Médicos y naranjas. Doctors and oranges. 医生和橙子



Para acercarse a alguien que está mal hace falta un mínimo de serenidad. Por instinto nos alejamos de aquellos que nos parecen tóxicos, pero también de los que tienen problemas, habitan márgenes, sufren enfermedades largas o están en fase de catástrofe vital. Cada vez toleramos menos los rollos ajenos empachados como estamos de los propios. No aguantamos la desgracia ajena, material, física y mental, y por supuesto no queremos saber nada de la muerte. 

Preferimos delegar. Y como sociedad lo hacemos en las "instituciones", en este caso sanitarias y sociosanitarias. Que para eso pago mis impuestos. El problema empieza cuando los sanitarios y sociosanitarios se hartan de ser estirados como chicles o exprimidos como naranjas. Cada vez es más difícil encontrar médicos y enfermeras, cada vez más difícil encontrarlos cuerdos. Porque hace falta un poquito de tregua, de tiempo, de calma, de espacio para poder ejercer la enfermería o la medicina en condiciones. 

A nadie le gusta que le metan en un exprimidor. Dicen que importando naranjas o creando más en la universidad se arregla el problema, no se lo crean.

To get close to someone who is ill requires a minimum of serenity. We instinctively distance ourselves from those who seem toxic to us, but also from those who have problems, live on the margins, suffer long illnesses or are in a phase of vital catastrophe. We are less and less tolerant of other people's problems, as we are so full of our own. Because we need a little respite, a little time, a little calm, a little space to be able to practise nursing or medicine properly. 

Nobody likes to be put through the wringer. They say that importing oranges or creating more at the university will solve the problem, but don't believe it.

Todo arde (en Atención Primaria). Everything burns (in Primary Care). 一切烧伤(在初级保健中)




Esta semana decidí hacerme un regalo y me leí el último libro de Juan Gómez Jurado que es igual de entretenido que una serie pero castiga menos el sueño por no tener luz azul. En la trama sigue habiendo heroínas intelijentísimas y malos malísimos pero la cosa engancha y se lee de un tirón. En mi día a día pasa un poco lo mismo, las heroínas en los centros de salud sufren canela pese a su inteligencia y los malvados gestores cada vez aprietan más. Bueno hasta que aguantan. La gerente de Atención Primaria de Madrid ha dejado el puesto en probable relación con el motín y el anuncio de huelga por la crisis de las urgencias rurales. La anterior gerente dimitió hace dos años y pico... En mi dirección asistencial no se cubren un tercio de los puestos de "directoras de centros", las antiguas subdirectoras...

El descontento de profesionales y usuarios es mayúsculo. Los primeros porque se ven desbordados, los segundos porque cada vez tienen que esperar más a una cita en su centro de salud y cada vez son menos resolutivas, dado que un médico de familia sin tiempo resuelve poco. Les resumo el plan estrella de los gestores de Atención Primaria de Madrid: "crear agendas de calidad y de rebosamiento para mejorar la accesibilidad". Traduzco: forzar a los médicos a ver más pacientes al día. Y claro "como no hay médicos" no hay baja o permiso que se cubra lo que aumenta cada vez más la presión sobre los que quedan, que cada vez son menos, especialmente en los centros más difíciles. 

A esto añadan la movida de las urgencias de Atención Primaria que ya les conté el otro día y aderecen con las jubilaciones que vienen, la ausencia de diálogo entre la dirección y los sindicatos y el humo que sale de los centros. Todo arde. Y más que va a arder.

¿Por qué peligran los servicios de urgencia de Madrid? Why are Madrid's emergency services in danger? 为什么马德里的紧急服务处于危险之中?

La situación de los servicios de urgencia rurales y de Atención Primaria de Madrid vive su peor crisis. La Consejería de Sanidad ha puesto encima de la mesa una propuesta que empeora mucho las condiciones laborales de sus profesionales, aumentado horas de trabajo, número de jornadas y turnos, suprimiendo libranzas y cambiando lugar y puesto de trabajo en muchos casos. Los sindicatos se han levantado de la mesa de negociación por haber sido ignorados y amenazan huelga, algo que médicos y enfermeras aborrecen por la dificultad de plantearla con los servicios mínimos abusivos que se impondrán. 


