Aprender del fracaso de la Atención Primaria. Learning from the drowning of Primary Care. 從基層醫療失敗中學習





Estos días unos compañeros hicieron una manifestación en Twitter anunciando (de nuevo) el fallecimiento de la Atención Primaria en una muestra de dolor por la situación de penosidad que sus profesionales llevan sufriendo demasiado tiempo. 


La pandemia parece haber dado la puntilla a un sistema que ya estaba en las últimas y que incluso con la que está cayendo nadie parece querer cambiar, reforzar,  redimensionar o mejorar. 


Quizá podamos plantear la posibilidad de contemplar lo que tenemos delante y aprender de ello. Es la historia de un fracaso, es cierto, pero también lo es que hay dignidad en ese hundimiento como la ha habido en otros muchos antes. 


Probablemente sea correcto decir que en este asunto hemos fallado todas las partes implicadas. Los profesionales por no haber sabido hacernos valer, los gestores por haber maltratado a los anteriores, los políticos por no haber querido invertir ni priorizar el servicio y los ciudadanos por haber sobreusado una sanidad que bien es cierto se ha dejado sobreusar. 


Y lo malo de esta historia es que si una institución pública se malogra ya saben ustedes lo que pasa, el mercado termina abduciéndola e imponiendo sus reglas. En el caso sanitario va a haber muchos perdedores, en concreto la totalidad de los actores excepto los accionistas de los servicios privados que terminen haciéndose con el mango de la sartén. 


Tengo cerca grandes profesionales que están sufriendo como nunca antes lo habían hecho con un día a día insufrible por la sobrecarga, la pérdida de horizonte y la desesperanza. Sin liderazgo, sin gestores comprometidos, sin salida a la vista. Tengo cerca una sociedad que está recibiendo de lo lindo. Ni unos ni otros son conscientes de que pese a la samanta de palos que reciben lo peor es un tren que se acerca a gran velocidad, como bien decía el filósofo Žižek hace unos días en relación al cambio climático. Pero no nos pongamos trágicos, estábamos hablando de aprender y para esto vamos a tener que mirar donde más duele, a los mimbres de una sociedad cada vez más depauperada, individualizada y dependiente. Y viendo la herida hacernos preguntas, ¿cómo curar tanta complejidad? ¿Quién debe meter el bisturí para desbridar las partes muertas? ¿Quién proveerá cuidados? ¿Por dónde empezar?...


Tras una mañana atendiendo a una multitud de pacientes con covid y sus correspondientes contactos no tengo la seguridad de haber actuado bien con algún caso. A nadie importarán mis desvelos, ni agobios. Y eso quizá sea lo más dramático, por que quizá llegue un día en que a ningún sanitario le merezca la pena desvelarse. 











Learning from the drowning of Primary Care



A few days ago, some colleagues made a demonstration on Twitter announcing (once again) the demise of Primary Care in a show of grief for the situation of hardship that its professionals have been suffering for too long. 


The pandemic seems to have put the final nail in the coffin of a system that was already on its last legs and which, even with the current crisis, nobody seems to want to change, reinforce, resize or improve. 


Perhaps we can consider the possibility of looking at what is in front of us and learning from it. It is the story of a failure, it is true, but it is also true that there is dignity in this collapse as there has been in many others before. 


It is probably correct to say that all parties involved have failed in this affair. The professionals for not having known how to assert ourselves, the managers for having mistreated the previous ones, the politicians for not having wanted to invest or prioritise the service and the citizens for having overused a healthcare system which, it is true, has allowed itself to be overused. 


And the bad thing about this story is that if a public institution fails, you know what happens: the market ends up abducting it and imposing its rules. In the case of healthcare, there are going to be many losers, in particular all the actors except the shareholders of the private services that end up getting the handle of the frying pan. 


I am close to great professionals who are suffering as never before with an insufferable day to day life of overload, loss of horizon and hopelessness. Without leadership, without committed managers, with no way out in sight. I am close to a society that is on the receiving end. Neither one nor the other is aware that despite the samanta of blows they are receiving, the worst is a train that is approaching at great speed, as the philosopher Žižek rightly said a few days ago in relation to climate change. But let's not get tragic, we were talking about learning and for this we are going to have to look where it hurts the most, at the fabric of an increasingly impoverished, individualised and dependent society. And looking at the wound we ask ourselves questions: how to heal so much complexity? Who should put the scalpel in to debride the dead parts? Who will provide care? Where to start?....


