Monocromía sanitaria. Sanitary monochromy. 衛生單色。
Por mucho que las tecnologías de la comunicación avancen nuestra sociedad sigue teniendo una enorme inercia a la hora de mezclarse. A nivel profesional es más evidente si cabe. Elijan el medio que elijan será fácil comprobar que cada oveja se junta con su pareja y no con las de otro pelaje. En el mundo sanitario pasa igual. Cada categoría profesional monta sus reuniones científicas, congresos y demás, se apoya en sus propios sindicatos, colegios y organizaciones, mirando con suspicacias cuando no con franca hostilidad a los que no son propios.
A nivel de organización sanitaria no ha habido nadie que haya sabido incentivar el trabajo en equipo, las jornadas multidisciplinares o los congresos híbridos, salvo honrosas excepciones habitualmente fuera del paraguas del sistema.
La imagen de la distribución en Twitter de nuestros políticos es muy aclaratoria de lo que estoy diciendo. Si lo hiciésemos en sanidad tendríamos muy definidos los distintos estamentos. Como me considero una excepción aporto los pros y contras de salirse de este patrón.
Pros:
1. Mayor amplitud de miras.
2. Mejor comprensión de la complejidad.
3. Mejor tolerancia a la incertidumbre.
4. Información contrastada de más calidad.
5. Mayor posibilidad de acceder a ideas, proyectos y personas.
6. Más fuentes de inspiración.
Contras:
1. Mayor probabilidad de conflicto.
2. Aumento de la suspicacia de tus pares.
3. Mayor posibilidad de ser malinterpretado.
4. Mayor posibilidad de cruzarse con trolls.
5. Necesidad de cuidar más el lenguaje y los mensajes.
Dada la complejidad del sistema sanitario como organización y la de los pacientes de más edad y polimedicados que cada vez son más numerosos, ¿nos lo podemos seguir permitiendo?
Sanitary monochromy.
As much as communication technologies advance, our society still has an enormous inertia when it comes to mixing. On a professional level it is even more evident. Whichever medium you choose, it will be easy to see that each sheep joins its partner and not those of another coat. It's the same in the sanitary world. Each professional category mounts its scientific meetings, congresses and others, relies on its own unions, schools and organizations, looking suspiciously if not with frank hostility to those who are not their own.
At the level of health organisation, no one has been able to encourage teamwork, multidisciplinary conferences or hybrid congresses, except for honourable exceptions that are usually outside the umbrella of the system.
The image of the Twitter distribution of spanish politicians is very clarifying of what I am saying. If we did it in health we would have very defined the different estates. As I consider myself an exception, I contribute the pros and cons of getting out of this pattern.
Pros:
1. More open-mindedness.
2. Better understanding of complexity.
3. Better tolerance for uncertainty.
4. Better quality contrasted information.
5. Greater possibility to access ideas, projects and people.
6. More sources of inspiration.
Cons:
1. Greater likelihood of conflict.
2. Increased suspicion of your peers.
3. Greater chance of being misinterpreted.
4. Greater chance of encountering trolls.
5. Need to take more care of language and messages.
Given the complexity of the health system as an organisation and that of older and polymedicated patients who are becoming more and more numerous, can we continue to allow it?
衛生單色。

- Disminución de la burocracia en consulta.
- Asistentes personales en salud al servicio de profesionales.
- Teleconsulta en entornos de realidad virtual.

Propuestas de mejora de la gestión:
- Desburocratización
- Eliminación de cursos de acción ineficientes (no hacer).
- Tránsito de una gestión por indicadores cuantitativos autogestionado por cada profesional a otro automatizado.

- Formación continuada en fundamento teórico de Atención Primaria (biopsicosocial, comunicación, comunitaria, enfoque familiar, determinantes sociales en salud…).
- Cadenas de motivación.
- Incentivación del trabajo en equipo.
- Protección de sobrecargas.
- Aumento de la perspectiva: dimensión biopsicosocial y existencial, dimensión bioética (valores).
- Mejora de la consciencia, compasión y delicadeza del profesional. Modelo del sanador herido.
- Potenciación de la dimensión artística y humana del profesional.
Propuestas de mejora de los pacientes.
- Canal de contenidos y formación en promoción de la salud y educación para la salud.
- Creación de canales telefónicos y de videoconferencia para orientación, promoción de la salud, educación para la salud y solución de problemas menores de salud.
- Asistentes personales en salud al servicio de los pacientes.
- Reforzar aprendizaje de utilización del sistema sanitario, consultas, pruebas diagnósticas y tratamientos
- Mejora de los autocuidados y responsabilización de hábitos de vida.
