harder they may be to treat and the slower and less complete the treatment response.
10. Good clinician/patient match is crucial to good outcome. If possible, interview several clinicians before picking the one you are most comfortable with. If choice permits, always avoid clinicians who don’t feel right for you.
11. Don’t be too ashamed to seek treatment—half the population has substantial symptoms at some point in their lives and any stigma of being in psychiatric treatment has mostly evaporated.
12. Be an informed and skeptical consumer—use the internet (though don’t believe everything on it); have probing questions prepared before visits; and expect good common sense answers. Second and third opinions also often help in good decision-making.
13. Put your heart, mind, and guts into the treatment; the more you put into it, the more you will get out of it.
14. Psychotherapy is usually the best treatment for mild to moderate problems and may also help people in managing more severe ones.
15. Psychiatric medication is essential for most severe problems; often necessary for moderate ones; and to be considered whenever symptoms persist and don’t respond to psychotherapy.
16. There are many effective specific psychotherapy techniques—cognitive; behavioral; interpersonal; psychodynamic; family systems; and group.
17. The best psychotherapists know most or all of the many techniques and apply them flexibly as needed by a particular patient at a particular moment.
18. A strong therapeutic alliance and good clinician/client personality match-up may be more important than the specific techniques used, so pick someone you like and want to work with.
19. Most psychotherapies are brief (5 to 12 sessions); goal oriented; and problem focused. Long-term treatment may be necessary for more persistent problems or more ambitious goals.
20. Most of the beneficial effect of psychotherapy comes from how well you use what happens within the sessions in your life outside the sessions. Expect homework from the therapist and figure out on your own how you can enhance your skills and stretch your boundaries based on what you are learning in therapy.
21. It is important to give psychotherapy a fair chance, but if it is not working, try something else or someone new.
22. Psychiatric medications are currently very overused—20% of the population is taking one; 12% are on antidepressants, including 25% of women over 40 and 4% of teens; 4% are taking a benzodiazepine, including 8% of the elderly (where they can cause falls, memory loss; confusion); 6% of kids are on stimulants. Antipsychotics are very overprescribed, especially in poor kids and seniors in nursing homes (where they shorten life expectancy).
23. Approximately 80% of psychiatric medications are prescribed by primary care doctors who have very little time to get to know their patients, receive very little training in psychiatry, and are often under undue pressure from patients wanting immediate relief as the outcome of a first visit.
24. It is easy to begin a psychiatric medication, but often hard to stop one. The decision whether to start should require careful thought and often an extended period of watchful waiting or psychotherapy before prescribing.
25. The placebo response rate combines the effects of time; expectation; and the fact that most symptoms are a response to a specific stress and remit spontaneously. Placebo response is very high (50% or more) for milder problems; very low (less than 10% for severe ones).
26. If you begin a medication for a transient problem, your positive response to it is much more likely due to placebo effect than to any active ingredient in the drug. But you can’t tell the difference; many will likely give undue credit to the medication for relief; and many (for purely magical, and no real) reasons stay on it for the long term. This is an important reason for medications to be a last resort, not a first reflex, for mild to moderately severe problems.
27. Drug ads emphasize benefits and minimize risks.
28. Patience is important. It takes time for medications to work; don‘t jump from med to med before giving each a fair chance at proper dose and duration. Systematic trials are the only way to figure out what works and what doesn’t.
28. Medications should be used carefully and only for specific indications. All drugs have adverse effects, and some have rare but serious complications.
29. The sacred principle of beginning medication should be “start low/go slow.”
30. Many psychiatric drugs have withdrawal effects. The sacred principle of stopping a medication should be a very slow taper. Medications should never be reduced suddenly or without medical supervision—especially benzodiazepines.
31. Except for certain specific situations (eg, bipolar disorder), polypharmacy usually does more harm than good. The drugs may interact negatively with each other and cause combined adverse effects. When new medications are gradually added, old ones should usually be gradually withdrawn.
32. Medications are essential, sometimes life-saving, for the severe forms of most psychiatric disorders.
33. Three difficult-to-give treatments (lithium, clozapine, and ECT) are especially useful for some special indications, but they are under-utilized, precisely because they are difficult to give. ECT would be my choice for me if I had a severe depression.
34. The outcome of severe psychiatric disorders varies widely, from crippling to just annoying inconvenience. Some of this is luck and clinician skill but a large part is determined by how well you participate in the treatment and learn how best to manage your symptoms and life problems.
35. Access to, and funding for, mental health treatment is shamefully inadequate in the United States, particularly for those suffering with the most severe disorders and with the fewest family resources. It breaks my heart that 600,000 people with severe mental illness are relegated to jail or homelessness for callous want of available treatment and affordable housing.
36. Always get help when you have suicidal feelings—sharing them with others greatly reduces risk and will help you feel better. Don’t be ashamed and don’t feel you have to struggle on your own.
37. Prepare an action plan when you are not suicidal listing the coping strategies and supports you can access immediately whenever you do become suicidal.
38. Know that if you do succeed in dying by suicide, it will haunt your family for decades—not (as you probably incorrectly believe) relieve them of a burden.
39. Suicidal impulses peak during periods of stress or disappointment and usually lessen dramatically in just a few days or weeks. It is a great mistake to make an eternal “life vs death” decision based on how you feel on one of your worst days.
40. Suicide is almost always a mistake. Most people who survive a suicide attempt are very grateful to still be alive.
41. Alcohol and street drugs greatly amplify all the risks of having a psychiatric disorder and greatly increase the risks of suicide and violence.
42. Patients who have combined psychiatric and substance problems require simultaneous treatment for both.
43. Be sure not to own or have any access to guns.
44. Don’t have perfectionist expectations about life. It certainly has its bad bounces—but remember that life is also implausibly beautiful and precious.
45. Don’t have perfectionist expectations about treatment. It is only sometimes completely curative, but usually helps a lot, and can always provide comfort and support.
46. Be resilient. Sure, it’s unfair to be burdened with psychiatric suffering, but it is not the end of the world, and almost always is manageable, compatible with a good life, and likely to get better with time and treatment.
47. Don’t be demoralized if treatments seem not to work. It sometimes requires many trials of different treatment methods and/or different treaters to find the right fit.
48. Don’t give up hope. Sooner or later most people are either relieved of their symptoms or learn to cope with them well.
49. Become expert in what psychiatric, health, social, insurance, and housing services are available in your area and how best to access them. You will probably have to become an expert in wrangling bureaucracies to get the services you need. And sadly, many people will never get the help they need because public neglect has made it unavailable or because cynical insurance companies make treatment inaccessible.
50. Be a vocal advocate for better services for people with mental illness. Politicians respond only to squeaky wheel constituents. Patients with severe mental illness have been ignored in part because they have felt shamed and have accepted neglect. Similarly, NIMH has a research agenda that seems to ignore patients’ practical needs and instead privileges scientists’ pet projects. Speak up, be heard, join patient advocacy groups.