After a morning of caring for a multitude of covid patients and their corresponding contacts, I am not sure if I have done well with two of them. No one will care about my sleeplessness, no one will care about my stresses and strains. And that is perhaps the most dramatic thing, because perhaps there will come a day when it will not be worthwhile for any doctor worring about.

“A la mierda” "To hell with it" “去死吧”






La historia se repite, eso lo sabemos. Sin embargo es duro asumir que lo haga tan rápido. Vamos a terminar enero como en los peores momentos de la primera ola del año pasado y eso va a pesar a muchos, en especial a los sanitarios de primera línea como buenos costaleros que somos de esta Semana Santa nacional que estamos viviendo. No es plato de gusto ver como las cifras empeoran cada semana, como en los centros de salud no paramos de diagnosticar nuevos casos, como los hospitales se van llenando de enfermos y las UCIs también.


El año pasado nos pilló desprevenidos, no teníamos ni siquiera mascarillas ni equipos de protección. Este vamos regular dado que la vacunación sigue un ritmo más lento que un virus al que el invierno da alas. Al final tendremos confinamiento duro, llevo tiempo diciéndolo. Mientras tanto la ciudadanía se echa las manos a la cabeza por lo que significa esa posibilidad que no quiere ver ni en pintura y los sanitarios también por ver retrasado lo que para nosotros es evidente. No hay sanidad que pueda soportar una pandemia desbocada mientras no se limite drásticamente la movilidad. Los políticos tendrán que plantear límites para evitar las llamas tal y como pasa en el Reino Unido donde el confinamiento es ya un hecho. 


A mi nivel de centro de salud voy viendo que el grado de mosqueo general sigue aumentando al no verse correspondidos los esfuerzos que llevamos meses desarrollando con una carga de trabajo descomunal y un nivel de estrés y preocupación extraordinario con el comportamiento social de nuestros conciudadanos y mucho menos con el de nuestros dirigentes y responsables directos. Es verdad que mucha gente es responsable e incluso se pasa de frenada con un autoconfinamiento que ya lleva meses. Pero también lo es que otros hacen de su capa un sayo y no atienden razones. Conductas de riesgo en hostelería, reuniones caseras multitudinarias, fiestas a go go, discotecas, encuentros varios, fumeteo grupal sin mascarilla, mal uso de las mismas y una larga lista de funestas posibilidades. Quizá la peor no confinarse ante síntomas catarrales o ante un contacto estrecho. “Total a mí no me va a tocar” piensan muchos. Pues al final toca, claro que toca. Y le toca a los más mayores, los más débiles, los más vulnerables. Toca también a los adolescentes y jóvenes, toca a los niños y niñas, a trabajadores, a estudiantes, a ricos y pobres. Miles cada semana con premio seguro, miren los datos y gráficas que tienen delante. Toca. 


Por eso me viene a la cabeza aquel enfado de Fernando Fernán Gómez que más de uno se habrá quedado con ganas de verbalizar alguna vez. Que una sociedad permita que sus sanitarios acaben odiando lo que hacen y a sus pacientes no es inteligente. En el Reino Unido lo saben, ¿y aquí?.







"To hell with it"





History repeats itself, we know that. However, it is hard to assume that it will do so so quickly. We are going to end January as in the worst moments of the first wave of last year and that is going to weigh on many, especially the frontline health workers as good costaleros that we are of this national Holy Week that we are living. Politicians will have to set limits to avoid the flames as is happening in the UK where confinement is already a fact of life. 


