Chapapote sanitario. Health care asphalt stain.

After ten days of strike in Madrid by paediatricians and family doctors, we are exhausted. In the days of the "movida" they used to say "Madrid kills me" because of the intensity of the pace of life and the excesses of the night. Today we can take it up again, given that the current situation and the passivity of the Regional Ministry of Health are going to do away with a large number of doctors, and this is not rhetoric. The sustained work overload is toxic, but the narrative of decadence we have in our heads is much more so. The healthcare dystopia we are witnessing is leading us to the end of the days of primary care as we knew it, and many cannot help but think of a final judgement of weeping and gnashing of teeth. Thoughts of ruin and feelings of failure and despair stick to us like that thick, black tar that stained the Galician coast a few years ago. Dreams are contaminated by that smell and our friends and loved ones see us crestfallen and overwhelmed.

The situation is a vital catastrophe for many of us, similar to what our patients present to us every day, whom we discharge and medicate on many occasions due to levels of suffering similar to our own. But we cannot go on like this, not for long. Many of us are already on medication and many others are in therapy, and soon there will be more. It is not sustainable. We need a new narrative, some good news, something to give us a little hope. Maybe some people are helped by their beliefs or their faith. But here we need something that we can all share, a path that will take us out of this infernal circle of complaint and rottenness that never rests.

We need silence. Courage to walk towards the empty and silent desert that each one of us harbours within ourselves. And there to listen. Usually, if we manage to insist, we end up hearing a faint inner voice that comforts us, the same one heard by mystics, artists and madmen. In the midst of so much shouting it is very difficult, but just as our patients manage to get out of their troubles, there is also salvation for us.

The situation is so hard for many that I am trying to systematise what I am summarising in these lines in a broader text and in a workshop which, if the elements permit, I will give with Dr Isabel Núñez at the Semfyc Communication Congress in a few months' time. Talking about health scatology and vital catastrophes may scandalise some, but the time has come to dare to take new approaches, given the difficulties and limitations of the different sectoral roundtables, where the Administration is usually the one who scores the goals by cheating to a greater or lesser extent.

El bloqueo. The blockade. 封锁

The blockade

First week of strike of family doctors and paediatricians in Primary Care in Madrid. In my case, minimum services covering the needs of my town in the morning shift alone. More of the same. In the case of my colleagues in large centres, two minimums per shift with a good following. In the case of the negotiating table: deadlock.

In general we are all blocked in thought, word, deed and omission. In thought because the positions are very far apart. On the one hand, the administration defends that its management is impeccable but it is enough to see a video of the president of the Community from a few years ago promising the gold and the silver lining to see that not much of what was said has been fulfilled. On the other hand, the union that defends the strike asking for a budget and less overload, despite the enormous support of the doctors, does not have a plan to implement improvements while there are no more resources.

We need mediators. You can't imagine how, given that the problem of European public health is complex, the Spanish one is the weakest and primary care is the canary in the mine.

Resources are needed, yes. This requires the groups to propose advances in their electoral programmes and to take it seriously. It also requires policies to be implemented at the national level that defend the human resources for health in this country so that they do not continue to be damaged and migrate when they can.

Other things are also needed. If we assume that the model is sick, perhaps it is time to try alternatives that go in the direction of self-management of small teams. Perhaps we can open the door to non-EU doctors to fill hard-to-fill positions, reinforcement positions or vacant points of continuous care. Perhaps we can open our hands to reform and strengthen primary care nursing, allowing the nurse prescription of a catalogue of basic drugs to make it easier to manage minor health problems that come to the health centre without an appointment.

The main problem, as I say, is the blockage. Each health collective has its own interests and their union representatives prioritise them without considering the common good. Nor has it so far been possible to create forums or platforms that bring together the voices of the different professionals and look after the good of all. This is why the administration cannot have a credible roadmap, choosing its path in fits and starts according to the difficulties encountered. Imagine if this were to happen on a boat... at the moment it is the rowers who are shouting, because they have consultations that in many cases double or triple the number of patients seen by family doctors in other countries, hospital specialists or even the nurse in the consulting room next door. But everyone knows how to shout and at some point ends up doing so. Will the Regional Ministry be wise enough to discern the weight of these or will it act according to electioneering criteria, you can sense it as well as I can.

What the average citizen may not be aware of is that the damage that paediatricians and family doctors have been carrying in their bodies since the pandemic is immense, because we were already affected beforehand. This damage means that most of us are taking psychotropic drugs to cope or are going to therapy or seeking help. This has never happened before at this level. There have been many years of unbearable overload in which we have seen how the few substitutes we had or the scarce reinforcements in rural areas in the summer have gone up in smoke.

And the worst thing is that they know it. The managers know what is happening with the residents who abandon the speciality of Family Medicine or go elsewhere. They know the level of discomfort and pain in the most overburdened centres. They know the number of doctors who are sick for this reason. They know that in five years 25% of the medical staff will retire. But they look to the ground, to the next elections, not beyond. And the managers who dare to do so are being killed, just look at the list of resignations in recent years, which is very illustrative. There is a responsibility in the management apparatus of Primary Care, hundreds of jobs that are not defending their care brothers. There is a responsibility at the senior management level who unquestioningly accept whatever the politician of the day imposes on them without drawing any red lines. There is a responsibility on the politicians whose discourse is "lower taxes" instead of prioritising tax management. And there is a responsibility on citizens to demand decent healthcare from their representatives. They will have to do so by taking to the streets, sending letters, going to talk to mayors or parties, organising in the neighbourhoods, informing themselves about what is happening in their locality, and of course when it comes to voting.

