No nos gusta la idea de quedarnos en casa a la fuerza. En una sociedad como la nuestra basada en el ruido y la prisa lo vivimos como castigo. Sin embargo hay situaciones que nos obligan a hacerlo, como una torcedura fortuita de tobillo o una pandemia global. Nos cuesta quedarnos quietos, dejar de hacer, dejar de consumir. Pero si nos atrevemos a hacerlo y a nos sumergimos en las calmadas aguas del aburrimiento tal vez encontremos cosas inesperadas. En estos días de aumento de casos de infección por Coronavirus lo más adecuado es quedarse en casa. Minimizar nuestras interacciones sociales y permanecer dentro de lo límites de la república independiente de nuestro domicilio. Habrá tiempo de sobra para ver la tele (dosifiquen por favor), tal vez alguno se anime a leer y quizá surja alguna reflexión, toma de conciencia, alguna creatividad o idea. Viene bien recordar que el silencio es uno de los grandes lujos modernos y que sin salir de casa se puede recorrer el mundo si uno se arma con las lecturas correctas. Mucho ánimo.
Stay home. We don't like the idea of staying home by force. In a society like ours based on noise and haste, we live it as punishment. Cada vez tenemos más clara la importancia de la higiene corporal pero seguimos sin cuidar otras facetas de nuestra vida más sutiles e invisibles, una de ellas el sueño. Lo habitual es que nos vallamos a la cama con los dientes limpios y los sueños sucios. Me explico, el ser humano lleva miles de años ajustando su ritmo sueño-vigilia según la luz solar. Desde la invención de la luz eléctrica hemos modificado dicho ajuste, más si cabe desde que disponemos de pantallas que además de ser una potente fuente de luz lo es de provisión de información. Lo más habitual es irnos a dormir tras haber consumido un tiempo de visualización de pantallas. El mercado lo sabe y sitúa en esas horas los contenidos audiovisuales más apetecibles que suelen incluir dosis de acción, entretenimiento y estímulos intensos. Nos acostamos pues sin haber dado tiempo a digerir esas potentes imágenes que teñirán inevitablemente el contenido onírico de la noche, dado que los sueños se valen de lo experimentado en la anterior jornada. Al ser lo soñado una realidad difícil de estudiar no tenemos mucha evidencia científica sobre cómo influye el contenido audiovisual en el sueño pero por lo que sabemos sí parece que hay relación tanto con el contenido onírico como con la calidad del sueño. Si se aumenta el nivel de alerta o ansiedad, el sueño perderá calidad, se hará más superficial y podrá volverse quebradizo con múltiples despertares nocturnos y menor sensación de haber descansado a la mañana siguiente. La recomendación es sencilla. La mejor higiene del sueño implica: 1. Cuidar los horarios y el hábito de ir a dormir a una misma hora. 2. Evitar cenas copiosas inmediatamente antes de acostarse. 3. Disminuir la intensidad de la luz eléctrica ambiental un tiempo antes de dormir. 4. Evitar el uso de pantallas la hora previa a acostarse. Es mucho más sano para el sueño acostumbrarse a leer, charlar tranquilamente o realizar alguna actividad no muy estimulante antes de irse a dormir. Dirty dreams. The importance of body hygiene is becoming increasingly clear to us, but we still do not care about other more subtle and invisible facets of our lives, one of them being sleep. We usually go to bed with clean teeth and dirty dreams. I mean, human beings have been adjusting their sleep-wake rhythm according to sunlight for thousands of years. Since the invention of electric light, we have modified this adjustment, even more so since we have screens that, in addition to being a powerful source of light, also provide information. The most common thing is to go to sleep after having spent some time viewing screens. The market knows this and places the most appealing audiovisual content in those hours, which usually includes doses of action, entertainment and intense stimuli. So we go to bed without having given ourselves time to digest those powerful images that will inevitably colour the dreamlike content of the night, given that dreams are based on what we experienced the previous day. Since dreams are a reality that is difficult to study, we do not have much scientific evidence on how audiovisual content influences dreams, but from what we do know, it seems that there is a relationship with both the dream content and the quality of the dream. If the level of alertness or anxiety is increased, sleep will lose quality, become more superficial and may become fragile with multiple nighttime awakenings and less feeling of having rested the next morning. The recommendation is simple. The best sleep hygiene implies: 1. Take care of the schedules and the habit of going to sleep at the same time. 2. Avoiding big dinners immediately before going to bed. 3. Reducing the intensity of ambient electric light for a while before going to sleep. 