Conclusión:
- Para que el sistema sanitario se centre en la persona necesita la longitudinalidad de la Atención Primaria. Crear relaciones de confianza entre pacientes y profesionales que se mantengan en el tiempo.
- Para cambiar del paradigma de la medicina centrada en la enfermedad al de la medicina centrada en la persona es fundamental incorporar una visión bio-psico-social-existencial.
- Centrarse en la persona requiere un mínimo de tiempo. Sin tiempo para escuchar no es posible crear relaciones profesional sanitario-paciente de calidad.
- La excelencia técnica sin componente humano es un logro parcial. Introducir indicadores de calidad cualitativos es imprescindible para alcanzar la excelencia.
- No es posible una atención centrada en la persona si los profesionales no pueden desarrollar su labor con dignidad dentro un sistema que les sobrecarga o dificulta su desempeño profesional pleno.
- Cada profesional sanitario tiene la obligación de cuidarse para poder cuidar. Atenderse para poder atender. Nadie puede dar a otro lo que no tiene.
- El despotismo ilustrado sanitario y el buenismo sanitario han hecho que el ciudadano se desempodere. Es fundamental que cada cual recupere y mejore su capacidad de autocuidado.
- Para poder centrarnos en la persona es necesario abrirse a su familia y su comunidad. Una sanidad que dé la espalda a estas realidades no será capaz de entender al paciente. Si los profesionales no salen de sus consultas a las familias y comunidades caminaremos a ciegas.

¿Es posible una medicina basada en la persona? Is Person-focused Medicine Possible? 以人為本的醫學可能嗎?
Llevo años reflexionando en cómo mejorar la atención sanitaria que presto a mis pacientes. No es fácil dentro de una sanidad de diseño industrial donde tanto los que consultan como los consultores somos números dentro de cadenas de proceso y de gasto.
Desde las instituciones se publicita que la medicina debe estar centrada en el paciente pero:
- ¿Es eso posible si el médico de familia ha de atender en una jornada laboral a más de 50 personas?,
- ¿Es posible si la enfermera, fisioterapeuta o doctora está estresada, profesionalemente quemada, o agotada tras una guardia?
- ¿Es posible si la ciudadanía acude al sistema sanitario por cualquier síntoma o sensación corporal desagradable?
La medicina actual se basa en la enfermedad. Fundamentalmente en su aspecto más biologicista y corporal.
Además de basarse en criterios económicos.
Para conseguir basarla en ciencia (MBE, medicina basada en la evidencia) y en arte (medicina basada en la persona), habrá que seguir dando pasos hacia una visión más amplia que la actual.
De todo esto reflexionaremos el martes 7 de mayo en el Aula Magna del hospital Gregorio Marañón. No te lo puedes perder.

Is Person-focused Medicine Possible?
I have been thinking for years about how to improve the health care I provide to my patients. It is not easy in an industrial designed healthcare system where both the patients and the consultants are numbers within process and expense chains.
The institutions advertise that medicine must be patient-centred but:
- Is this possible if the family doctor has to treat more than 50 people in one working day?
- Is it possible if the nurse, physiotherapist or doctor is stressed, professionally burned, or exhausted on duty?
- Is it possible if citizens come to the health system for any symptoms or unpleasant body sensation?
Today's medicine is based on illness. Fundamentally in its most biological and corporal aspect.
In addition to being based on economic criteria.
In order to be able to base it on science (MBE, evidence-based medicine) and art (person-focused medicine), it will be necessary to continue taking steps towards a broader vision than the current one.

Una Atención Primaria desenfocada. An out-of-focus Primary Health Care. 一個失焦的初級衛生保健。

De
tarde en tarde me gusta releer los primeros capítulos del Martín Zurro.
Repasar las excelentes revisiones de Borrell, Gené, Casajunana, de la
Revilla y tantos otros. Intento buscar inspiración y ánimo, orientación y
guía cuando la confusión o la complejidad que enfrento en consulta me
sobrepasan. Hay días o semanas que navego los mares de la incertidumbre,
que diría Juan Gérvas, con suficiente dignidad pero en otros naufrago
cuando la marejada de mi ignorancia, las carencias del sistema o la
presión asistencial de un número inasumible de pacientes complejos me
desbordan.
En esos momentos de mareo con
cortejo vegetativo me miro a mi y a nuestra Atención Primaria y nos veo
desenfocados. Seguimos ahí, nos mantenemos funcionando, pero fuera de
foco. Oigo las quejas de los pacientes, de los gestores, de los
compañeros de trabajo... todos se dan cuenta de que estamos borrosos.
Los sabios lanzan planes y libros blancos, el ministerio convoca
reuniones, las sociedades científicas no dejan de programar congresos
pero nadie consigue enfocar debidamente la situación.