At my level as a rural health centre, I can see that the degree of general annoyance continues to increase as the efforts we have been making for months with an enormous workload and an extraordinary level of stress and concern about the social behaviour of our fellow citizens, let alone that of our leaders and those directly responsible, are not being reciprocated. It is true that many people are responsible and even go overboard with a self-confinement that has been going on for months. But it is also true that others are making a fool of themselves and are not listening to reason. Risky behaviour in the bars and catering industry, large house gatherings, parties, discos, various meetings, group smoking without masks, misuse of masks and a long list of disastrous possibilities. Perhaps the worst is not to confine oneself when faced with catarrhal symptoms or close contact. Many people think, "It's not going to touch me anyway". Well, of course it does, of course it does. And it is the oldest, the weakest, the most vulnerable. It also touches adolescents and young people, children, workers, students, rich and poor. Thousands every week with a sure prize, look at the data and graphs in front of you. It touches. 


That's why Fernando Fernán Gómez's anger comes to mind, something that more than one of you may have wanted to say at some point. For a society to allow its healthcare professionals to end up hating what they do and their patients is not intelligent. In the United Kingdom they know it, but what about here?

Una vacuna de esperanza. A vaccine of hope. 希望的疫苗。




Estos tiempos complejos me están enseñando la importancia de depositar la atención en lo que verdaderamente lo merece. Con tanta crispación y tanto ruido de fondo va a ser cada vez más importante no caer en la tentación de atender lo que más grita, más se ilumina, o más intenta a atraernos, sino en la virtud de lo callado, casi invisible o delicado. Viene a mi mente aquel relato del profeta Elías en el libro primero de los Reyes que consigue encontrar la presencia de Dios en una brisa suave y no en el fragor del huracán, la violencia del terremoto o el brillo del rayo en mitad de la noche. 


Nosotros hemos dejado de oír la tenue voz divina en un mundo perpetuamente atenazado por pantallas que no dejan de vomitar todo tipo de escándalos, desgracias y cataclismos. Y mira que buscamos en el móvil cientos de veces al día con regulares resultados. 


Lo que quería traerles esta semana son tan solo dos pequeños detalles. El primero las caras de los profesionales sanitarios al recibir las primeras vacunas. Mírenlas bien. Lo tendrán fácil por el gran número de fotos que se están compartiendo en redes sociales estos días. Verán ilusión, esperanza, alivio, fe, ganas de luchar, confianza, tranquilidad, alegría, incluso entusiasmo. 


Las sesenta personas de mi equipo de Atención Primaria nos vacunamos ayer. Fuimos convocados en dos turnos y se organizó un buen jaleo en la sala de espera. Como tenía bastante faena dejé la puerta abierta y esperé que pasaran todos mientras seguía con mis llamadas y demás. Me llegaba el ruido de fondo de las alegres conversaciones y pese a no distinguir las palabras si acertaba a adivinar las emociones que traslucían. La sensación era parecida a cuando hace unos meses empezamos a recibir los equipos de protección individual tras llevar muchas semanas poniéndonos bolsas de basura encima: alivio y esperanza


Y algo que necesitamos tanto es necesario compartirlo con la sociedad dado que a su vez esta está gravemente enferma de miedo, crispación y ceguera. Una enfermedad viscosa que contamina cuerpos y almas ocasionando un sufrimiento gris que se queda pegado a la existencia. 


No puedo asegurarles el grado de eficacia de lo que me inyectaron en el hombro. No será tanto como dicen los laboratorios. Lo que sí puedo asegurar es que merece la pena soportar un pinchazo, más allá de lo que uno piense o sienta al recibirlo, por el simple hecho de avivar la esperanza en que algún día toda esta pesadilla acabará. Vendrán quizá otras, pero esta en concreto hallará fin. 


La otra pequeña imagen que quería compartirles tuvo lugar en la calle Toledo de Madrid pocos minutos después de la deflagración de gas que destruyó un edificio matando cuatro personas. Los vecinos empezaron a retirar grandes piedras y cascotes del pavimento para favorecer el acceso de los equipos de emergencia que aún no habían llegado. Fue algo espontáneo que casi no requirió palabras, ni pensamiento. Unos pocos se pusieron a ello escatimando el riesgo de estar en una zona peligrosa para beneficiar a otros. Coincido con el alcalde en que hay algo de milagro en esta historia, habida cuenta de que había un colegio y una residencia de mayores justo al lado y pudo haber sido una catástrofe mayor. Aunque quizá lo verdaderamente milagroso sea reconocer que en el fondo las enfermedades mortales y las grandes explosiones nos enseñan que la vida que habitamos es paradójicamente leve y tenaz al mismo tiempo. Capaz de apagarse como la llama de una vela con un soplo y a la vez soportar todo tipo de brasas y seguir adelante. 


