Reconstruyendo los pilares. Rebuilding the pillars. 重建支柱

 

Ayer asistí a la jornada anual que organiza la Sociedad Madrileña de Medicina Familiar y Comunitaria. Me he animado a compartir algunas impresiones por ser la ocasión proclive para tomar el pulso a la especialidad en particular y a la Atención Primaria en general. Lo mejor de este tipo de jornadas es el valor que aportan tanto dentro de las salas como en los pasillos. En las primeras mucho nivel, invitados ilustres que aportaron formación y participación de representantes de los grupos de trabajo que dieron jugosas pistas. Estos últimos en un formato breve de 6 minutos que nos permitió confirmar que pese a todo sigue habiendo profesionales motivados. Hubo un toque de humor a cargo de @1cada8horas, otro institucional (en los últimos años no vino nadie de la gerencia), y guiños variados en las mesas. Complementaban la jornada algunos talleres prácticos y el espacio científico de presentaciones y poster que reconozco disfruté.

No puede hablar con los 300 asistentes pero sí que abracé y saludé a muchos amigos tanto en activo como jubilados. Me agradó especialmente que estos últimos se animaran a acudir. Agradecí a José Manuel Freire su intermediación para que la jornada tuviera lugar en el Ateneo de Madrid, un lugar ilustre y magnético. Después de hablar con Alberto Cutillas y algunos miembros de la actual junta, con compañeros seniors y algún residente creo poder certificar un nivel de perfusión distal aceptable. No es poca cosa tras las asistolias de la pandemia y la postpandemias, tras las huelgas y desencuentros con la administración. Sentí también mucho cansancio de fondo y flotaba en el ambiente ese olor a desesperación que no nos terminamos de quitar de encima. Salí contento. Produce un alivio en el alma tomar consciencia de la tenacidad de tanta gente buena que pese al viento en contra sigue navegando.

Rebuilding the pillars

Yesterday I attended the annual conference organised by the Madrid Society of Family and Community Medicine. I have decided to share some of my impressions as it is the perfect opportunity to take the pulse of the speciality in particular and of Primary Care in general. The best thing about this type of conference is the value it provides both inside the rooms and in the corridors. In the first ones, there was a high level, illustrious guests who provided training and participation of representatives of the working groups who gave juicy clues. The latter in a short 6-minute format that allowed us to confirm that despite everything there are still motivated professionals. There was a touch of humour by @1cada8horas, another institutional one (in recent years no one from the management came), and a variety of winks at the tables. 

The day was complemented by some practical workshops and the scientific area of presentations and posters, which I must admit I enjoyed. I cannot speak to the 300 attendees but I did hug and greet many friends, both active and retired. I was especially pleased that the latter were encouraged to attend. I thanked José Manuel Freire for brokering the event to take place at the Ateneo de Madrid, an illustrious and magnetic venue. 

After speaking with Alberto Cutillas and some members of the current board, with senior colleagues and some residents, I believe I can certify an acceptable level of distal perfusion. This is no mean feat after the pandemic and post-pandemic asystole, after the strikes and disagreements with the administration. I also felt a lot of fatigue in the background, and there was a whiff of desperation in the air that we still can't shake off. I left happy. It is a relief for the soul to be aware of the tenacity of so many good people who, despite the headwinds, continue to sail.

昨天，我参加了马德里家庭与社区医学学会举办的年会。我决定与大家分享我的一些感想，因为这是一次把握本专业乃至整个初级医疗脉搏的绝佳机会。这类会议的最大特点是，无论是在会场内还是在走廊上，都能体现出它的价值。在前者，有高水平、杰出的嘉宾提供培训，也有工作组代表的参与，他们提供了多汁的线索。后者以短短 6 分钟的形式让我们确认，尽管发生了一切，但仍有积极进取的专业人士。@1cada8horas（另一位体制内人士，近年来管理层没有人来过）的幽默，以及餐桌上的各种眨眼。当天还举办了一些实用的研讨会，以及演讲和海报科学区，我必须承认，我很喜欢这些活动。

我不能对 300 名与会者说什么，但我确实拥抱和问候了许多朋友，有在职的，也有退休的。我尤其高兴的是，大会鼓励退休人员参加会议。我感谢何塞-曼努埃尔-弗莱雷（José Manuel Freire）促成这次活动在马德里雅典耀大学（Ateneo de Madrid）举行，这是一个辉煌而富有磁性的场所。

在与阿尔贝托-库蒂亚斯（Alberto Cutillas）和现任董事会的一些成员、资深同事和一些住院医生交谈之后，我相信我可以证明远端灌注达到了可接受的水平。在经历了大流行和大流行后的窒息，在经历了罢工和与行政部门的分歧之后，这绝非易事。我还感觉到了许多疲惫，空气中弥漫着一股我们仍然无法摆脱的绝望气息。我高兴地离开了。能够意识到有这么多善良的人们在逆风中依然坚韧不拔，继续扬帆起航，我的心灵得到了慰藉。
