La penitencia. Penance. 忏悔。

Llevo dos semanas seguidas sin pisar la consulta por causa de la huelga de médicos de familia de Madrid, la más larga que recuerdo. Ayer estuve en una manifestación, otra más, y no puedo dejar de pensar en esta crisis. Durante años hemos trabajado como el burro con anteojeras arando el campo. Sin parar. Detenerme estos días está permitiendo que afloren muchos fantasmas largo tiempo escondidos en los profundos fondos de armario de nuestra especialidad. Estamos viviendo una especie de cuaresma de purificación. Un desierto. Y en el pasado se nos decía que era necesario ayudarse de la oración, el ayuno y la abstinencia para superar las dificultades que inevitablemente encontraremos. Tras ver la nómina de enero creo que de ayuno y abstinencia vamos bien servidos, y en cuanto a la oración voy progresando, gracias a los generosos tiempos de silencio que esta quincena me está proveyendo. 


Es descorazonador ver como la negociación no avanza. No puede hacerlo, no hay voluntad política para ello. Porque esta situación, como comprenderán, es altamente política. La Consejería de Sanidad sigue la suya y denosta la de los demás. Y así seguimos, el Consejero contando a los medios lo malos que son sus facultativos y sus subalternos atendiendo al comité de huelga al que no pueden responder cómo implementarán las famosas agendas de 34 pacientes diarios. Algo que, como convertir el agua en vino, nadie sabe cómo podrá hacerse. 


Seguimos sin entender que no es cuestión de negociar un número concreto de pacientes al día, sino de garantizar que ninguna agenda médica supere lo admisible. Porque durante mucho tiempo hemos asumido que ver 50, 60 o 70 pacientes era lo que había, pero objetivamente es una barbaridad tanto para profesionales como para pacientes. Blindar la seguridad de pediatras y médicos de familia para proteger su salud mental debería ser la base del sistema. Y por supuesto la del resto de profesionales, aunque me temo que si tuvieran agendas parecidas ya habría ardido la sanidad hace mucho tiempo. 


La negociación con un sindicato o en una mesa sectorial es un puente pero no puede ser el único a la hora de afrontar un problema tan complejo como gestionar la Atención Primaria que precisa acciones poliédricas en muchos ejes. Será necesario crear más, dado que la política de parches parece haber dado todo lo que podía. Las ideas y propuestas de mejora que están aflorando estos días son muy valiosas. Será necesario canalizarlas y trabajar por su implementación en líneas de arriba a abajo y de abajo a arriba. 


Reconozco que me flaquea la fe. He perdido la confianza en mis gestores y con alarma percibo que de alguna forma si la situación revienta me da igual. En estos momentos de desesperación me veo haciendo huelga hasta final de año y comiendo patatas. Lo único que tengo por cierto es que nadie asumirá la responsabilidad por el servicio devastado y la enorme cantidad de profesionales enfermos, heridos o de baja por esta situación. Un millón de madrileños no tiene pediatra o médico de familia asignado. En poco tiempo serán bastantes más. Y no hay plan real con medidas concretas para afrontarlo, solo palabras, palabras, palabras. 









Penance.


I have been two weeks in a row without going to the doctor's surgery because of the family doctors' strike in Madrid, the longest I can remember. Yesterday I was at a demonstration, yet another one, and I can't stop thinking about this crisis. For years we have worked like the donkey with blinkers ploughing the field. Without stopping. Stopping these days is allowing many ghosts long hidden in the deep cupboard depths of our speciality to surface. We are living a kind of Lent of purification. A desert. And in the past we were told that prayer, fasting and abstinence were necessary to overcome the difficulties we will inevitably encounter. After looking at January's payroll I think we are well served by fasting and abstinence, and as for prayer I am making progress, thanks to the generous times of silence that this fortnight is providing me with. 



It is disheartening to see how the negotiations are not progressing. It cannot, there is no political will for it. Because this situation, as you will understand, is highly political. The Regional Ministry of Health follows its own line and denigrates that of others. And so we go on, the Regional Minister telling the media how bad his doctors are and his subordinates attending to the strike committee to whom they cannot answer how they will implement the famous 34-patient-daily agendas. Something that, like turning water into wine, nobody knows how it can be done. 



We still do not understand that it is not a question of negotiating a specific number of patients per day, but of ensuring that no medical agenda exceeds what is admissible. Because for a long time we have assumed that seeing 50, 60 or 70 patients was the norm, but objectively it is an outrage for both professionals and patients. Shielding the safety of paediatricians and family doctors to protect their mental health should be the basis of the system. And of course that of other professionals, although I fear that if they had similar agendas the health system would have burnt down long ago. 



Negotiation with a trade union or at a sectoral table is a bridge, but it cannot be the only one when it comes to tackling a problem as complex as managing primary care, which requires multifaceted actions on many axes. It will be necessary to create more, given that the patchwork policy seems to have given all it could. The ideas and proposals for improvement that are emerging these days are very valuable. It will be necessary to channel them and work for their implementation along top-down and bottom-up lines. 




I admit that my faith is shaken. I have lost confidence in my managers and with alarm I perceive that somehow if the situation blows up I don't care. In these moments of despair I see myself going on strike until the end of the year and eating potatoes. The only thing I know for sure is that no one will take responsibility for the devastated service and the enormous number of professionals sick, injured or on sick leave because of this situation. One million Madrilenians have no paediatrician or family doctor assigned to them. In a short time they will be many more. And there is no real plan with concrete measures to tackle it, just words, words, words.