Venimos de una situación precaria, la Consejería cerró los centros de urgencias de Atención Primaria (SUAP) durante la pandemia destinando sus médicos al Hospital Zendal y a las unidades de Atención Domiciliaria provisionales. Quedaron abiertos únicamente los servicios de atención rural (SAR) que se vieron sobrecargados por asumir en parte el volumen de urgencias que no se atendían en los SUAP. Ahora quieren volver a refundirlo todo en un nuevo sistema en el que siguen faltando profesionales y con condiciones de trabajo peores. ¿Qué va a pasar?:


  1. Habrá huelga.
  2. Los profesionales se terminarán marchando del servicio.
  3. El servicio se precarizará todavía más o se cerrará en muchos casos.
  4. Probablemente se termine obligando a la ya sobrecargada plantilla de los centros de salud a asumir turnos extra.
  5. Se sobrecargarán las urgencias de hospital.
  6. La ciudadanía se quedará con menos servicios cerca de ella y tendrá que desplazarse más lejos y esperar más. 


Y las elecciones a seis meses vista, no duden en que la actual Consejería prometerá algún hospital nuevo (en la sierra norte, donde lo que faltan son mejores servicios de urgencia rural y de Atención Primaria) o nuevos centros de salud (sin aumento de efectivos). Lamentablemente de momento los ladrillos no saben atender al ciudadano. 


Durante la pandemia se debilitó la Atención Primaria de Madrid en lugar de reforzarla y desgraciadamente esa sigue siendo la política. ¿Se puede permitir Madrid tener una sanidad pública agonizante?

Why are Madrid's emergency services in danger?


The situation of Madrid's rural and primary care emergency services is going through its worst crisis ever. La esclavitud del algoritmo. Slavery to the algorithm. 算法的奴役性







Es arriesgado depender de algo que no controlas. Lo sabemos pero no hacemos caso y con frecuencia nos escurrimos por pendientes resbaladizas que nos meten en problemas. Hoy vamos todos por la vida con un móvil en el bolsillo. Cada vez delegamos más cosas en él. Empezamos guardando la agenda telefónica, luego las fotos, más tarde vídeos, las cuentas bancarias, las compras, la agenda social, las comunicaciones personales, los datos biométricos y de salud… 


La herramienta nos da poder a cambio de datos personales, esto va a cambiar. El hilo inicial se convirtió en soga y la soga mutará a cadena antes de lo que pensamos. Se nos va a exigir conducta, se nos va exigir sumisión. Donde más fácilmente se ve es al comparar los algoritmos de las diferentes redes sociales industriales. Vamos hacia un mundo donde la socialización precise de ayudas digitales, desde para formar un grupo de adolescentes, ligar, buscar pareja o encontrar alguien para hablar. Y cada vez será más fácil, aunque el verdadero precio del asunto quede velado y no se muestre a las claras. Ya les adelanto que no será barato, pagar con información personal nos hace vulnerables y entrega nuestras manos para que sean amarradas con cadenas. Si el algoritmo termina conociéndonos mejor que nuestras madres imaginen lo que podrá hacer con nosotros. No es distopía, es lo que hay. 






Slavery to the algorithm

We start by saving our phone book, then our photos, then videos, bank accounts, shopping, social calendar, personal communications, biometric and health data....

Salud mental en la tierra media. Mental Health in Middle-earth. 中土世界的心理健康

Tolkien creó con su imaginación una mitología y la situó en un lugar donde se enfrentaban diversas fuerzas e intereses. Pero si lo miramos bien tras el trasfondo épico se escondía la eterna lucha de contrarios que los seres humanos llevan experimentando desde el paleolítico. El bien contra el mal, la luz contra la oscuridad, la salud contra la enfermedad…


Hoy nuestro mundo es quizá más sofisticado pero nuestras necesidades, deseos y pasiones no distan mucho de los que viven los personajes de la tierra media. Nos enfrentamos a retos similares, a todos nos toca enfrentarnos con el mismo señor oscuro. Permítanme llamarlo Malestar y ponerle mayúscula. 