En eso nos quedamos, en seguir adelante. 








A vaccine of hope.


These complex times are teaching me the importance of paying attention to what truly deserves it. With so much tension and so much noise in the background, it will be increasingly important not to fall into the temptation of paying attention to what shouts the loudest, shines the brightest, or tries to attract us the most, but to the virtue of what is quiet, almost invisible or delicate. I am reminded of the story of the prophet Elijah in the first book of Kings who manages to find God's presence in a gentle breeze and not in the noise of a hurricane, the violence of an earthquake or the brightness of lightning in the middle of the night. 


We have ceased to hear the faint voice of the divine in a world perpetually gripped by screens that keep spewing out all kinds of scandals, misfortunes and cataclysms. And we search our mobile phones hundreds of times a day with regular results. 


What I wanted to bring you this week are just two small details. The first are the faces of the health professionals as they receive the first vaccines. Take a good look at them. It will be easy for you to do so because of the large number of photos that are being shared on social networks these days. You will see excitement, hope, relief, faith, desire to fight, confidence, calm, joy, even enthusiasm. 


The sixty people in my Primary Care team were vaccinated yesterday. We were called in two shifts and there was quite a racket in the waiting room. As I had a lot of work to do, I left the door open and waited for everyone get the shot while I continued with my calls and so on. I could hear the background noise of the cheerful conversations and although I couldn't make out the words, I could guess the emotions they conveyed. The feeling was similar to when we started to receive our personal protective equipment a few months ago after weeks of putting rubbish bags over our bodies: relief and hope. 


And something we need so badly needs to be shared with society, which in turn is seriously ill with fear, tension and blindness. A viscous disease that contaminates bodies and souls causing a grey suffering that clings to existence. 


I can't tell you how effective what they injected into my shoulder is. It will not be as much as the laboratories claim. What I can assure you is that it is worth enduring a jab, regardless of what you think or feel when you receive it, for the simple fact of reviving the hope that one day this nightmare will come to an end. Perhaps others will come, but this one will find an end. 


The other small image I wanted to share with you this week took place in Madrid a few minutes after the gas explosion that destroyed a building and killed four people. Neighbours began to remove large stones and rubble from the pavement to facilitate the access of the emergency teams that had not yet arrived. It was spontaneous and required almost no words, not even thought. A few people set about it, skimping on the risk of being in a dangerous area for the benefit of others. I agree with the city mayor that there is something of a miracle in this story, given that there was a school and a retirement home right next door and it could have been a major catastrophe. But perhaps the real miracle is to recognise that what deadly diseases and big explosions teach us is that the life we inhabit is paradoxically light and tenacious at the same time. Capable of being extinguished like a candle flame with a blow and at the same time withstand all kinds of embers and keep going. 


That is what we are left with, to carry on.

Los cuatro cuadrantes. The four quadrants. 四個像限。

 




Los animales más desarrollados exponen distintos catálogos de conductas sociales que tienden a favorecer la supervivencia. El Homo sapiens dio un paso más en la revolución neolítica con el surgimiento de las sociedades complejas basadas en la especialización y estratificación social que lograron superar el número de Dunbar (el máximo de 150 individuos por tribu) con la aparición de las ciudades. 


Antes de esto, en el paleolítico medio, tuvo lugar otra revolución menos llamativa de la que sabemos poco. Consistió en pasar del triángulo bio-psico-social de los primates y homínidos superiores al cuadrado bio-psico-social-espiritual de nuestros antepasados directos. El perfeccionamiento del lenguaje y la aparición de narrativas religiosas y trascendentes ayudó a que los clanes cooperaran entre si y acometieran proyectos antes vedados como levantar monumentos megalíticos o acometer grandes viajes.


El cuadrante espiritual abría la posibilidad de reconocer que el ser humano individual podía encontrar sentido en la vida y alimentar una esperanza de trascendencia más allá de la muerte. Permitía relacionarse con esa parte oscura, desconocida y terrible que hay más allá del conocimiento humano, tanto dentro de él, como en el mundo visible y en el invisible. 