Mientras
tanto todos expresamos malestar disparándonos entre nosotros:
directivos, diferentes profesionales, colegios, sindicatos y
asociaciones, pero nadie se mira a si mismo con la suficiente
consciencia, voluntad y valor como para enfocarse. Es más fácil esperar
que los demás se adapten a nuestra distorsión que entendemos es culpa de
factores externos fuera de nuestro ámbito competencial.
Es
muy incómodo vivir desenfocado, muchos terminan como Woody Allen en
"Desmontando a Harry" atiborrándose a pastillas, yendo al psicoterapeuta
o hundidos en la neurosis. Otros se queman lentamente en pilas
humeantes que anuncian por todo lugar la muerte lenta de una Atención
Primaria que nadie sabe cómo reanimar.
La
pregunta sanadora sería ¿cómo reenfocar la Atención Primaria? Y la
respuesta no la encontraremos en el Martín Zurro. Entre otras cosas
porque los teóricos de la especialidad no preveyeron la posibilidad de
un desenfoque generalizado como el que vivimos, en el que una gran
mayoría de los equipos han devenido en grupos de supervivencia, el
número de centros de salud sobrecargados es altísimo y la proporción de
médicos quemados es abrumadora.
El sistema no
contempla auditorias de control de la calidad del trabajo en equipo, ni medidas correctoras de la presión asistencial inasumible, ni
aporta medidas de gestión de la motivación del personal, entre otros
factores. Competencias que tal vez deberían ejercer los numerosos
cuadros directivos bloqueados en una eterna rumiación de bases de datos e
indicadores que impiden que puedan salir a ayudar, orientar y motivar a
sus fatigados y desenfocados compañeros asistenciales.
No
es mi intención señalar una única responsabilidad en este desenfoque,
de hecho mi tesis es la misma que la del fragmento cinematográfico que
acompaña estas letras: el mundo no se va a adaptar a nuestra distorsión,
nos toca a nosotros aclararnos.
Para hacerlo
habrá que releer a los clásicos pero también pedir ayuda. Mirar con
atención aquellos centros de salud que han conseguido trabajar en
equipo, pedir a los gestores que intervengan en aquellos sobrepasados e
iniciar procesos de reflexión y toma de consciencia generalizados que
puedan servir para reiniciar aquellos equipos que como tantos
ordenadores bloqueados se hayan quedado "colgados".
Ante
la tormenta de falta de recursos, aumento etario y de complejidad
bio-psico-social es fundamental que la marinería despierte y se coordine
para evitar un seguro hundimiento si cada cual se sigue retirando a un
rincón a llorar en el momento más delicado de la navegación con tifones
de incertidumbre social, rodeados de icebergs privatizadores y con el
velamen recortado permanentemente.

From time to time I like to reread the first chapters of the Martín Zurro Family Medicine Book. Review the excellent reviews of Borrell, Gené, Casajunana, de la Revilla and many others. I try to seek inspiration and encouragement, guidance and light when the confusion or complexity I face in my clinic overwhelms me. In the meantime we all express discomfort by shooting each other: managers, different professionals, professional colleges, unions and associations, but no one looks at himself with enough conscience, will and courage to focus. It is easier to expect others to adapt to our distortion that we understand is the fault of external factors outside our sphere of competence. It is very uncomfortable to live out of focus, many end up like Woody Allen in "Deconstructing Harry" stuffing himself with pills, going to the psychotherapist or sunk in neurosis. Others burn slowly in smoking piles that announce everywhere the slow death of a Primary Care that nobody knows how to reanimate. The healing question would be how to refocus Primary Care? And we will not find the answer in the Martín Zurro. Among other things because the theorists of the specialty did not foresee the possibility of a generalized blur like the one we live in, in which a great majority of the teams have become survival groups, the number of overloaded health centers is very high and the proportion of burned doctors is overwhelming. The system does not contemplate audits to control the quality of teamwork, nor corrective measures of the unbearable welfare pressure, nor does it provide measures to manage staff motivation, among other factors. Competences that perhaps should be exercised by the numerous managerial cadres blocked in an eternal rumination of databases and indicators that prevent them from going out to help, guide and motivate their fatigued and unfocused fellow caregivers. It is not my intention to point out a single responsibility in this blur, in fact my thesis is the same as that of the cinematographic fragment that accompanies these letters: the world is not going to adapt to our distortion, it is up to us to clarify ourselves. In order to do so, we will have to reread the classics but also ask for help. Cuando nos sentimos mal hacemos lo posible por zafarnos del problema. Da igual que sea por una desavenencia de pareja, con el jefe, con padres o hijos, con alguna amistad, con un cliente, por una pérdida, por una enfermedad, por un futuro incierto... Las reacciones básicas suelen ser dos:
1. Huida.
2. Ataque.
La más común es la primera para la que usamos toda nuestra capacidad de distracción, despiste, negación... Hacemos grandes esfuerzos para quitar de encima de la mesa el problema en cuestión y meterlo en un cajón o en algún armario fuera de la vista para que no moleste. Los marrones pequeños caben, los grandes no. Como el mercado nos conoce bien ofrece una infinita lista de posibilidades para mantenernos entretenidos: series, películas, programas de televisión, youtube, redes sociales, páginas de Internet, música... Además de todos los productos y servicios que son susceptibles de mercantilizarse. El universo misteriosamente confabula para que compremos algo cuando estamos mal.
