Mañana día 12 de febrero a las 19 horas tendrá lugar la presentación del poemario Arconte Enfurecido en el Colegio de Médicos de Madrid sito en la calle Santa Isabel 51.
En el panorama de narrativas contemporáneo destaca Ana Merino con su reciente premio Nadal por El mapa de los afectos, un libro escrito por una autora que sabe lo que dice y lo sabe decir. Nos comparte la visión de una catedrática de escritura creativa en Iowa, por un lado ofreciendo su oficio de escritora, por otro el hecho de ser ciudadana del medio oeste americano que será el escenario de la obra. La composición del texto en capítulos prácticamente independientes hace que sea muy fácil de leer a lo que ayuda el maduro uso del lenguaje, la economía de medios superfluos y la enorme base poética de la autora. La trama y los personajes permiten ofrecer un abanico de acciones y escenas donde cabe la vida y la muerte, el horror y la ternura. No es un libro sobre la bondad, más bien sobre la humanidad que contiene siempre un lado de luz y otro de sombra. Tampoco se puede decir que ganen los buenos, en eso se aleja del discurso manido de tanta cultura cinematográfica de masas y se acerca a la vida misma que, a mi criterio, dibuja con delicadeza y precisión. Creo que los lectores reconocerán a los numerosos personajes que les recordaran familiares, amigos o conocidos, además de encontrar en sus gestos o circunstancias vivencias propias similares. Porque este libro es a fin de cuentas como un pantallazo de un GPS convertido en un gran mapa de humanidad, algo que solo se puede hacer con la magia de la gran literatura.
Human cartographies
In the contemporary narrative scene, Ana Merino stands out with her recent Nadal Award for El mapa de los afectos, a book written by an author who knows what she is saying and can say it properly. She shares with us the vision of a professor of creative writing in Iowa, on the one hand offering her craft as a writer, on the other hand the fact of being a citizen of the American Midwest that will be the scene of the work. The composition of the text in practically independent chapters makes it very easy to read which is helped by the mature use of language, the economy of superfluous means and the enormous poetic base of the author. The plot and characters allow for a range of actions and scenes where life and death, horror and tenderness fit. It is not a book about goodness, but rather about humanity that always contains one side of light and the other of shadow. Nor can it be said that the good guys win, moving the text away from the hackneyed discourse of so much mass film culture and stands closer to life itself which, in my opinion, draws with delicacy and precision. I believe that readers will recognize the many characters who remind them of family members, friends or acquaintances, as well as find similar experiences of their own in their gestures or circumstances. Because this book is ultimately like a screenshot of a GPS turned into a great map of humanity, something that can only be done with the magic of great literature.

Si, ya lo sabemos, la sanidad se hunde. Lo lleva haciendo años por múltiples motivos que no vamos a repetir hoy. Sin embargo todavía podemos hacernos preguntas y comparto la que me estoy haciendo hoy, ¿qué puedo aportar como profesional de la salud? 1. Lo que los demás esperan de mí. La sociedad espera que sus sanitarios sean eficaces, bien formados, trabajadores y útiles. Que sean buenos técnicos y sepan solucionar sus problemas. Que además sean humanos. 2. Lo que espera el sistema sanitario de mi. Eficacia y productividad. Sacar adelante el máximo trabajo posible de la mejor manera. Asumir todo lo que llegue, incluso cuando la demanda sea excesiva por falta de compañeros, enfermedades invernales u otras causas. Que no me queje mucho. 3. Lo que yo espero de mi.
Conseguir trabajar de médico lo mejor que sé pese a que las condiciones de sobrecarga a menudo lo hagan difícil.
Paciencia con la dificultad del trabajo con personas que sufren y un sistema que no está pensado para permitir encuentros clínicos de calidad.
Asertividad para tener el valor de decir NO cuando haya que decirlo a pacientes, compañeros y superiores.
Creatividad para tener la capacidad de contemplar las situaciones complejas de mis pacientes desde otros puntos de vista. Poder escribir reflexiones, poemas o dislates y encima compartirlos.