Con la aparición de la escritura se abrió el tiempo de la historia que nos ha traído hasta hoy. Disponemos de una enorme herencia de relatos que han configurado infinidad de tradiciones religiosas pese a que paradójicamente nuestro tiempo les haya dado la espalda en favor de un desarrollo tecnológico que obra el milagro de producir bienestar y calmar nuestra necesidades.


Lo que quizá no nos hayamos planteado suficientemente es el hecho de que la actual situación globalizada en la que las antiguas fronteras nacionales sirven de poco, no se puede seguir entendiendo ni manejando con el punto de vista de los estados-nación. Lo mismo ocurrió en el pasado con la caída de las ciudades-nación del mundo antiguo o las regiones-nación del mundo medieval. 


Las actuales amenazas y retos son globales y global habrá de ser la respuesta  que encontremos. Esto implicará una nueva forma de organización política, social y probablemente nuevas narrativas que aporten sentido y trascendencia. Al igual que una pandemia mundial es imposible de corregir tomando medidas locales, la situación de cambio climático no se revertirá mientras los cambios de actitud y conducta no sean globales. 


Transformar sociedades basadas en el consumo material en otras sostenibles basadas en el respeto y en el buen trato no será fácil. Se requerirá pasar de una conciencia individual y nacional a otra global y supranacional en la que sintamos a todos los seres vivos como hermanos. Aprender a construir desarrollo humano basado en productos y servicios no materiales. Impulsar la creatividad y las artes como motor social. Recordar que la naturaleza es un espacio de belleza, armonía y disfrute y no únicamente un filón de recursos. Asimilar que cada ser vivo tiene dignidad y valor por sí mismo.


Mientras no rescatemos nuestro cuadrante espiritual y trascendente todo esto no será posible. Probablemente las religiones tradicionales no ofrezcan todas las respuestas pero sería poco inteligente no valorar el legado de tantos hombres y mujeres que han buscado y aportado testimonio con sus vidas. No sabemos cómo serán las narrativas que consigan sostener la sociedad que surgirá. Tan solo intuimos que será imprescindible relacionarse con la fuente de la que surge la vida, el sentido y la trascendencia, esa de la que nos hablaron los místicos y que está más cerca del corazón de cada cual de lo que imaginamos.

The refinement of language and the emergence of religious and transcendent narratives helped the clans to cooperate with each other and to undertake previously unheard of projects such as erecting megalithic monuments or undertaking great journeys.


The spiritual quadrant opened up the possibility of recognising that the individual human being could find meaning in life and nurture a hope of transcendence beyond death. It allowed him to relate to that dark, unknown and terrible part that is beyond human knowledge, both within him and in the visible and invisible world. 


With the appearance of writing, the time of history that has brought us to this day opened up. We have an enormous heritage of stories that have shaped an infinite number of religious traditions despite the fact that, paradoxically, our time has turned its back on them in favour of a technological development that works the miracle of producing well-being and calming our needs.


What we may not have sufficiently considered is the fact that the current globalised situation in which former national borders serve little purpose, can no longer be understood or handled from the point of view of the nation states. The same thing happened in the past with the fall of the nation-cities of the ancient world or the nation-regions of the medieval world. 


Today's threats and challenges are global and global will be the answer we find. This will imply a new form of political and social organisation and probably new narratives that will bring meaning and transcendence. As long as we do not rescue our spiritual and transcendent quadrant all this will not be possible. Traditional religions probably do not offer all the answers, but it would be unwise not to value the legacy of so many men and women who have sought and given testimony with their lives. We do not know what the narratives will be that will sustain the society that will emerge. We only intuit that it will be essential to relate to the source from which life emerges, the meaning and the transcendence, that which the mystics spoke to us about and which is closer to the heart of each of us than we imagine.

¿Por qué no conseguimos doblegar la pandemia? Why don't we succeed in bringing the pandemic to a halt? 我們為什麼沒有成功遏制大流行?






