Empezar. Start.





Cada semana hago unas 14 ecografías. Son ecoscópias rápidas, no me suelen llevar más de 6 minutos. Es verdad que motivan profesionalmente al aumentar la capacidad diagnóstica y poder afinar más y que también lo hacen a nivel personal al desvelar un poquito más esa zona invisible que se oculta al otro lado de la superficie del paciente. Esa posibilidad de mirar más allá siempre me ha fascinado y probablemente sea lo que me ha llevado a elegir ser generalista. Aunque si me paro a pensar, he de reconocer que lo que me sigue dando más satisfacciones es otra tecnología mucho más antigua que el ultrasonido. Para ello sólo necesito un par de sillas y algo de tiempo para escuchar. Cuando le pongo la suficiente atención, los años de experiencia y capacitación me abren puertas a mundos sorprendentes, los que mis pacientes me traen a consulta cada día. 


Quizá debería ser más crítico conmigo y reconocer que sigo quedándome fascinado con la tecnología y esperando más de ella de lo que realmente da. Tratar de reconocer la energía y el tiempo de más que dedicó a quien menos lo necesita y lo que realmente ofrezco a los más necesitados. Pero no es fácil, lleno como estoy de traicioneros sesgos cognitivos que, a fin de cuentas, son lo que constituyen lo que llamamos identidad. Por eso, si tengo que empezar a replantearme lo que hago es necesario regresar a la preguntas fundamentales, ¿qué sentido tiene todo esto?, ¿para qué lo hago?, ¿para qué sirve?. Y al mirarlas de cerca sonreír al comprobar que son pura filosofía. Al final todos queremos encontrar el camino hacia una buena vida y un buen ejercicio profesional. Lastima que hoy la niebla informativa nos entorpezca encontrar referentes de calidad que guíen nuestros caminos. Aunque quizá buscar excusas o defendernos con la queja no sean más que fútiles intentos para intentar renegar de nuestra responsabilidad. Pese a las circunstancias es posible desarrollar humanidad, como nos enseñan tantas personas sometidas a las más penosas situaciones. Esa posibilidad es la que siento con fuerza que debo transmitir a mis pacientes. Para ello es fundamental que yo mismo la encarne ante mis propias dificultades. Por eso he decidido detenerme estos meses y hoy acometo esta reflexión. Es verdad que sigue siendo fundamental la denuncia de la injusticia o el error dentro del mundo profesional, pero he acabado agotado de tanta protesta en un medio que es muy difícil de cambiar. Si he de centrarme en algo elijo la relación medico-paciente que sigo considerando lo más valioso que esta profesión puede darme. Lo demás es secundario y como tal lo he de manejar.


Dado que en mi nuevo destino tengo la posibilidad de ser docente para estudiantes de medicina y residentes creo que puede ser buena idea reorientar este cuaderno virtual en esa dirección. A fin de cuentas uno empieza a comprender algo cuando es capaz de enseñarlo y compartirlo con otros, sea esa la intención que guíe mis pasos.

Every week I do about 14 ultrasound scans. They are quick ultrasounds, they don't usually take me more than 6 minutes. It is true that they are professionally motivating as they increase my diagnostic capacity and allow me to be able to refine my diagnosis, and they are also motivating on a personal level as they reveal a little more of that invisible area that is hidden on the other side of the patient's surface. That possibility of looking beyond has always fascinated me and is probably what led me to choose to become a generalist. But if I think about it, I have to admit that what still gives me the most satisfaction is another technology that is much older than ultrasound. All I need is a couple of chairs and some time to listen. When I pay enough attention to it, years of experience and training open doors to amazing worlds, which my patients bring to me every day. 


Perhaps I should be more critical of myself and recognise that I keep getting fascinated with technology and expecting more from it than it actually delivers. Try to recognise the extra energy and time I devote to those who need it least and what I really offer to those who need it most. But it is not easy, filled as I am with treacherous cognitive biases that, in the end, are what constitute what we call identity. So if I am to begin to rethink what I do, it is necessary to go back to the fundamental questions, what is the point of it all, what am I doing it for, what is it good for? . And when you look at them closely, you can smile when you realise that they are pure philosophy. In the end we all want to find the way to a good life and a good professional practice. It is a pity that today's information fog hinders us from finding quality references to guide our paths. Perhaps, however, making excuses or defending ourselves by complaining are nothing more than futile attempts to try to deny our responsibility. In spite of the circumstances, it is possible to develop humanity, as so many people who have been subjected to the most painful situations teach us. It is this possibility that I feel strongly that I must pass on to my patients. In order to do so, it is essential that I myself embody it in the face of my own difficulties. That is why I have decided to pause for a few months and today I am undertaking this reflection. It is true that it is still essential to denounce injustice or error in the professional world, but I have become exhausted by so much protest in an environment that is very difficult to change. If I have to focus on something, I choose the doctor-patient relationship, which I still consider to be the most valuable thing this profession can give me. The rest is secondary and I have to handle it as such.


Since I have the opportunity to teach medical students and residents in my new position, I think it might be a good idea to reorient this virtual notebook in that direction. After all, one begins to understand something when one is able to teach it and share it with others, be that the intention guiding my steps.