4. Avoid screens the hour before going to bed. It is much healthier for your sleep to get used to reading, talking quietly or doing something not very stimulating before going to bed. Nos defendemos del mundo con narrativas. Lo llevamos haciendo miles de años desde que el hombre robó el verdadero fuego de los dioses que es la palabra. Gracias a ella nos podemos enfrentar a los abismos de la muerte, el sufrimiento, lo desconocido y el sinsentido. Gracias a ella nos es posible construir herramientas y civilizaciones. Ayudamos a los niños a construir un armazón narrativo que llevarán como una segunda piel toda su existencia. No es posible vivir sin ella. Su tejido incorpora infinidad de historias trenzadas desde la noche de los tiempos que vamos recibiendo en forma de cuento, lección, conversación y demás formas. Vamos añadiendo hebras de forma pasiva según nos relacionamos con el medio social y aportamos también las propias según aprendemos a hablar y conocernos a nosotros mismos. Es una envuelta que dará calor al alma cuando toque atravesar las frías y oscuras noches de la dificultad, la enfermedad o el duelo. A esta coraza narrativa le quieren añadir hoy una nueva cubierta, esta vez más sutil y transparente. Imaginen que fuera como un huevo de cristal que nos envuelve y que no somos capaces de identificar. Un nuevo medio silencioso que como el agua consiguiera envolver al pez sin que este tenga posibilidad de saberlo. Esta capa se está constituyendo gracias a la tecnología, que cada día que pasa tiene más capacidad de saber quiénes somos. Al ir vertiendo información personal en Internet, cada vez es más sencillo para esta devolvernos la información que le requerimos ligeramente modificada según mandan los algoritmose usando cookies y demás información personal que vamos dejando en redes sociales. Las posibilidades comerciales y manipulativas de este cambio son terribles. Por eso se está invirtiendo tanto dinero en algo tan poderoso, capaz de vender cualquier producto o idea. Para defendernos hace falta mucha conciencia, crítica y reflexión, propiedades que cada vez están más mermadas según aumenta el grado de entretenimiento social, ruido de fondo, prisa social y estimulación intensiva. Si estudiamos la capacidad de pensamiento en ratones de laboratorio será fácil demostrar que si son estimulados por incontables calambrazos al día se disminuirá fácilmente la capacidad de reacción y pensamiento de estos. Añadan epidemias de miedo y otras herramientas de manipulación masiva. No va a ser fácil plantear alternativas. Ni de dar el primer paso que es llamar las cosas por su nombre. The narrative shell. We defend ourselves from the world with narratives. We have been doing so for thousands of years since human beings stolen the true fire of the gods which is the word. Thanks to it we can face the abysses of death, suffering, the unknown and nonmeaning. Thanks to it we can build tools and civilizations. We help children to build a narrative framework that they will carry like a second skin throughout their existence. It is not possible to live without it. Its fabric incorporates an infinite number of stories woven from the night of time that we receive in the form of a story, a lesson, a conversation and other forms. We add threads passively as we relate to the social environment and we also contribute our own as we learn to speak and know ourselves. It is a wrap that will warm the soul when it is time to go through the cold and dark nights of difficulty, illness or mourning. To this narrative shell they want to add today a new cover, this time more subtle and transparent. Imagine it as a glass egg that surrounds us and that we are unable to identify. A new silent medium that, like water, manages to envelop the fish without it having the chance to know it. This layer is being formed thanks to technology, which every day has a greater capacity to know who we are. As we pour personal information on the Internet, it is increasingly easy for this return the information we require slightly modified as the algorithms send using cookies and other personal information that we are leaving on social networks. The commercial and manipulative possibilities of this change are terrible. That's why so much money is being invested in something so powerful, capable of selling any product or idea. To defend ourselves we need a lot of awareness, criticism and reflection, properties that are increasingly diminished as the degree of social entertainment, background noise, social rush and intensive stimulation increases. If we study the thinking capacity of laboratory mice, it will be easy to demonstrate that if they are stimulated by countless cramps a day, their capacity to react and think will be easily diminished. Add epidemics of fear and other tools of mass manipulation. It will not be easy to come up with alternatives. Nor will it be easy to take the first step which is to call things by their name.