Las adicciones también tienen mucho que decir aquí. Cuando uno está chungo es fácil caer por la pendiente resbaladiza del alcohol, las drogas (incluidos medicamentos), el juego, las conductas de riesgo, la sexualidad compulsiva... Hecho que también reporta pingues beneficios a corporaciones y personas sin muchos escrúpulos.
Cada vez es más frecuente acudir al médico en estos casos que, como estará sobrecargado de trabajo hasta las trancas, nos despachará con un diagnóstico de ansiedad/depresión, una cajita de pastillas y quizá una derivación a los servicios de salud mental donde, si terminamos yendo, nos darán más pastillas.
Es verdad que cualquier chungo termina disolviéndose con el tiempo. Pero ¿quién es el majo que sabe esperar en estos tiempos de prisa permanente? Nos tomamos las pastillas y lo que nos den. Lo malo es que no hay medicamento que consiga frenar la cabeza, y la nuestra sigue en modo lavadora centrifugando a toda pastilla el problema de marras. Agotador.
Terminamos yendo al psicólogo o al terapeuta alternativo. El primero nos dará una lista de consejos, el segundo nos hará alguna terapia y nos venderá una nueva caja de pastillas (naturales) o algo similar. Tampoco con eso se nos va el problema.
La mala noticia: los problemas no se van. Pasa como con la sombra, uno siempre la tiene, a veces se ve más otras menos pero suele quedarse pegada a uno de forma vitalicia tirada por el suelo como aquella serpiente que hace bastante tiempo expulsaron de un remoto jardín.
A mis pacientes les suelo recordar que hay otra estrategia vital más sofisticada que la huida y el ataque: la toma de consciencia. Uno siempre tiene la posibilidad de contemplar cualquier contenido de la consciencia, por muy feo o terrible que parezca, sin sufrir daño alguno. Podemos mirar nuestro problema de frente aunque sea precisamente lo que evitamos con todas nuestras fuerzas. La buena noticia: atrevernos a mirar un problema no lo vaporiza pero nos permite ponerlo en su sitio y asignarle el peso que le corresponde.
Cuando el Buda tras una vida intensa de búsqueda y huida se sienta al fin bajo el árbol del Bhodi y se atreve a contemplar todos sus demonios y tentaciones sin reaccionar a ellas ni escapar termina iluminándose que es como decir: entendiendo que no hay absolutamente nada que no sea susceptible de ser sostenido con la atención. Lo mismo le pasó al Cristo cuando se metió en su desierto 40 días y se enfrento a los mismos demonios y tentaciones. Salió de allí con una nueva comprensión. Pongan ustedes el profeta, místico o héroe preferido y comprobaran que la historia se repite.
En ambos casos los problemas y sombras no desaparecieron. Pero dejaron de fastidiar. Ya no era necesario seguir huyendo o atacando(se) y por ende a los demás.
Hasta que consigamos iluminarnos dejo diez pistas que creo pueden venir bien a una gran mayoría:
Los decálogos y las listas son una manera como otra cualquiera de perder el tiempo. Sirven para muy poco. En cualquier caso aquí les dejo esta que está redactada con buena intención y bastante ingenuidad. Me paso el día rodeado de gente que está fatal, tengo la consulta atestada y no me da la vida para contarlo todo en los seis minutos que tengo para atender cada paciente. Espero por lo tanto que esto pueda servir de ayuda. Si es así compártelo, que seguro tienes también un montón de gente chunga alrededor.
Decalogue for hard times
When we feel bad we do our best to get out of the problem. It doesn't matter if it is due to a disagreement between the couple, with the boss, with parents or children, with some friendship, with a client, for a loss, for an illness, for an uncertain future... There are usually two basic reactions:
1. Flight.
2. Fight.
The most common is the first for which we use all our capacity for distraction, absent-mindedness, denial... We make great efforts to remove the problem from the table and put it in a drawer or some closet out of sight so that it does not disturb. Small problems fit, big ones don't. As the market knows us well, it offers an infinite list of possibilities to keep us entertained: series, movies, television programs, youtube, social networks, Internet pages, music... In addition to all the products and services that are likely to be marketed. The universe mysteriously conspires to sell us something when we are wrong.