No olvidarme de reír.
¿Sirve de algo que siga publicando un blog? Como no busco ganancia me lo puedo permitir igual que quien tira piedras al río o lanza mensajes en botellas al mar. Tal vez algún día seamos muchos lo que consigamos que en lugar de plásticos los océanos estén llenos de versos e ingenio.
Instead the collapse of the health system, What can health professionals do? Yes, we know, the health system is collapsing. It's been doing so for years for multiple reasons that we're not going to repeat today. However, we can still ask ourselves questions and I share one I am asking myself today, How can I help others as a health professional?
1. What others expect from me. Society expects their health care providers to be effective, well-trained, hard-working and useful. That they are good technicians and know how to solve their problems. They should also be humane. 2. What the health system expects from me.
Efficiency and productivity. To carry out the maximum work possible in the best way. To take on everything that comes along, even when the demand is excessive due to lack of colleagues, winter illnesses or other causes. Not to complain too much.
3. What I expect from me.
To be able to work as a doctor to the best of my ability, even though overloaded conditions often make it difficult.
Patience with the difficulty of working with people who suffer and a system that is not designed to allow quality clinical encounters.
Assertiveness to have the courage to say NO when it comes to telling patients, colleagues and superiors.
Creativity to have the capacity to contemplate the complex situations of my patients from other points of view. To be able to write reflections, poems or speeches and share them.
Not to forget to laugh.
Is it useful to keep publishing a blog?
As I do not seek profit I can afford it just like someone who throws stones into the river or throws messages in bottles into the sea.
Perhaps one day there will be many of us who will manage to make the oceans full of verse and artfulness instead of plastic.
Comparto una entrevista que me hicieron en las jornadas de la Asociación de Salud Digital del año pasado en las que hablo de Internet para sanitarios, presencia en redes sociales, marca personal, valor social, poesía y ruido de fondo.
El 12 de febrero tendrá lugar en Madrid la próxima edición de las mismas con inscripción gratuita.
Nos han vendido el progreso como la religión imperial. Nos han dicho que la tecnología resolvería todos nuestros problemas. Era mentira. Lo primero nos lleva hacia el acantilado del desastre ambiental, lo segundo hacia escenarios de control y poder absoluto.
No es plato de buen gusto ser portador de malas noticias. Pero son las que son. Hemos llegado a un punto que no tendrá retorno y si no enmendamos la dirección lo que encontremos no nos gustará nada.
Por otra parte nadie puede imaginar lo que está a punto de emerger. Hemos recibido visiones y sueños pero lo que viene es mucho más grande de lo que podemos concebir. Por eso necesitamos más poetas, locos e iluminados que traduzcan a un lenguaje entendible esos augurios.
Las modernas distracciones globales parecen intentar que la atención social descanse en asuntos alejados de lo que verdaderamente lo merece. La causa de que el mundo esté en llamas radica en la decisión de incontables sujetos de dilapidar dicha atención. Encontrar narrativas que pongan el foco en lo verdaderamente importante es un reto para esta sociedad. La poesía puede ayudarnos en esa empresa al permitirnos grados de libertad que la prosa no ofrece.
Dante fue un valiente por atreverse a cartografiar la sombra en verso. Bajar a los infiernos es fácil, lo complicado es salir de ellos y contarlo. Repetir esa gesta en nuestro tiempo es igual de arriesgado, uno pude quedarse pegado a la sombra o ver su oscuro mensaje rechazado. Seguimos su estela y lo que nos encontramos es a la vez viejo y nuevo, esperado y sorprendente. Pero debemos recordar que la facultad más valiosa de los seres humanos no es su inteligencia sino su capacidad de imaginar lo que no ha visto. Desde ahí es posible visualizar una nueva escatología.