El hundimiento de las narrativas La crisis sistémica que padecemos configura el escenario del colapso actual de civilización. Se caracteriza por un aumento globalizado del malestar y el miedo que disminuye el pensamiento, la reflexión y la comprensión. No encontramos nada dónde agarrarnos, ni líderes, ni ideas ni narrativas, lo que está permitiendo que el poder se posicione detrás de tecnologías que ofrecen sucedáneos. Esta situación afecta a todos los ámbitos sociales desde la política a la educación, desde la organización laboral a la sanidad. Me centraré en esta última que es la que conozco bien. Desde hace años los profesionales de la salud somos conscientes del derrumbe de los Sistemas Sanitarios públicos en distintos países. Conocemos las causas pero no disponemos de la capacidad de implementar soluciones. Pensamos que con cambios de organización, financiación y gestión es posible cambiar y mejorar el sistema. Nos frustra que no haya motivación política ni gestora para ello. Tal vez estemos equivocanado el enfoque. En mi opinión no existe organización sanitaria que pueda resistir la situación social actual que ha convertido la salud en un bien económico más y ha establecido las leyes de mercado como lex artis. El ciudadano siempre va a querer consumir "más salud", y ante cualquier síntoma pedirá soluciones. Hemos perdido la batalla de la narrativa al asumir como cierto para la salud el paradigma de mercado que impera en nuestro tiempo. Al definir salud como bienestar abrimos la puerta a la generación de infinitos bienes y servicios susceptibles de mercantilización. El paciente se convierte en consumidor y como tal solicita ser tratado. El sobrediagnóstico y el sobretratamiento son los negros ángeles que surgen de esta perversión. La dependencia del ciudadano ante el sistema sanitario es cada vez mayor. Da igual la edad y circunstancia de la persona, ante el mínimo síntoma se acude a un centro sanitario. Hemos fracasado claramente en el "empoderamiento" del paciente tras años de buenismo sanitario en el que se han asumido como lícitas cualquier tipo de consulta. Y esto tiene difícil vuelta atrás. Pero ¿cómo transformar esta narrativa de salud?, ¿cómo ayudar al que padece a conseguir una nueva perspectiva de lo que le sucede? Las respuestas pasan por favorecer una comunicación de calidad que permita una relación de confianza entre pacientes y profesionales. Lo segundo será aprender a escuchar lo que hay debajo del motivo de consulta, conseguir visualizar a la persona como un todo. Atrevernos a ver sus cuatro cuadrantes: físico, psicológico, social y existencial. Lo tercero ser capaces de acompañar en la construcción de una nueva narrativa por parte del paciente que le permita hacerse cargo de lo que le pasa, aceptarlo, sostenerlo y seguir caminando. La sanación implica que cada cual complete este proceso interno independientemente de lo que suceda con sus síntomas. Muchos se preguntarán cómo será posible hacerlo en consultas de cinco o seis minutos en agendas cada vez más sobrecargadas. Y ante esto respondo con otra propuesta de cambio de narrativa, en este caso para profesionales. Si seguimos centrados en la queja y en la limitación no conseguiremos darnos cuenta del enorme poder que tienen los clínicos para ayudar a sus pacientes a transformar el plomo de sus vidas en el ansiado oro de la serenidad, el sentido y la alegría. La alquimia la hace siempre el que padece, pero que bien viene una ayudita externa cuando uno está en mitad de una noche oscura.