Addictions also have a lot to say here. When you're cranky it's easy to fall down the slippery slope of alcohol, drugs (including medications), gambling, risky behaviors, compulsive sexuality... It also brings profits to corporations and unscrupulous people.
It is more and more frequent to go to the doctor in these cases that will dispatch us with a diagnosis of anxiety/depression, a box of pills and perhaps a referral to mental health services where, if we end up going, they will give us more pills.
It's true that any issue ends up dissolving over time. But who is the nice one who knows how to wait in these times of permanent haste? We take the pills and whatever they give us. The bad thing is that there is no medicine that can slow down the mind, and ours is still in washing machine mode centrifuging the whole trouble. Exhausting.
We end up going to the psychologist or the alternative therapist. The first will give us a list of advice, the second will do some therapy and sell us a new box of pills (eco) or something similar. We won't rid off the problem either.
The bad news: problems don't go away. It happens as with the shadow, you always have it, sometimes you see more others less but it usually stays stuck to one for life thrown on the ground like that snake that a long time ago expelled from a remote garden.
I often remind my patients that there is another more sophisticated life strategy than flight or fight: awareness. One always has the possibility of contemplating any content of consciousness, no matter how ugly or terrible it may seem, without suffering any harm. We can look our problem head-on even if it is precisely what we avoid with all our might. The good news: daring to look at a problem does not vaporize it, but it allows us to put it in its place and give it the weight it deserves.
When the Buddha, after an intense life of search and flight, finally sits under the Bhodi tree and dares to contemplate all his demons and temptations without reacting to them or escaping, he ends up illuminating himself as if to say: understanding that there is absolutely nothing that cannot be sustained with attention. The same thing happened to Christ when he went into his desert 40 days and faced the same demons and temptations. He left there with a new understanding. Put the prophet, mystic or favourite hero and you will see that history repeats itself.
In both cases the problems and shadows did not disappear. But they stopped teasing. It was no longer necessary to keep on running or attacking oneself and therefore the others.
Until we get enlightened I leave ten clues that I think can come in handy to a large majority:

1. EXERCISE
The key word is movement. Just walk half an hour a day in the open air, try it and you will see that it has almost magical effects. If you manage to persevere you may end up joining the select club of walkers who are capable of observing life and its ups and downs from the perspective of the one who dares to get up from the chair.
2. NATURE
A wild animal is happy if it is free and unhappy if it is put in a cage. How many hours a day do you spend in yours? Probably many. Even if your cage is made of gold, comfortable, pleasant, heated... it is still a cage. Get free for a while and if you have a park, mountain or field nearby try to breathe some fresh air and take a walk.
3. PETS.
Caressing an animal is a source of good vibrations. That is well known to those who have them. If this is not the case, ask a friend to let you pet his, or someone to take his dog for a walk in the street. I'm sure they'll let you and you'll see how good you feel.
4. HELPING OTHERS
When one is feeling bad it is inevitable to stare at one's own navel. And of course, we get worse. Helping others allows us a little bit of our circular problem and to put it into perspective. The others are usually much worse off. If you don't have anyone at hand, you can go for a walk around the neighbourhood and talk for a while with some homeless person or someone who asks at the exit of a supermarket. Or take a walk around an old people's home.
5. HOBBIES
Hobbies are those things we like to do. They have the ability to bring out our good side and make us have a good time and have fun. They also allow us to channel creativity and this is one of the most potent medicines there is to have good vibes.
6. FULL ATTENTION
Evil leads us to the past (guilt) and to the future (uncertainty and fear) by stealing the present. But serenity can only be found in the here and now which is the only thing we can manipulate. Becoming fully aware of what we are doing in each moment is a way of putting our feet on the ground. To feel the body, the breath in this instant. Notice how the head fills up and empties of feelings, ideas and contents. Taste what I put in my mouth or what my skin notices at this moment. It is basic to have patience with ourselves and to return to the present every time I realize that I have returned to ruminant thought.
Techniques such as yoga, mindfulness, meditation and many others can perhaps help. Take a look at the Internet, or better yet, try something right now.
7. ADD MORE GOOD MINUTES TO THE DAY.
No matter how bad it is, there is always some good moment in every day. To feel the pleasure of the water of the shower, to notice the clean clothes when we dress, to enjoy the flavor of the food... But it is not easy to find them when our head has a marked tendency to get into the manure. May be some poetry or music can help.
