Los triunfos y los títulos ofuscan. Es bueno alegrarse pero no confiarse. En estos tiempos de rapidez y confusión son muchos los partidos que se juegan a la vez y muchas las porterías que son amenazadas. Hoy nos levantamos con la resaca del mundial de fútbol, la alegría de unos y la pena de otros. Algo que lleva pasando desde la caída de Troya, que precisamente pereció por confiada. 

No puedo evitar reflexionar sobre la liga del Colegio de Médicos de Madrid. Una candidatura ganó las elecciones hace año y medio pero el astuto perdedor solicitó una pequeña demora para que los nuevos cargos firmasen. En este tiempo armó un recurso que hundió las aspiraciones de los vencedores. 

Miro hacia oriente, allí no veo mucho fútbol pero sí mucha ingeniería de sistemas. Se están merendando la industria automovilística, baterías, energía solar, bienes de equipo, construcción naval, infraestructura pesada, robótica industrial, telecomunicaciones… y modelos ampliados de lenguaje.

Miro a nuestras instituciones, carreteras, servicios públicos, sistema político, judicial… un imperio resquebrajado.

Miro a mis manos vacías de médico de pueblo. El último mes y medio he compartido consulta con un residente de segundo año en rotación rural. Les contaré lo que hemos hecho para no desfallecer. 

En primer lugar me propuse aportar una nueva perspectiva que nos ayudase a los dos en nuestra labor clínica. La prioridad era ampliar la mirada. Lo contaremos más adelante. Tan solo adelantar que nos ayudamos de Homero, y como no de Odiseo. Él también ganó su mundial para luego perderlo absolutamente todo. Salvo el ingenio. Y eso es lo que le llevó de regreso a la casilla de salida. 

La película de Nolan es una buena ocasión de recordarlo. No podemos permitirnos el lujo de seguir en Europa durmiéndonos en los laureles. Vienen curvas, fuegos y catástrofes. Es hora de empezar a pensar, dialogar y construir.

Triumphs and titles cloud the judgement. It is good to rejoice but not to grow complacent. In these times of speed and confusion, many matches are being played at once and many goals are under threat. Today we wake up with the hangover from the football World Cup, the joy of some and the sorrow of others. Something that has been happening since the fall of Troy, which precisely perished through complacency.I cannot help reflecting on the election at the Madrid College of Physicians. One slate won the elections a year and a half ago, but the astute loser requested a small delay so that the new office-holders could sign. In that time he prepared an appeal that sank the aspirations of the winners.I look eastwards; there I see little football but a great deal of systems engineering. They are devouring the automotive industry, batteries, solar energy, capital goods, shipbuilding, heavy infrastructure, industrial robotics, telecommunications… and large language models.I look at our institutions, roads, public services, political system, judiciary… a cracked empire.I look at my empty hands as a village doctor. For the last month and a half I have shared my surgery with a second-year resident on a rural rotation. I shall tell you what we have done so as not to despair.First of all, I set out to bring a new perspective that would help both of us in our clinical work. The priority was to broaden our gaze. We shall tell that story later. For now, I shall just mention that we turned for help to Homer, and of course to Odysseus. He too won his world cup only to lose absolutely everything afterwards. Everything except his ingenuity. And that was what took him back to the starting square.Nolan’s film is a good occasion to remember it. We in Europe cannot afford the luxury of continuing to rest on our laurels. Curves, fires and catastrophes are coming. It is time to start thinking, talking and building.

Tormenta perfecta en Medicina. The Perfect Storm in Medicine. 医学中的完美风暴

En verano son frecuentes las tormentas estivales vespertinas derivadas de la gran acumulación de agua atmosférica condicionada por la evaporación que producen las altas temperaturas. 

Cuando esta acumulación es extremadamente intensa y choca con una masa de aire muy frío en altura, la tormenta puede ser perfecta. Toda esa acumulación de agua cae de golpe… y origina catástrofes. 

En medicina venimos soportando altas temperaturas desde hace tiempo:

• Condiciones laborales adversas, convenio laboral mucho peor que el resto de trabajadores, exigencia de acumular 3-4 turnos sin descanso ni remuneración justa, sobrecarga laboral habitual, ninguneo institucional, falta de representatividad social (sindical, colegial, organizacional…).
• Aumento de las jubilaciones en los próximos años, huida de profesionales a otros países, aumento del burnout y de las enfermedades y bajas de causa laboral…
• Aumento del gasto sanitario en medicamentos y tecnología en detrimento de en recursos humanos, sobre todo en Atención Primaria. 

Por el horizonte se aproxima una masa de aire fría que puede quedar estrangulada por diversas circunstancias. Ese frío se llama Inteligencia Artificial y va a condicionar enormes cambios a todos los niveles, también en medicina. 