The collapse of narratives The systemic crisis that we are experiencing sets the stage for the current collapse of civilization. It is characterized by a globalized increase in unrest and fear that diminishes thinking, reflection and understanding. We find nothing to hold on to, no leaders, no ideas and no narratives, which is allowing power to position itself behind technologies that offer substitutes. This situation affects all social areas from politics to education, from labour organisation to health. I will focus on the latter, which is the one I know well. For years, health professionals have been aware of the collapse of public health systems in different countries. We know the causes but we do not have the capacity to implement solutions. We think that changing organization, financing and management it would be possible improve the system. We are frustrated that there is no political or institutional motivation for this. Maybe we are wrong in our approach. In my opinion there is no health organization that can resist the current social situation that has turned health into just another economic good and has established the laws of the market as lex artis. The citizens will always want to consume "more health", and in the face of any symptom they will ask for solutions. We have lost the narrative battle by assuming as true for health the market paradigm that prevails in our time. By defining health as well-being we open the door to the generation of infinite goods and services susceptible to commodification. The patient becomes a consumer and as such asks to be treated. Overdiagnosis and overtreatment are the black angels that arise from this perversion. The citizen's dependence on the health system is increasing. No matter the age and circumstances of the person, at the slightest symptom they go to a health centre. We have clearly failed in "empowering" the patient after years of good samaritan care in which any type of consultation has been assumed to be appropriate. And this is difficult to reverse. But how to transform this health narrative, how to help the sufferer get a new perspective on what is happening to them? The answers lie in favouring quality communication that allows a relationship of trust between patients and professionals. The second thing will be to learn to listen to what lies beneath the reason for the consultation, to manage to visualise the person as a whole. Dare to see its four quadrants: physical, psychological, social and existential. The third is to be able to accompany the patient in the construction of a new narrative that allows him/her to take charge of what is happening to him/her, to accept it, to sustain it and to continue walking. Healing implies that everyone completes this internal process regardless of what happens to their symptoms. Many will wonder how this will be possible in five or six minute consultations in increasingly overloaded schedules. And to this I respond with another proposal for a change of narrative, in this case for professionals. If we continue to focus on complaint and limitation we will not be able to realize the enormous power that clinicians have to help their patients transform the lead in their lives into the longed-for gold of serenity, meaning and joy. Alchemy is always done by the sufferer, but what good is a little external help when one is in the middle of a dark night.
Estaría muy bien que como sociedad aprendiéramos algo de las pandemias. 1. Los memes y los virus se transmiten igual. De persona a persona y son muy contagiosos. 2. Hay memes y virus peligrosos y otros no tanto. 3. Merece la pena defenderse de ambos. De los memes: minimizado el ruido de fondo, buscando información de calidad en cantidad justa y evitando fuentes de mala información. De los virus:

Cubriéndonos la boca si tosemos, con la flexura del codo o un pañuelo desechable.
No exponiéndonos innecesariamente a aglomeraciones o ambientes muy fríos.
Los periodistas y los medios de comunicación tendrían que tener una directriz clara de información veraz cuando se aborden estos temas. Los profesionales sanitarios recordar que son agentes de educación para la salud y eso implica algún curso de acción que visibilice información de calidad. La ciudadanía en general podría aprovechar estas situaciones para reforzar hábitos como el lavado de manos y el cubrirse la boca correctamente si tose delante de otros. Más educación y menos mascarillas por favor.

More education and less mask It would be great if we as a society could learn something from pandemics. 1. Memes and viruses are transmitted the same way. Orla de la madre de la dra. Carmen Suárez. En esa promoción de médicina solo había 4 mujeres. Las cosas han cambiado.
Ayer participé en una jornada sobre vocación médica y salí agotado. Me resultó una experiencia exigente por tener, como ponente, que dar de alguna forma testimonio de algo tan sutil. Porque no me resulta fácil de definir el término, ni siguiera de garantizar que exista. Probablemente si miramos en lo profundo todos podamos descubrir algún tipo de llamada o atracción hacia algún tipo de desempeño laboral. Una intuición que nos ayuda a dar el paso. Los adolescentes tienen que enfrentarse, sin apenas conocerse, a decisiones formativas que determinarán su futuro. Su conocimiento del mundo tiene base audiovisual y es impreciso y cinematográfico. Por tanto se enfrentan al reto de orientar su futuro desde una densa niebla que muchas veces familia y educadores no somos capaces de despejar. La parte racional interviene con datos objetivos pero al final