8. RELAXATION
The problems usually associate muscular tension to add to the cognitive and emotional. Contractures hurt. A little homemade physiotherapy wisely applied to the battered anatomy with warmth and gentle massage may come in handy. Nose and ask someone, if it is not possible use your hands. In parallel we can add some simple technique of relaxation or conscious breathing that we can copy from some video or audio taken from the Internet.
9. SUPPORT FROM FAMILY AND FRIENDS
Asking for help from those who love us or simply letting off steam for a while by talking, going for a walk or spending time together is priceless. It is not easy to have the courage to show our wounds and worries. Nor do we know how to measure how far to show so as not to overburden our own with an endless stream of permanent complaint that in the end is toxic to everyone and will make them flee.
10. PHILOSOPHY
Taking things philosophically means not giving oneself so much importance. When we wander lost in the shady forest there is nothing better than looking for a height to try to see things with perspective, to look for some reference in the landscape and to recover the orientation. Coming out of the "poor me" and the catastrophic and self-destructive attitudes in order to humbly recognize that life always has some meaning, even though sometimes it is difficult for us to find it. Others may be served by a moral code or a religious belief, all of which may make sense of the difficult question of why bad things happen to good people.
Decalogues and lists are just another way of wasting time. They are of little use. In any case here I leave you this one that is written with good intention and quite naïve. I spend the day surrounded by people who feels terrible, my practice is overcrowded and I can´t tell everything I want in the six minutes I have to attend each patient. I therefore hope that this can help. If that's the case, I'm sure you also have a lot of needy people around you to share this clues.
艱難時期的十誡

1, EJERCICIO
La palabra clave es movimiento. Basta con caminar media hora al día al aire libre, pruébenlo y verán que tiene efectos casi mágicos. Si consiguen perseverar tal vez terminen apuntándose a ese selecto club de paseantes que son capaces de observar la vida y sus visicitudes desde la perspectiva del que se atreve a levantarse de la silla.
2. NATURALEZA
Un animal salvaje es feliz si está libre y desdichado si le meten en una jaula. ¿Cuántas horas pasas al día en la tuya? Seguramente muchas. Aunque tu jaula sea de oro, cómoda, agradable, climatizada... no deja de ser una jaula. Libérate un rato y si tienes un parque, montaña o campo cerca prueba a respirar un poco de aire fresco y darte una vuelta.
3. MASCOTAS.
Acariciar un animal es una fuente de buen rollo. Eso lo saben bien los que los tienen. Si no es el caso pide a algún amigo que te dejen acariciar el suyo, o a alguna persona que saque de paseo a su perro por la calle. Seguro que te dejan y verás que bien te sientes.
4. AYUDAR A OTROS
Cuando uno está chungo es inevitable quedarse mirando el propio ombligo. Y claro, nos ponemos peor. Ayudar a otros nos permite descentrarnos un poquito de nuestro problema circular y de paso ponerlo en perspectiva. Los demás habitualmente están mucho peor. Si no tienen nadie a mano basta con darse una vuelta por el barrio y hablar un rato con alguna persona sin techo o que pida a la salida de un super. O darse una vuelta por una residencia de ancianos.
5. AFICIONES
Las aficiones son aquellas cosas que nos mola hacer. Tienen la facultad de sacar nuestro lado bueno y hacernos pasarlo bien y divertirnos. También nos permiten canalizar la creatividad y esta es una de las medicinas más potentes que existen para tener buen rollo.
6. ATENCIÓN PLENA
El mal rollo nos lleva al pasado (culpa) y al futuro (incertidumbre y miedo) robándonos el presente. Pero solo se puede encontrar serenidad en el aquí y el ahora que es lo único que podemos manipular. Tomar plena consciencia de lo que hacemos en cada momento es una manera de poner los pies en el suelo. Sentir el cuerpo, la respiración en este instante. Notar como la cabeza se va llenando y vaciando de sentimientos, ideas y contenidos. Saborear lo que meto en la boca o lo que mi piel nota en este instante. Es básico tener paciencia con nosotros mismos y volver al presente cada vez que me doy cuenta de que he vuelto al pensamiento rumiante.
Técnicas como el yoga, el mindfulness, la meditación y otras muchas pueden tal vez ayudar. Echa un vistazo a Internet, o mejor aún, prueba algo ahora mismo.
7. AÑADIR MÁS MINUTOS BUENOS AL DÍA.
Por muy mala que esté la cosa siempre hay algún instante bueno en cada día. Sentir el placer del agua de la ducha, notar la ropa limpia al vestirnos, disfrutar del sabor de la comida... Pero no es nada sencillo encontrarlos cuando nuestra cabeza tiene una marcada tendencia a meterse en el estiércol. Quizá la poesía y la música puedan servirte de ayuda.