Muchos nos preguntamos cómo con todo lo que llevamos haciendo en el último año y los avances científicos conseguido estemos en la casilla de salida. La realidad parece un endiablado tablero del juego de la oca en el que nos enfrentamos con poca fortuna a un jugador bastante bueno. 


La respuesta corta al por qué de nuestra aparente ineptitud contra el coronavirus la podemos escribir con dos palabras: gestión de la complejidad. Y es que nuestras sociedades contemporáneas son redes complejas globalizadas cuya gestión sigue basándose en algoritmos de redes simples no coordinadas. De este modo lo que gestiona un político a nivel nacional no se coordina con el político regional ni con el local, lo que gestiona un ministerio no lo hace con las autonomías, lo que se dice en un hospital no llega al centro de salud y así con todas las redes que la sociedad tiene. 


De esta forma el sacrificio de un confinamiento en zonas rurales es inútil si en la gran ciudad sigue funcionando el transporte público saturado o si algunos jóvenes siguen organizando fiestas. De nada sirve que miles usen la mascarilla por zonas despobladas si al mismo tiempo muchos más se la quitan en presencia de otros mientras desayunan en el trabajo o se toman un refresco con amigos en el bar. Poco conseguirá un médico de familia que diagnostica un caso y hace correctamente un estudio de contactos si algunos de estos no se confinan y siguen haciendo vida normal. 


Por todo esto pareciera que estamos poniendo puertas al campo y no hay medida que consiga mejorar las cosas. A esto le añadimos que ahora somos todos expertos epidemiólogos y defendemos con vehemencia el confinamiento o su contrario, el uso de una medida o de la otra, dar caña o todo lo contrario. Y con tanto griterío no escuchamos las voces de los que están en primera línea atendiendo cada vez más enfermos ni a los que llevan toda la vida trabajando en control de epidemias. No es una cuestión de sabiduría ni conocimiento sino de reconocer que ante situaciones excepcionales globalizadas ya lo puedes hacer perfectamente en tu casa, barrio, región o país, que si hay un foco de inestabilidad en alguna parte al final te alcanzará dado que no hay barreras ni murallas que nos defiendan de la incertidumbre. 


Por lo tanto seremos todos testigos de una nueva ola invernal que afectará a mucha gente. Volveremos a ver el sistema sanitario desbordado. Volveremos a tener confinamiento duro. Iremos viendo como la vacuna va protegiendo cada vez más y quizá lleguemos al verano algo más tranquilos. 


Pero estamos hablando de futuro y lo que nos interesa es el presente. Toca cuidarse, dentro de lo posible, y mantener la esperanza de que pase lo que pase esta situación, como todo en la vida, terminará pasando.







Why don't we succeed in bringing the pandemic to a halt?

Many of us wonder how, with all that we have been doing over the past year and the scientific advances we have made, we are at square one. The reality seems to be a devilish goose chase where we are playing with little luck against a fairly good player. 

The short answer as to why our apparent ineptitude against the coronavirus can be written in two words: complexity management. The fact is that our contemporary societies are complex globalised networks whose management is still based on uncoordinated simple network algorithms. In this way, what is managed by a politician at a national level is not coordinated with the regional or local politician, what is managed by a ministry is not coordinated with the autonomous regions, what is said in a hospital does not reach the local health centre and so on with all the networks that society has. 

In this way, the sacrifice of a confinement in rural areas is useless if in the big city public transport continues to be saturated or if some young people continue to organise parties. There is no point in thousands of people wearing masks in depopulated areas if at the same time many more take them off in the presence of others while they are having breakfast at work or having a drink with friends at the bar. Little will a family doctor diagnose a case and make a correct contact study if some of them are not confined and continue to live a normal life. 

For all these reasons it seems that we try to stem the tide and there is no measure that will make things better. To this we add that we are now all expert epidemiologists and we vehemently defend confinement or its opposite, the use of one measure or the other, to use force or the opposite. And with so much shouting, we do not hear the voices of those who are on the front line caring for more and more sick people or those who have been working on controlling epidemics all their lives. It is not a question of wisdom or knowledge but of recognising that in exceptional globalised situations you can do this perfectly well in your home, neighbourhood, region or country, that if there is a source of instability somewhere it will eventually reach you because there are no barriers or walls to defend us from uncertainty. 