nos apoyamos en el instinto optar. Uno va descubriendo que se decide como se puede y no hay más remedio que caminar entre la bruma corrigiendo el rumbo según vayan surgiendo peligros, errores o crisis vitales. Los estudiantes de medicina ya vienen sesgados por su curriculum, habitualmente el más brillante de su clase. Vienen lastrados por altos niveles de autoexigencia y generalmente un enorme esfuerzo personal de dedicación al estudio y la formación. Pero estas características no les proveen de mayor visión personal o entendimiento del mundo. Siguen siendo jóvenes sin experiencia vital que desean llegar a ser felices, signifique eso lo que signifique. Las asistentes a la jornada irradiaban intensidad porque querían ver el futuro. Acudían al oráculo de los mayores buscando respuestas y al final fueron despedidas como lo hacían en Delfos, con el lacónico "conócete a ti mismo" que pocos adultos conseguimos llevar a plenitud. Es cierto que también hubo ánimos para reconocer la propia valía personal y recuperar la confianza, pero al final he de reconocer que todos caminamos viendo tan solo el próximo paso a dar, en un medio cada vez más líquido y complejo como es esta sociedad del conocimiento que cambia a un ritmo endiablado. Otro hecho perturbador tuvo que ver con la perspectiva de género que la dra. Carmen Suárez, jefa de servicio de Medicina Interna de la Princesa, expuso desde su gran experiencia clínica y humana. La conciliación laboral es un reto que amenaza la promoción profesional de las mujeres y que en las profesiones sanitarias tienen enorme calado por la gran feninización (85% de enfermeras y 70% de médicas). Poco que añadir, salvo que tan poco es fácil para los hombres en un sistema sanitario rígido como el nuestro que no facilita la flexibilidad horaria suficiente para poder desarrollar una crianza en condiciones. Puedo decir que me gusta lo que hago, que ser médico es para mi una posibilidad de servir a los demás que me proporciona sentido y satisfacciones. Creo que hace falta algo más que un curriculum brillante, hay que valer para poder aguantar el lado oscuro que el contacto con el sufrimiento ajeno inevitablemente tiene. Por eso doy la bienvenida a todos los que sientan una intuición en este sentido, vengan y traten de aprender. Al final la vida nos va poniendo en nuestro sitio y si la clínica no es el propio hay muchos más caminos.
Medical vocation. Yesterday I participated in a conference on medical vocation and I left exhausted. It was a demanding experience for me, as a speaker, to have to witness something so subtle in some way. Because it is not easy for me to define the term, nor to guarantee that it exists. Probably if we look deep down we can all discover some kind of call or attraction to some kind of work performance. An intuition that helps us to take the step. Teenagers have to face, without hardly knowing each other, formative decisions that will determine their future. Their knowledge of the world has an audiovisual basis and is imprecise and cinematic. Therefore they face the challenge of orienting their future from a dense fog that many times family and educators are not able to clear. The rational part intervenes with objective data but in the end we rely on the instinct to choose. Time show us to make decisions the best we can, and there is no choice but to walk through the mist, correcting the course as dangers, errors or life crises arise. Medical students are already biased by their curriculum, usually the brightest in their class. They are weighed down by high levels of self-demand and usually a huge personal effort in dedication to study and training. But these characteristics do not provide them with greater personal vision or understanding of the world. They are still young people without life experience who want to become happy, whatever that means. Those attending the conference radiated were on fire because they wanted to see the future. They went to the oracle of the elders looking for answers and in the end they were dismissed as they did in Delphi, with the laconic "know thyself" that few adults manage to bring to fullness. It is true that there was also encouragement to recognize one's own personal worth and to regain confidence, but in the end I have to recognize that we all walk on seeing only the next step to take, in an environment that is more and more liquid and complex as this society of knowledge changes at a devilish pace. Another disturbing fact had to do with the gender perspective that Dr. Carmen Suárez, head of the Internal Medicine service of the Princess Hospital, exposed from her great clinical and human experience. The reconciliation of work and family life is a challenge that threatens the professional promotion of women and that in the health professions is very important due to the high level of pheninization (85% of nurses and 70% of doctors). There is little to add, except that it is not easy for men also in a rigid health system such as ours, which does not provide sufficient time flexibility to be able to raise their children in good conditions. I can say that I like what I do, that being a doctor is for me a possibility to serve others that gives me meaning and satisfaction. I believe that it takes more than a brilliant resume; it takes courage to be able to endure the dark side that contact with the suffering of others inevitably has. That's why I welcome everyone who feels an intuition in this sense, come and try to learn. In the long run life puts us in our place and if the clinic is not your own there are many more ways.

