El canto de la Atención Primaria. The Song of Primary Health Care. 初級衛生保健之歌。



Saben mis manos
Que pese a la sequía
La tierra espera.




No son buenos tiempos para las instituciones públicas. La marejada de la globalización y la revolución tecnológica están tambaleando hasta la médula las viejas estructuras de este viejo barco que llamamos Europa. En los sistemas sanitarios pasa lo mismo. Por eso a mi alrededor oigo un permanente coro de quejas y lamentos. En parte son necesarios por cuanto desahogan y ayudan a no terminar ahogado en la marea. Pero por otra parte son inútiles por que no conozco ninguna conversación de café que termine teniendo impacto alguno.

Hace falta dar pasos y hay gente que los está dando.

Rafa Bravo cita hoy en su blog a:  Pilar, Nuria y Sonia (Medicas de familia Burgos), @Bastaya Andalucia, Médicos jefes de servicio de Vigo, Carlos Bautista y @BastayaMalaga, Médicos y AP de La Rioja, Huelga Cat, Lehen Arreta Arnasbirretzen (Aires Nuevos para la Atención Primaria). Amplio la lista con los profesionales de valor: administrativos, fisios, pediatras, enfermeras, gestores y médicos de familia que voy incluyendo en los mapas de Activos en Atención Primaria.

También me gustaría traer aquí a tantos profesionales de Atención Primaria que lo están haciendo bien. Que navegan con dignidad con condiciones climatológicas muy adversas. Me gustaría darles (darnos) ánimo y recordar que nuestra función es hermosa y necesaria. Que nos toca defender a nuestros pacientes, que es más necesario que nunca mantenernos cerca unos de otros para generar ideas y ponerlas en marcha. No me cabe duda de que nos tocará hacerlo fuera de las organizaciones clásicas que lamentablemente no han dado los suficientes pasos. Tampoco de que nos tocará ver cambios y afrontar retos mucho más complejos de los que tenemos ahora.

Hay que seguir navegando. Yo hoy desde un pequeño pueblo tengo esperanza. Lo que haga o deje de hacer en mi pequeño consultorio local depende en gran medida de mi mismo, depende en gran medida de las conexiones que establezca tanto con mi comunidad local como con mis compañeros de trabajo a nivel global. Dependerá de querer seguir caminando manteniendo en lo posible la sonrisa.

These are not good times for public institutions.  The tide of globalisation and the technological revolution are shaking the old structures of this old ship we call Europe to the core. The same thing is happening in public health systems. That is why around me I hear a permanent chorus of complaints and lamentations. In part they are necessary because they let off steam and help not to end up drowned in the tide. But on the other hand they are useless because I don't know of any coffee conversation that ends up having any impact.



You need to take steps and there are people who are taking them.

I would also like to bring here so many Primary Care professionals who are doing well. They sail with dignity in very adverse weather conditions. I would like to give them (give us) encouragement and remember that our function is beautiful and necessary. That it is up to us to defend our patients, that it is more necessary than ever to stay close to each other to generate ideas and get them going. I have no doubt that we will have to do it outside of the classic organizations that unfortunately have not taken enough steps forward. Nor that we will have to see changes and face much more complex challenges than we have now.



We must continue to sail. Today, from a small village, I have hope. What I do or don't do in my little local clinic depends to a great extent on myself, and depends to a great extent on the connections I establish both with my local community and with my co-workers at a global level. It will depend on wanting to keep on walking while keeping a smile as much as possible.