El proyecto Arconte Enfurecido nació de una inspiración que produjo visiones que fueron convertidas en verso durante varias semanas. Lo que surgió produjo un sentimiento de extrañeza en el autor que no se veía reflejado en el texto por lo que fue depositado en un cajón donde permaneció un año. Pasado ese plazo en un nuevo sueño aparecía el poemario a la par que un amigo, lo que animó al autor a contactar con él y compartirle sus dudas. Afortunadamente esa persona aportó lo que en ese momento se necesitaba: confianza. Su ayuda ha sido decisiva para editar e ilustrar la obra.
El proceso de ilustración contó con la participación de Paula Alvear que con paciencia acogió las palabras y orientaciones del autor para crear unas imágenes que dotan la obra del ambiente onírico y expectante que la caracteriza.

La intención del libro es sembrar semillas de reflexión y toma de conciencia que ayuden al lector a rescatar las preguntas que le permitan mirar a sus postrimerías personales. Cada ser humano contiene infierno, cielo y purgatorio, y merece la pena aprender a convivir con ellos.
La obra se ha impreso en una edición limitada en formato cuadrado con papel de calidad de gran gramaje cuyas copias firmadas se pueden conseguir en las presentaciones de la obra o solicitándolas por correo electrónico (info@doctorcasado.es).
Se ha preparado una versión en formato electrónico apta para móviles, tabletas y libros electrónicos, así como otra versión en papel en tamaño normalizado disponibles ambas en el portal de Amazon (a partir del 30 de enero).

They have sold us progress as the imperial religion. We've been told that technology would solve all our problems. That was a lie. The first leads us to the cliff of environmental disaster, the second to scenarios of control and absolute power.
It's not easy for anyone to be the bearer of bad news. But it is what it is. We have reached a point of no return, and if we do not make amends for what we find, we will not like it at all.
On the other hand no one can imagine what is about to emerge. We have received visions and dreams but what is to come is much greater than we can conceive. That's why we need more poets, madmen and enlightened people to translate these predictions into understandable language.
Modern global distractions seem to be trying to put social attention on issues far removed from what it really deserves. The cause of the world being on fire is the decision of countless subjects to squander such attention. Finding narratives that focus on what is truly important is a challenge for this society. Poetry can help us in that endeavour by allowing us degrees of freedom that prose does not offer.
Dante was courageous in daring to map the shadow in verse. Going down to hell is easy, what is complicated is getting out of it and telling about it. To repeat that feat in our time is just as risky, one could stay stuck in the shadow or see his dark message rejected. We follow its trail and what we find is both old and new, expected and surprising. But we must remember that the most valuable faculty of human beings is not their intelligence but their ability to imagine what they have not seen. From there it is possible to visualize a new eschatology.
The Angry Archon project was born from an inspiration that produced visions that were turned into verse for several weeks. What emerged produced a feeling of strangeness in the author so it was placed in a drawer where it remained for a year. After that time, in a new dream, the book of poems appeared along with a friend, which encouraged the author to contact him and share his doubts. Fortunately, this person contributed what was needed at that moment: trust. His help has been decisive in editing and illustrating the work.
Paula Alvear took part in the illustration process and patiently accepted the author's words and guidelines to create images that give the work the dreamlike and expectant atmosphere that characterizes it.

Explicar matemáticas no se nos suele dar bien a los profesionales sanitarios pero en esta época de noticias falsas y medias verdades cada vez es más necesario.
Pongamos un ejemplo reciente. Imaginen que elevamos al máximo la tensión político militar de un país. Imaginen que en un momento dado alguien tiene la duda de que un objeto volante pueda ser un avión enemigo. Imaginen que aprieta un botón y un misil derriba un avión de pasajeros. Esto es un falso positivo, etiquetar como nocivo algo que no lo es.
En medicina lo hacemos con frecuencia dado que toda técnica diagnóstica, subrayo toda, produce falsos positivos. Desde las más sencillas a las más sofisticadas, con mayor o menor probabilidad, es un hecho objetivo que aumenta a medida que lo hace el número de pruebas que se llevan a cabo.