• Los algoritmos de lenguaje ampliado seguirán mejorando y sin duda serán capaces de hablar y simular cualquier perfil profesional, lo que permitirán que ejerzan multitud de roles. Administrativos de salud, secretariado y burocracia, cribado de pacientes, auxiliares de enfermería, auxiliares médicos… y seguirán escalando hasta bots personales de facultativo que hagan funciones de avatar de un profesional para tareas puntuales. 
• Las organizaciones sanitarias inevitablemente tendrán que adaptarse e incluirlos en sus cadenas de procesos dado que la curva de necesidades de salud sigue creciendo y la de recursos humanos cayendo. 
• Aparte tendremos el desarrollo de robots para procesos simples o dedicados por un lado y la enorme expansión de aplicaciones de consultoría médica personalizada para pacientes que trabajarán usando las pantallas de los propios pacientes. 

Pese a la negrura atmosférica no hay suficiente conversación ni entre los profesionales de la salud ni a nivel de la propia sociedad. El hombre del tiempo ya lo ha dicho, pero seguimos entretenidos con nuestras cosas. Hasta que se abran los cielos y tras la catástrofe nos volvamos a convertir todos en expertos. 

The Perfect Storm in Medicine

In summer, late-afternoon storms are common, triggered by the vast accumulation of atmospheric moisture driven by the evaporation that high temperatures produce.

When this accumulation is exceptionally intense and collides with a mass of very cold air at high altitude, it can create a perfect storm. All that collected water pours down all at once... causing catastrophes.

In medicine, we have been enduring high temperatures for quite some time now:

• Adverse working conditions.

• Employment contracts that are far worse than those of other workers.

• The requirement to log three to four consecutive shifts without rest or fair pay.

• Chronic workload overload.

• Institutional disregard.

• A lack of social representation (trade unions, professional bodies, organisations...).

We are also facing an increase in retirements over the coming years, an exodus of professionals to other countries, and rising burnout alongside work-related illnesses and sick leave. Concurrently, there is an increase in healthcare spending on medicines and technology at the expense of human resources, especially in Primary Care.

On the horizon, a cold air mass is approaching, which could become cut off due to various circumstances. This cold front is called Artificial Intelligence, and it is going to drive massive changes at all levels, including in medicine.

Large language models will continue to improve and will undoubtedly be capable of speaking and simulating any professional profile, allowing them to take on a multitude of roles: healthcare administrators, secretaries and bureaucrats, patient triage, nursing assistants, medical assistants... and they will keep escalating up to personal doctor bots acting as a professional's avatar for specific tasks.

Healthcare organisations will inevitably have to adapt and integrate them into their workflow chains, given that the curve of healthcare needs continues to grow while that of human resources declines.

Furthermore, we will see the development of robots for simple or dedicated processes on one hand, and the massive expansion of personalised medical consultation apps for patients on the other, operating directly on the patients' own screens.

Despite the gathering storm clouds, there is not enough discussion either among healthcare professionals or within society itself. The weatherman has already warned us, but we remain preoccupied with our own affairs. Until the heavens open and, in the wake of the catastrophe, we all turn into experts overnight.

在夏天，由于高温引起的蒸发导致大气中大量水分积聚，频繁发生夏季傍晚的暴风雨。

当这种积聚极其剧烈并与高空中的极冷气团相撞时，暴风雨可能会变得完美。所有这些积聚的水瞬间倾泻而下……并引发灾难。

在医学领域，我们很久以来就一直在忍受高温：

• 不利的工作条件，比其他劳动者差得多的劳动协定，要求连续承担 3-4 个班次而没有休息或公平的报酬，常态化的工作超负荷，体制性的忽视，缺乏社会代表性（工会、协会、组织……）。

• 未来几年退休人数增加，专业人员逃往其他国家，职业倦怠、职业病和因工请假增加……

• 医疗卫生支出在药品和技术上增加，从而损害了人力资源，尤其是在基层医疗。

地平线上正逼近一个冷气团，它可能会因各种情况而受阻/被孤立。这种寒冷被称为人工智能，它将在所有层面上带来巨大的变化，在医学领域也是如此。

大语言算法将继续改进，毫无疑问将能够说话并模拟任何职业形象，从而允许它们承担多种角色。卫生行政人员、秘书和官僚、患者筛选、护士助理、医生助理……并继续升级，直至成为充当专业人员替身以处理特定任务的个人医生机器人。

医疗卫生组织将不可避免地必须适应并将其纳入其流程链中，因为健康需求的曲线持续增长，而人力资源的曲线却在下降。

此外，一方面我们将迎来用于简单或专属流程的机器人发展，另一方面，面向患者的个性化医疗咨询应用程序将迎来巨大扩张，这些程序将利用患者自己的屏幕进行工作。

尽管天气一片漆称（漆黑），但在医疗卫生专业人员之间以及在社会自身层面上都没有足够的对话。天气预报员已经说过了，但我们仍在忙于自己的事情。直到天空裂开，在灾难发生后，我们又都再次变成了专家。