8. RELAJACIÓN
Los problemas suelen asociar tensión muscular a sumar a la cognitiva y emocional. Las contracturas duelen. Un poco de fisioterapia casera aplicada sabiamente a la maltrecha anatomía con calorcito y masaje suave puede venir bien. Échale morro y pídeselo a alguien, si no es posible usa tus manos. En paralelo podemos añadir alguna técnica sencilla de relajación o respiración consciente que podemos copiar de algún vídeo o audio sacado de Internet.
9. APOYO FAMILIAR Y DE AMIGOS
Pedir ayuda a los que nos quieren o simplemente desahogarnos un rato hablando, dando una vuelta o pasando tiempo juntos no tiene precio. No es fácil tener el valor para enseñar nuestras heridas y preocupaciones. Tampoco saber medir hasta dónde mostrar para no sobrecargar a los nuestros con un chorreo inacabable de queja permanente que al final resulta tóxico para todos y que los hará huir.
10. FILOSOFÍA
Tomarse las cosas con filosofía significa no darse tanta importancia. Cuando vagamos perdidos en el bosque umbrío no hay nada mejor que buscar una altura para tratar de ver las cosas con perspectiva, buscar alguna referencia en el paisaje y recuperar la orientación. Salir del "pobre de mi" y de las actitudes catastrofistas y autodestructivas para con humildad reconocer que la vida siempre tiene algún sentido aunque en ocasiones nos resulte complicado encontrarlo. A otros quizá les sirva un código moral o una creencia religiosa, todo puede valer para encontrar sentido a la difícil pregunta de por qué a la gente buena les pasan cosas malas.
8. RELAJACIÓN
Los problemas suelen asociar tensión muscular a sumar a la cognitiva y emocional. Las contracturas duelen. Cuando vagamos perdidos en el bosque umbrío no hay nada mejor que buscar una altura para tratar de ver las cosas con perspectiva, buscar alguna referencia en el paisaje y recuperar la orientación. Salir del "pobre de mi" y de las actitudes catastrofistas y autodestructivas para con humildad reconocer que la vida siempre tiene algún sentido aunque en ocasiones nos resulte complicado encontrarlo. A otros quizá les sirva un código moral o una creencia religiosa, todo puede valer para encontrar sentido a la difícil pregunta de por qué a la gente buena les pasan cosas malas.
Los decálogos y las listas son una manera como otra cualquiera de perder el tiempo. Sirven para muy poco. En cualquier caso aquí les dejo esta que está redactada con buena intención y bastante ingenuidad. Me paso el día rodeado de gente que está fatal, tengo la consulta atestada y no me da la vida para contarlo todo en los seis minutos que tengo para atender cada paciente. Espero por lo tanto que esto pueda servir de ayuda. Si es así compártelo, que seguro tienes también un montón de gente chunga alrededor.
Decalogue for hard times
When we feel bad we do our best to get out of the problem. It doesn't matter if it is due to a disagreement between the couple, with the boss, with parents or children, with some friendship, with a client, for a loss, for an illness, for an uncertain future... There are usually two basic reactions:
1. Flight.
2. Fight.
The most common is the first for which we use all our capacity for distraction, absent-mindedness, denial... We make great efforts to remove the problem from the table and put it in a drawer or some closet out of sight so that it does not disturb. Small problems fit, big ones don't. As the market knows us well, it offers an infinite list of possibilities to keep us entertained: series, movies, television programs, youtube, social networks, Internet pages, music... In addition to all the products and services that are likely to be marketed. The universe mysteriously conspires to sell us something when we are wrong.
Addictions also have a lot to say here. When you're cranky it's easy to fall down the slippery slope of alcohol, drugs (including medications), gambling, risky behaviors, compulsive sexuality... It also brings profits to corporations and unscrupulous people.
It is more and more frequent to go to the doctor in these cases that will dispatch us with a diagnosis of anxiety/depression, a box of pills and perhaps a referral to mental health services where, if we end up going, they will give us more pills.
It's true that any issue ends up dissolving over time. But who is the nice one who knows how to wait in these times of permanent haste? We take the pills and whatever they give us. The bad thing is that there is no medicine that can slow down the mind, and ours is still in washing machine mode centrifuging the whole trouble. Exhausting.
We end up going to the psychologist or the alternative therapist. The first will give us a list of advice, the second will do some therapy and sell us a new box of pills (eco) or something similar. We won't rid off the problem either.
The bad news: problems don't go away. It happens as with the shadow, you always have it, sometimes you see more others less but it usually stays stuck to one for life thrown on the ground like that snake that a long time ago expelled from a remote garden.
I often remind my patients that there is another more sophisticated life strategy than flight or fight: awareness. One always has the possibility of contemplating any content of consciousness, no matter how ugly or terrible it may seem, without suffering any harm. We can look our problem head-on even if it is precisely what we avoid with all our might. The good news: daring to look at a problem does not vaporize it, but it allows us to put it in its place and give it the weight it deserves.