Como el mantra “más es mejor” contamina los servicios sanitarios, la ciudadanía reclama todas las pruebas posibles “porque yo lo valgo” sin saber el peligro que corren por someterse a ellas. Si una prueba se interpreta como positiva implicará otras pruebas o medidas. De este modo si el análisis de heces detectó sangre oculta se ordenará una colonoscopia y si la mamografía identifica una imagen extraña una punción biopsia y en ambos casos resección de lo que se considere sospechoso.
Existe un gran debate con los programas de cribado que se aplican a población sana por ser de alguna manera una forma de elevar la tensión “político militar” sanitaria. Mientras más enfermedad se espere, más se encontrará (verdaderos positivos) y de paso generamos un buen puñado de efectos colaterales (faltos positivos).
A nadie le gustaría que a un familiar o a uno mismo le derribará un misil por error pero no es tan evidente que una prueba médica pueda potencialmente ser igual de peligrosa. Y si miramos la cifras de personas que mueren por reacciones a contrastes, perforaciones, sangrados, complicaciones y demás veremos que superan al año las de varios aviones. Si incluimos las personas a las que se les opera, amputa o somete a procesos innecesarios por falsos positivos la lista se alargaría muchísimo más.
Por estas razones me parecería de mucha ayuda incluir en las hojas de consentimiento informado habituales una pequeña infografía donde se ilustre este tema para que quien tenga que someterse a una prueba diagnóstica sepa a qué probabilidades se somete tanto de posibles complicaciones como de falsos positivos. El consejo y el criterio del profesional sanitario seguirá siendo imprescindible pero quizá los números ayuden por sí mismos a más de uno.
False positives, the sanitary Achilles' heel. Explaining mathematics is not usually easy to health professionals but in this age of fake news and half-truths it is increasingly necessary.
Let's take a recent example. Imagine that we raise the political-military tension in a country to the maximum. Imagine that at some point someone has doubts that a flying object could be an enemy plane. Imagine that you push a button and a missile knocks down a passenger plane. This is a false positive: to label as harmful something that is not.
In medicine we often do this because every diagnostic technique, I emphasize, produces false positives. From the simplest to the most sophisticated, with greater or lesser probability, it is an objective fact that increases as the number of tests carried out increases. As the mantra "more is better" contaminates the health services, citizens demand all possible tests "because I am worth it" without knowing the danger they run by submitting to them. If a test is interpreted as positive it will imply other tests or measures. Thus, if the stool analysis detected occult blood, a colonoscopy will be ordered and if the mammography identifies a strange image, a puncture biopsy and in both cases a resection of what is considered suspicious. There is a great deal of debate with screening programs that are applied to healthy populations because they are somehow a way of raising the "political-military" health tension. The more disease you expect, the more you will find (true positives) and in the process we generate a good handful of side effects (false positives). No one would like to see a family member shot down by mistake but it is not so obvious that a medical test can potentially be just as dangerous. And if we look at the numbers of people who die from reactions to contrast, punctures, bleedings, complications and so on, we see that they exceed the numbers of several crashed planes in a year. If we include the people who are operated on, amputated or subjected to unnecessary procedures for false positives the list would be much longer.
For these reasons I think it would be very helpful to include in the usual informed consent forms a small computer graphic illustrating this issue so that anyone who has to undergo a diagnostic test knows what the chances are of both possible complications and false positives. The advice and judgement of the health professional will still be essential but perhaps the numbers themselves will help more than one.

Los Reyes Magos de hoy en día están rematadamente ciegos. Su fina sensibilidad colapsó al enfrentarse a incontables pantallas, focos, anuncios y noticias que quemaron unas pupilas acostumbradas a vigilar estrellas. Vagan por el desierto pero no saben hacia donde, en el camino se cruzaron con trampas que confiscaron toda posibilidad de orientación. En sus camellos van consultando el móvil sin cesar pero ninguna aplicación les sirve para encontrar la plenitud que añoran. La Luz que llevan toda la vida buscando está naciendo en el barro de un campo de refugiados, bajo unos plásticos, y ningún influencer ni canal televisivo cubrirá la noticia. No lo verán en sus redes sociales y se lo perderán. Regresarán a sus casas con las manos y el corazón vacío mientras los reyes del mercado se solazan en sus palacios digitales aprovechándose del hambre de sentido de las perdidas muchedumbres.