Therefore we will all witness a new winter wave that will affect many people. We will see the health system overflowing again. We will once again have hard confinement. We will see how the vaccine will protect us more and more and perhaps we will arrive at the summer with a little more peace of mind. 

But we are talking about the future and what interests us is the present. We have to take care of ourselves, as far as possible, and maintain the hope that whatever happens, this situation, like everything else in life, will end up happening.

Resbalar. Slide. 滑動

 



Hace unos meses resbalé en mi calle y salí proyectado hacia la izquierda. Durante un instante perdí pie y experimenté una agradable sensación de vuelo e ingravidez que el impacto en el suelo borró de inmediato. Como no he olvidado el dolor estos días de nieves y hielos tomo todas las precauciones y salgo a la calle con calzado de montaña y bastones. Pese a su mala prensa el dolor y el miedo son valiosos instrumentos de supervivencia, algo que nuestra sociedad edulcora y al hacerlo nos vuelve desvalidos sin que muchos lo sepan.


No estamos acostumbrados a lo imprevisto. Pese a llevar casi un año al borde del ataque de nervios por la sucesión de acontecimientos adversos seguimos sin darnos cuenta de lo importante que es estar atentos y reaccionar. Lo básico que es leer los acontecimientos y prepararnos antes de que nos alcancen. El frío sigue siendo un poderoso adversario al igual que los virus. Y estas semanas nos lo recordará con rigor. Mucha gente padece pobreza energética y otros muchos edades avanzadas para las que el crudo invierno supone toda una prueba de supervivencia que algunos no podrán superar. 


Delegarlo todo en los políticos y responsables no es suficiente. A las pruebas me remito. La capacidad de adaptación de una sociedad depende fundamentalmente de que sus ciudadanos estén despiertos. Si el grado de atontamiento es alto estamos perdidos. Por eso es vital contrarrestar las imágenes de descerebrados que nos llegan por nuestras pantallas por actos personales que aporten valor a otros o por lo menos nos cuiden o protejan. 


Usar doble calcetín y ropa adecuada a la temperatura, mantener un grado de movilidad aceptable, tener precaución si hay hielo en las aceras, interesarse por los mayores de la familia o el vecindario que puedan necesitar ayuda para los recados, coordinarse con otros vecinos para quitar la nieve de la calle... son actos que quizá tengan poca visibilidad pero que construyen humanidad y civilización. Creo que merece la pena impregnar con delicadeza nuestra agenda y cuidar los pequeños detalles, esos que al final son los que terminan construyendo lo que llamamos realidad.

A few months ago I slipped on my street and came out projected to the left. For a moment I lost my footing and experienced a pleasant sensation of flight and weightlessness that the impact on the ground immediately erased. As I haven't forgotten the pain, this week of snow and ice I take all the precautions and go out into the street with mountain shoes and sticks. Despite its bad press, pain and fear are valuable instruments of survival, something that our society sweetens and in doing so makes us helpless without many knowing it.

We are not used to the unexpected. Despite having been on the verge of a nervous breakdown due to a succession of adverse events for almost a year, we still do not realise how important it is to be alert and to react. The basic thing is to read the events and prepare ourselves before they reach us. Winter remains a powerful adversary just like viruses. And these weeks will remind us of that fact. Many people suffer from energy poverty and many others from advanced age for whom the harsh winter is a test of survival that some will not be able to overcome. 

Delegating everything to politicians and decision-makers is not enough. I refer to the evidence. A society's ability to adapt depends crucially on its citizens being awake. If the level of numbness is high, we are lost. That is why it is vital to counteract the images of mindless people that reach us on our screens through personal acts that add value to others or at least care for or protect us. 

Wearing double socks and clothes suitable for the temperature, maintaining an acceptable degree of mobility, being careful if there is ice on the pavement, taking an interest in the elderly in the family or neighbourhood who may need help with errands, coordinating with other neighbours to remove snow from the street... these are acts that may have little visibility but which build humanity and civilisation. I think it is worthwhile to delicately impregnate our agenda and take care of the small details, those that in the end are the ones that end up building what we call reality.