When the Buddha, after an intense life of search and flight, finally sits under the Bhodi tree and dares to contemplate all his demons and temptations without reacting to them or escaping, he ends up illuminating himself as if to say: understanding that there is absolutely nothing that cannot be sustained with attention. The same thing happened to Christ when he went into his desert 40 days and faced the same demons and temptations. He left there with a new understanding. Put the prophet, mystic or favourite hero and you will see that history repeats itself.
In both cases the problems and shadows did not disappear. But they stopped teasing. It was no longer necessary to keep on running or attacking oneself and therefore the others.
Until we get enlightened I leave ten clues that I think can come in handy to a large majority:

1. EXERCISE
The key word is movement. Just walk half an hour a day in the open air, try it and you will see that it has almost magical effects. If you manage to persevere you may end up joining the select club of walkers who are capable of observing life and its ups and downs from the perspective of the one who dares to get up from the chair.
2. NATURE
A wild animal is happy if it is free and unhappy if it is put in a cage. How many hours a day do you spend in yours? Probably many. Even if your cage is made of gold, comfortable, pleasant, heated... it is still a cage. Get free for a while and if you have a park, mountain or field nearby try to breathe some fresh air and take a walk.
3. PETS.
Caressing an animal is a source of good vibrations. That is well known to those who have them. If this is not the case, ask a friend to let you pet his, or someone to take his dog for a walk in the street. I'm sure they'll let you and you'll see how good you feel.
4. HELPING OTHERS
When one is feeling bad it is inevitable to stare at one's own navel. And of course, we get worse. Helping others allows us a little bit of our circular problem and to put it into perspective. The others are usually much worse off. If you don't have anyone at hand, you can go for a walk around the neighbourhood and talk for a while with some homeless person or someone who asks at the exit of a supermarket. Or take a walk around an old people's home.
5. HOBBIES
Hobbies are those things we like to do. They have the ability to bring out our good side and make us have a good time and have fun. They also allow us to channel creativity and this is one of the most potent medicines there is to have good vibes.
6. FULL ATTENTION
Evil leads us to the past (guilt) and to the future (uncertainty and fear) by stealing the present. But serenity can only be found in the here and now which is the only thing we can manipulate. Becoming fully aware of what we are doing in each moment is a way of putting our feet on the ground. To feel the body, the breath in this instant. Notice how the head fills up and empties of feelings, ideas and contents. Taste what I put in my mouth or what my skin notices at this moment. It is basic to have patience with ourselves and to return to the present every time I realize that I have returned to ruminant thought.
Techniques such as yoga, mindfulness, meditation and many others can perhaps help. Take a look at the Internet, or better yet, try something right now.
7. ADD MORE GOOD MINUTES TO THE DAY.
No matter how bad it is, there is always some good moment in every day. To feel the pleasure of the water of the shower, to notice the clean clothes when we dress, to enjoy the flavor of the food... But it is not easy to find them when our head has a marked tendency to get into the manure. May be some poetry or music can help.
8. RELAXATION
The problems usually associate muscular tension to add to the cognitive and emotional. Contractures hurt. A little homemade physiotherapy wisely applied to the battered anatomy with warmth and gentle massage may come in handy. Nose and ask someone, if it is not possible use your hands. In parallel we can add some simple technique of relaxation or conscious breathing that we can copy from some video or audio taken from the Internet.
9. SUPPORT FROM FAMILY AND FRIENDS
Asking for help from those who love us or simply letting off steam for a while by talking, going for a walk or spending time together is priceless. It is not easy to have the courage to show our wounds and worries. Nor do we know how to measure how far to show so as not to overburden our own with an endless stream of permanent complaint that in the end is toxic to everyone and will make them flee.
10. PHILOSOPHY
Taking things philosophically means not giving oneself so much importance. When we wander lost in the shady forest there is nothing better than looking for a height to try to see things with perspective, to look for some reference in the landscape and to recover the orientation. Coming out of the "poor me" and the catastrophic and self-destructive attitudes in order to humbly recognize that life always has some meaning, even though sometimes it is difficult for us to find it. Others may be served by a moral code or a religious belief, all of which may make sense of the difficult question of why bad things happen to good people.
Decalogues and lists are just another way of wasting time. They are of little use. In any case here I leave you this one that is written with good intention and quite naïve. I spend the day surrounded by people who feels terrible, my practice is overcrowded and I can´t tell everything I want in the six minutes I have to attend each patient. I therefore hope that this can help. If that's the case, I'm sure you also have a lot of needy people around you to share this clues.