La medicina moderna ha conseguido un desarrollo espectacular en el último siglo en paralelo al resto de la sociedad. Espoleada por la ciencia y la tecnología ha conseguido superar ambiciosos objetivos y brilla orgullosa en base a sus logros. Lamentablemente hay una sombra alargada que contiene lastres y amenazas. Las inequidades en salud, el sobrediagnóstico y el sobretratamiento, la yatrogenia, la progresiva dependencia del ciudadano de los sistemas sanitarios son realidades cada vez más presentes.
Por otro lado los sistemas sanitarios privados y los enormes conglomerados de industrias farmacéuticas y tecnológicas basados en lucro buscan intereses que cada vez se alejan más el bien común. Frente a ellos los sistemas sanitarios públicos van lentamente hundiéndose lastrados por la infrafinanciación, los recortes y la sobrecarga crónica de sus profesionales. Todos coinciden en la complejidad de gestionar alternativas pero nadie se atreve a implementarlas. Mientras tanto los sistemas sanitarios públicos van fundiéndose como la mantequilla viendo como se externalizan servicios, se precariza a sus profesionales o se reconvierten y cierran unidades (Salud Pública y demás).
La situación es de bloqueo dado que no hay político que quiera arriesgarse a dar pasos que no tendrán rédito en votos, no hay gestor que se atreva con modelos novedosos, no hay profesional que tenga posibilidad de cambiar el sistema y no hay usuario que no quiera más por menos.
Por estas razones tal vez haya que mirar el problema desde una nueva perspectiva.
El paradigma actual de las ciencias médicas tiene base científica. Sin embargo asistimos por un lado a una corrupción de la ciencia manipulada por intereses mercantiles y por otro a una búsqueda de mayor eficiencia que se deja de lado factores tan importantes como los humanistas.
Es cierto que el progreso implica aumentar el beneficio económico y el bienestar, pero si nos atrevemos a meter en la ecuación el progreso ético tendremos que conseguir aumentar la compasión.
Toca revisar los valores. Y de entre ellos elegir lo más valioso para que nos sirva de brújula y dejemos de vagar perdidos y atontados dentro de mercados que nos aturden con sus mensajes y nos impiden clarificar lo que verdaderamente tiene relevancia para nosotros. La axiología tiene un componente personal y otro social, el primero es íntimo, el segundo público. Hoy los valores sociales están condicionados por los intereses del mercado que domina tanto las esferas económico políticas como las sociales.
Se beneficia el individualismo y la competencia, “más es mejor”, el consumir todo lo que uno se pueda permitir, el “yo primero, luego los demás”, la bajada de precios aunque se perjudique a los trabajadores productores. El ganador se lo lleva todo y el pez grande se come al chico. Es el mundo que hemos construido con un puñado de conglomerados industriales que cada vez tiene más poder.
Los servicios sanitarios terminarán abducidos por esos conglomerados que cada vez serán capaces de ofrecerlos a menor coste gracias al uso intensivo de tecnología y a la precarización de los profesionales sanitarios.
Lo único que puede revertir esta tendencia es la toma de conciencia social del valor de las cosas. ¿Qué valor tiene la salud en mi vida? ¿Cómo me gustaría que me trataran cuando enferme? ¿Quiero relacionarme con máquinas, teleoperadores o profesionales sanitarios estresados o por profesionales que puedan dedicarme el tiempo de atención de calidad que necesito?
Pero ¿qué sociedad será capaz de enfrentar estos retos sin cultura, reflexión y diálogo?. Por eso más que quedarnos en la mera queja, por muy justificada que pueda estar, es cada vez más perentorio favorecer dinámicas que propicien la información correcta, la visión crítica y la toma de conciencia.
La ética, la comunicación y la narrativa pueden servir de ayuda. Conseguir reconvertir la complejidad del debate sanitario en un lenguaje comprensible para la sociedad en general será lo que determine si el barco aguanta o termina hundiéndose.
Modern medicine has achieved a spectacular development in the last century in parallel with the rest of society. Spurred on by science and technology it has managed to overcome ambitious goals and shines proudly on the basis of its achievements. Unfortunately, there is a long shadow that contains burdens and threats. Inequalities in health, overdiagnosis and overtreatment, iatrogeny, the progressive dependence of citizens on health systems are increasingly present realities.