To bring a patient out of cardiac arrest you have to do things. We can't just stand by while he or she shuts down. The same thing happens here, and we have been warning for many years, the patient already has a bad colour.

The solution for Primary Care in Madrid.

Foto: Fernando Villar (EFE)

The solution for Primary Care in Madrid.

The problem of Madrid's primary care cannot be solved in a sectoral round table where the administration and a trade union are seated. It is too complex. I will try to simplify it in two minutes:

1. After years of underfunding the workforce of paediatricians and family doctors is overstreched.

2. Young resident doctors are leaving Madrid, and so are the older doctors who are able to do so.

3. In five years 25% of the current family doctors will retire.

4. The overload is immense: long-term unfilled vacancies (20% of the entire staff) + absences due to leave and IT (9%). This means that in addition to a large patient schedule, an average of 30% more work has to be taken on permanently, with peaks of varying duration.

5. The figures for burnout and psychological discomfort among professionals are higher than ever.

Consequence: the situation is unsustainable.

What can be done?

1. Assume what is there. The situation is critical and it is essential that both professionals and the public accept it.

2º. Ask the political parties in Madrid to make budgetary and organisational proposals.

3ª. To propose a debate in the Assembly of Madrid that seeks a minimum consensus.

4º. Ask the national political parties and the government for a strategy to strengthen primary care, including a budget allocation.

Family doctors have been losing out for years (mental health) and citizens are going to lose more and more (disadvantaged villages and neighbourhoods without doctors, longer waiting lists, worse service).

The time has come to accept that primary care is an essential basic service that requires priority funding or to accept that as a society we are running out of paediatricians and public family doctors.

Marc Chagall

Huelga con calcetines verdes. Strike with green socks.

.

Hoy me he puesto unos calcetines nuevos para protestar.

 Strike with green socks.

Today I put on new socks to protest. They are green. They will say that this is an absurd way to go on strike, but I have no choice. In my rural practice we are one paediatrician and three family doctors. The paediatrician resigned months ago and her post has not been filled, my morning colleague is on leave all week, without a substitute. We are left with my afternoon colleague and myself as minimum service...

Doctors' strikes are an oxymoron, which is why we don't like them at all. They usually result in fine words and agreements that the administration does not usually fulfil, because the situation is too complex to be solved in meetings with the unions. We need more political will and more interlocutors, both professional and civil society.

That is why I silently protest with my green socks and starting Joanna Macy's book, Active Hope. We are going to need it.

Malestar sanitario. Healthcare malaise.

Healthcare malaise

Madrid is once again suffering another health crisis, this time not because of a catastrophe but because of bad governance. Let me summarise the situation in one paragraph. As you will remember, in the middle of the pandemic, Mrs Ayuso decided to open a pandemic hospital, but as she could not find any staff, she mobilised doctors and nurses from the capital's primary care emergency services and closed them. A few weeks ago she decided to open them and as she could not find any staff, she mobilised doctors and nurses from the rural emergency centres. Without dialogue or any care, moving professionals with a "permanent position" won in public examination and in this plan. As a result, the rural A&E centres have been destroyed and the urban ones are at minimum performance due to the resignation and flight of professionals.

The malaise of the health workers is enormous. Because of the disregard, the mismanagement and the outrage that some have suffered with the latest outrage. But the whole health system is a single entity. If primary care emergencies are damaged, the rest of primary care is indirectly damaged by the consultations that should have been attended in those emergencies and hospital emergencies are damaged by saturating them. If a health centre is saturated due to overload, it saturates the reference hospital... the law of communicating vessels, as you will remember from GBS.

And the discomfort of the citizens is also important. Those who live in villages see that from one day to the next they are more unprotected, the rest suffer the progressive increase in waiting lists to be seen in their health centre or in the corresponding hospital consultation. Not everyone suffers in the same way, in wealthy neighbourhoods they still have an appointment for their family doctor the next day and if they need a specialist they have a private doctor nearby (in Madrid 38% have private medical insurance).

What no one likes is the fact that the debt of the subsidised hospitals continues to grow (there is talk of 1 billion). They are the ones who are making money from the increase in waiting lists in conventional hospitals and the deterioration of health centres and primary care emergencies. The Quirón group charges a good price for any assistance to patients from other areas, and they are increasingly charging more and more.

Tomorrow, Sunday 13th, a large demonstration has been called, followed by an indefinite strike in health centres on Monday, in addition to the one in Primary Care emergencies (with 100% minimum services). Obvious signs of unrest. Unfortunately, neither of these two initiatives will correct the underlying problem: primary care is in a critical state, with no young people wanting to stay and many older people wanting to leave. And that doesn't affect the politicians of the day. They will fix the odd chipping, put some paint on and carry on whistling.