Gratitud. Gratitude. 感恩的心

Esta semana he participado en las Jornadas de Ecografía de la Semfyc en Alcalá de Henares, asistiendo en calidad de invitado y de novato. Me propusieron dar la conferencia de clausura y aproveché para mejorar mi incipiente formación dado que en este ámbito soy del todo novel.


Por un lado me sentía extraño dado que en Madrid los pediatras y médicos de familia llevamos dos meses de huelga y eso no parece muy compatible con dedicar mucha energía a la formación, pero por otro experimento una enorme gratitud por haber participado en un acontecimiento cuyo valor excede su función. Además de aprender me he llevado a casa un buen montón de abrazos a personas que admiro y la satisfacción de reconocer que sigo siendo médico, y eso, peso a las amargas circunstancias, es para mí motivo de alegría. 


El formato de las Jornadas ha sido muy bueno, 400 inscritos acogidos por un equipo profesional y una organización que han cumplido objetivos. Ha sido todo muy fácil, con un menú a base de talleres, salas prácticas, mesas, comunicaciones y demás. Una mayoría de residentes (80%), acompañados de tutores y compañeros senior, 2/3 de fuera de Madrid según mi estimación. 


Creo que vamos en buena dirección favoreciendo este tipo de encuentros prácticos que permiten adquirir habilidades técnicas y compartir conocimiento. En mi caso lo prefiero en comparación al nuevo formato docente de 20 charlas teóricas en un día que considero excesivo. Un encuentro médico debe obligatoriamente cuidar los pasillos, permitir cafés, favorecer conversaciones. Sin ellas nuestros curriculum formativos seguirán creciendo pero no crearemos ni cuidaremos relaciones profesionales que son imprescindibles dado que siempre habrá compañeros que podrán aportarnos su experiencia, conocimientos, narrativas y además escuchar las nuestras. 


Mención especial tiene haber podido conocer personalmente a Yale Tung. Participó en una mesa plenaria y nos regaló un buen montón de pistas formativas. Un material valiosísimo que por mi cuenta me hubiera requerido años de búsqueda y que él nos hilvanó de la manera más sencilla. Yale, además de ser un internista de prestigio en el Hospital La Paz de Madrid, y un ecografista líder en el panorama internacional, es buena persona. Quizá ese adjetivo parezca simple pero no lo es. No es fácil encontrar médicos asistenciales que transmitan esa mezcla de facilidad, alegría, sencillez y cercanía que él irradia.


Los que llevamos tiempo en estas lides sabemos lo que cuesta organizar un evento. Las horas de trabajo que hay detrás y que no se ven. Por eso doy las gracias a todo el equipo que ha logrado la proeza de reunir y formar a 400 médicos en Madrid. Hoy nos veremos en la manifestación de la Castellana, la vida sigue. 






Gratitude

This week I participated in the Semfyc Ultrasound Conference in Alcalá de Henares, attending as a guest and as a novice. I was asked to give the closing lecture and I took the opportunity to improve my incipient training, given that I am a complete novice in this field.

On the one hand I felt strange given that in Madrid paediatricians and family doctors have been on strike for two months and that does not seem very compatible with devoting a lot of energy to training, but on the other hand I feel an enormous gratitude for having participated in an event whose value exceeds its function. In addition to learning, I have taken home a lot of hugs from people I admire and the satisfaction of recognising that I am still a doctor, and that, despite the bitter circumstances, is a source of joy for me. 

The format of the Conference has been very good, 400 registered participants welcomed by a professional team and an organisation that has fulfilled its objectives. It was all very easy, with a menu based on workshops, practical rooms, tables, communications and so on. A majority of residents (80%), accompanied by tutors and senior colleagues, 2/3 from outside Madrid according to my estimate. 

I think we are moving in the right direction by favouring this type of practical meetings that allow us to acquire technical skills and share knowledge. In my case, I prefer it to the new teaching format of 20 theoretical talks in one day, which I consider excessive. A medical meeting must necessarily take care of the corridors, allow for coffee breaks, encourage conversations. Without them, our training curricula will continue to grow, but we will not create or nurture professional relationships, which are essential, given that there will always be colleagues who can contribute their experience, knowledge and narratives and also listen to our own. 

It is worth mentioning that I was able to meet Yale Tung in person. He participated in a plenary table and gave us a lot of formative hints. Valuable material that would have taken me years of searching on my own, and which he put together for us in the simplest of ways. Yale, besides being a prestigious internist at the Hospital La Paz in Madrid, and a leading ultrasonographer on the international scene, is a good person. That adjective may sound simple, but it is not. It is not easy to find doctors who transmit that mixture of ease, joy, simplicity and closeness that he radiates.

Those of us who have been in this field for a long time know what it takes to organise an event. The hours of work that go into it and that are not seen. That is why I would like to thank the whole team that has achieved the feat of bringing together and training 400 doctors in Madrid. We will see you today at the demonstration in the Castellana, life goes on.

值得一提的是,我能够亲自见到耶鲁大学的学生。他参加了全体会议,给了我们很多形成性的提示。这些宝贵的材料本来需要我自己花数年时间去寻找,而他以最简单的方式为我们整理了这些材料。耶鲁除了是马德里拉巴斯医院著名的内科医生和国际舞台上领先的超声学专家外,还是一个好人。这个形容词可能听起来很简单,但它并不简单。要找到像他所散发的那种混合着轻松、快乐、简单和亲密的医生并不容易。