On the other hand, private health systems and the enormous conglomerates of pharmaceutical and technological industries based on profit seek interests that are increasingly distant from the common good. In the face of these interests, public health systems are slowly collapsing due to under-funding, cutbacks and the chronic overloading of their professionals. Everyone agrees on the complexity of managing alternatives but no one dares to implement them. Meanwhile, the public health systems are melting like butter as services are outsourced, professionals working conditions get worse or units are reconverted and closed (Public Health and others).
The situation is one of deadlock, since there is no politician who wants to risk taking steps that will not yield a profit in votes, there is no manager who dares with new models, there is no professional who has the possibility to change the entire system and there is no user who does not want more for less.
For these reasons, it may be necessary to look at the problem from a new perspective.
The current paradigm of the medical sciences has a scientific basis. However, on the one hand we are witnessing a corruption of science manipulated by commercial interests and on the other hand a search for greater efficiency that leaves aside factors as important as the humanists.
It is true that progress implies increasing economic benefit and well-being, but if we dare to bring ethical progress into the equation we will have to manage to increase compassion.
It is time to review values. And from among them we must choose what is most valuable so that it serves as a compass and we stop wandering, lost and dazed, within markets that boggle us with their messages and prevent us from clarifying what is truly relevant to us. Axiology has both a personal and a social component, the former being intimate, the latter public. Today, social values are conditioned by the interests of the market, which dominates both the economic and political spheres.
Individualism and competition benefit, "more is better", consuming everything one can afford, "me first, then the others", lowering prices even if it is to the detriment of producing workers. The winner takes all and the big fish eats the small one. This is the world we have built with a handful of industrial conglomerates that are increasingly powerful.
Health services will end up being abducted by these conglomerates, which will be able to offer them at a lower cost thanks to the intensive use of technology and the precarisation of health professionals.
The only thing that can reverse this trend is the social awareness of the value of things. What is the value of health in my life? How would I like to be treated when I am ill? Do I want to be associated with machines, teleoperators or stressed health professionals or by professionals who can dedicate the time of quality care that I need?
But what society will be able to face these challenges without culture, reflection and dialogue? That is why more than just complaining, no matter how justified it may be, it is more and more urgent to favour dynamics that promote correct information, critical vision and awareness.
Ethics, communication and narrative can help. Turning the complexity of the health debate into a language understandable by society in general will determine whether the ship holds out or ends up sinking.
Esta semana he empezado a trabajar en el centro de salud de Soto del Real por jubilación de un compañero del equipo. Dejo mi consulta de Miraflores que he ocupado casi dos años agradecido por los estupendos profesionales del consultorio y por la amabilidad y acogida de los pacientes, muchos de ellos muy mayores. Lo primero que he hecho en mi nueva consulta es colocar mis libros, plantas y cuadros. Lo siguiente echar la mesa a un lado para poder atender a quien acuda sin barreras físicas. Al compartir el espacio con otro compañero de turno de tarde me tocará mover el mueble todos los días pero lo doy por bueno si eso me hace sentir más cómodo y facilita en algo la comunicación. En estos tiempos de tecnología e innovación sigo pensando en la importancia de las pequeñas cosas. Un gesto no cambiará nunca el sistema sanitario pero tal vez pueda producir efectos positivos e inesperados. Me seguiré descalzando al entrar para recordarme que mi labor ha de ir pegada a tierra y trataré de seguir mirando a la gente a los ojos para no olvidar que compartimos una misma mirada y una misma vida. Espero poder estar a la altura. The importance of the little things This week I started working at Soto del Real primary health centre due to the retirement of a teammate. I am leaving my Miraflores practice which I have occupied for almost two years, grateful for the great professionals in the office and for the kindness and welcome of the patients, many of them very old. The first thing I have done in my new practice is to place my books, plants and pictures. The next thing I do is set the table aside so that I can attend to anyone who comes in without any physical barriers. I share the space with another colleague from the afternoon shift, so I will have to move the table every day, but I will accept it with joy if it makes me feel more comfortable and facilitates communication. In these times of technology and innovation I still think about the importance of the little things. A gesture will never change the health system but it can perhaps produce positive and unexpected effects. I will continue to take off my shoes when I start working to remind myself that my suty must be grounded and I will try to continue to look at their faces so I don't forget that we share the same life. I hope to be up to the task.
