El Ministerio de Sanidad ha aprobado recientemente la posibilidad de que las enfermeras de Atención Primaria puedan recetar antibióticos para manejar las infecciones urinarias no complicadas en mujeres adultas. Automáticamente muchas enfermeras y médicos han protestado alegando diferentes motivos.
Pero más allá del ruido de fondo creo que es una oportunidad muy interesante para reflexionar. ¿Qué nos está diciendo realmente el Ministerio de Sanidad con esta medida?
1. Está apostando fuertemente por la enfermería de Atención Primaria. Confía en ella y tiene claro que el camino es empoderarlas y darles más competencias.
2. Asume que los médicos de familia ya no pueden más. La actual plantilla no tiene capacidad de asumir toda la carga asistencial, lo que está sobrecalentando el sistema sanitario por entero, primero las urgencias hospitalarias y luego todo lo demás.
3. Asumen que en un futuro cercano habrá consultorios y centros de salud sin médico. Lo saben con certeza. No va a haber recambio con una tasa de abandonos y jubilaciones mayor a la de nuevos residentes. Las enfermeras que los mantengan tendrán que tener mayor formación y competencias.
La resultante es que para mantener una Atención Primaria funcionante es fundamental reforzarla. Como no lo van a hacer con pediatras y médicos de familia tendrá que ser con enfermeras. Es lo que hay.
Increasing the value of nurses
The Ministry of Health has recently approved the possibility for primary care nurses to prescribe antibiotics to manage uncomplicated urinary tract infections in adult women. Automatically many nurses and doctors have protested on various grounds.
But beyond the background noise I think it is a very interesting opportunity for reflection. What is the Ministry of Health really telling us with this measure?
1. It is making a strong commitment to primary care nursing. It trusts them and is clear that the way forward is to empower them and give them more competences.
2. It assumes that family doctors can no longer cope. The current workforce does not have the capacity to take on the entire care load, which is overheating the entire health system, first the hospital emergencies and then everything else.
3. They assume that in the near future there will be clinics and health centres without doctors. They know this for sure. There is not going to be a replacement with a higher rate of departures and retirements than the number of new residents. The nurses who maintain them will have to have more training and skills.
The bottom line is that in order to maintain a functioning primary care system, it is essential to strengthen it. As they are not going to do it with paediatricians and family doctors, they will have to do it with nurses. That is the way it is.
The big problem is to get the different actors to agree on how to organise and regulate a very complex situation in which changes will inevitably have to be made and which will be met with stiff resistance from all sides. Will it be possible for someone to pick up the torch of leadership and motivation to bring the situation back on track?