The Three Wise Men of today are all blind. Their fine sensibility collapsed when faced with countless screens, spotlights, advertisements and news items that burned out their pupils used to watching stars. They wander through the desert but don't know where becouse on the way they come across traps that confiscate any possibility of orientation. On their camels they consult their mobile phones incessantly, but no application helps them find out the fullness they long for. The Light they have been looking for all their lives is being born in the mud of a refugee camp, under some plastic, and no influencer or television channel will cover the new. They will not see it in their social networks and they will miss it. They will return home with empty hands and empty hearts while the kings of the marketplace bask in their digital palaces, taking advantage of the lost crowds' hunger for meaning.
La humanidad constituye un ente singular. Encarna la paradoja de ser por un lado una gran tribu de primates y al mismo tiempo tener la capacidad de desplegar sorprendentes propiedades emergentes.
Sin embargo pudo más su deseo insaciable que su inteligencia y no está siendo capaz de evitar un previsible colapso civilizatorio global. Los ajustes necesarios nos obligarían a iniciar un proceso de decrecimiento para el que nadie está preparado, ni siquiera los ecologistas más convencidos o los científicos más conscientes.
Estamos en una situación de emergencia global ante lo que probablemente sea el mayor reto que como especie hayamos enfrentado. Y no tenemos una buena jugada, las cartas son malas. En situaciones así estamos en manos de la providencia y dependemos de lo inesperado. Es verdad que la capacidad creativa y de adaptación del ser humano es enorme. También que bajo presión somos capaces del mayor heroísmo. Habrá que ver.
Creo que es importante dejar espacio a la esperanza. Vamos a tener que soportar mil discursos catastrofistas que de paso tratarán de vendernos desde productos de consumo a ideas políticas. El miedo, el agobio y la furia alcanzarán cotas nunca vistas dado que las emociones se contagiarán globalmente y las masas se moverán ciegas y terribles.
Dicen los místicos que en el mismo centro de la noche oscura del alma habita una presencia que ilumina. Cuando nos toca atravesar una crisis vital terminamos recurriendo a esa fuente que sacia nuestra sed de sentido, equilibrio y transcendencia. Quizá también ahora sea sensato buscar ahí las respuestas que cada cual precise en lugar de dejarnos llevar por la locura colectiva y la tormenta de ruido de fondo que ya empieza a desatarse ahí afuera.
Humanity is a unique entity. It embodies the paradox of being a great tribe of primates and at the same time having the ability to unfold amazing emergent properties.
However, it’s insatiable desire prevailed over its intelligence and is not being able to avoid a predictable global civilisational collapse. The necessary adjustments would force us to initiate a process of decline for which no one is prepared, not even the most convinced ecologists or the most conscientious scientists.
We are in a situation of global emergency in the face of what is probably the greatest challenge that we as a species have ever faced. And we don't have a good move, the cards are bad. In situations like this we are in the hands of providence and we depend on the unexpected. It is true that the creative and adaptive capacity of human beings is enormous. It is also true that under pressure we are capable of the greatest heroism. We will have to see.
I think it is important to leave room for hope. We are going to have to endure a thousand catastrophic speeches that will try to sell us from consumer products to political ideas in the process. Fear, oppression and anger will reach unprecedented heights as emotions will be globally transmitted and the masses will move blindly and terribly.
The mystics say that in the very centre of the dark night of the soul there is a presence that illuminates. When we have to go through a life crisis we end up turning to that source that satisfies our thirst for meaning, balance and transcendence. Perhaps now it's also clever to look for the answers we each need there instead of letting ourselves be carried away by the collective madness and the storm of background noise that's already beginning to break out there.