El malestar siempre ha estado vinculado a la vida corriente. Nuestros antepasados lo experimentaron en grado sumo en sus viajes y aventuras. Pasaron mucha hambre, padecieron muchas enfermedades, frío, ataques de animales salvajes o de otros congéneres, también guerra y sus desmanes. 


Si el malestar era pequeño, una rozadura en un pie sin zapato, un sabañón, una pequeña herida, una diarrea, no se hacía absolutamente nada. Si era más importante quizá se hiciera algo y si era muy intenso habitualmente te acababa matando y así terminaba el problema. No había muchas complicaciones, la muerte se asumía como consustancial a la vida y se integraba en esta. Se lloraban los muertos pero se tiraba para adelante, no se derrochaba en dramatismo, la vida era muy dura. 


Hoy las cosas han cambiado. Nuestra sociedad entroniza el luminoso bienestar y trata de esconder la oscura muerte, minimizar la enfermedad y expatriar el Malestar. Los locos y moribundos, se quitan del medio, los decesos son tratados con la máxima higiene y se alejan de niños y personas sensibles. 


Por otro lado la sociedad ha cambiado enormemente de un modelo industrial opresivo a otro postindustrial auto explotador, donde cada vez la presión por ser más eficiente se delega en cada cual, aumentando las horas de trabajo y la exigencia del mismo. La conciliación es imposible para muchos y mantener una vida privada y familiar de calidad cada vez es más difícil. Las relaciones familiares y personales sufren con regímenes laborales que explotan y agotan a individuos que llegan sin energía a sus hogares.


Una de cada cuatro personas que acude a un médico de familia lo hace por sentirse agobiado, nervioso, sin ánimo o sin fuerzas. La oscuridad de Mordor crece sin parar y el Malestar extiende su dominio con mano de hierro. 


Es verdad que los Elfos pueden capearlo mejor gracias a su elevado estrato social, sus mejores condiciones de vida y su desahogada situación económica y quizá los Enanos en sus minas o algunos en sus comarcas no noten mucho la opresión. Pero al final a todos los afecta el chapapote emocional que va a ir a más en cuanto el cambio climático u otras desgracias condicionen nuevas catástrofes que globalicen nuevas olas de pandemia, carestía o penalidades. 


Lo que parece que como sociedad hemos olvidado es tanto el verdadero significado del malestar como su trascendencia. Nos hemos empeñado en una cruzada inganable contra él por cuanto la sombra jamás desaparece del todo cuando hay luz y cuando no la hay reina en el mundo. ¿Para qué sirve la oscuridad? Me preguntaba mi hija de cinco años esta semana, ¿para qué sirve el malestar? Pregunto yo. Y las respuestas tienen que ver con su opuestos, dado que tanto luz y oscuridad como bienestar y malestar son inseparables. El malestar nos sirve para saber lo que es el bienestar y agradecerlo cuando lo disfrutamos. Nos permite detectar lo que no marcha bien y nos da la oportunidad de hacer algo al respecto. Nos avisa cuando nuestra vida se dirige a un precipicio o se ha acercado mucho a algo que quema. 


Cualquier vehículo viene de fábrica con indicadores visuales y sonoros que avisan si vamos mal de aceite, el motor se ha calentado, o fallan los frenos. Esas lucecitas rojas o pitidos agudos son insufribles pero tienen gran valor. Nos permiten actuar y evitar males mayores. A nadie se le ocurre golpearlos con un martillo cuando aparecen para que dejen de molestar. Hoy quizá estemos algo huérfanos de formas de manejar los malestares asociados al estilo de vida dado que ese mismo estilo de vida nos anima a ignorarlos y mirar a otro lado. “Ponga un poco más de distracción en su vida y siga adelante” parecen decirnos. Pero vemos que con malestares grandes no nos sirven de nada las series de televisión, los vídeos y tontunas de las redes sociales, ni la música que nuestras pantallas y auriculares no dejan de servirnos. 


Marta Carmona y Javier Padilla en su libro Malestamos nos comparten la tesis de que ante el malestar que amenaza nuestra salud mental es necesario ir más allá del enfoque individual (psicoterapia o sindicato) y tomar medidas políticas y sociales que incidan en las causas directas de los diversos malestares y en las causas de las causas que suelen ser sociales o estructurales. Coincido con ellos pero me parece muy difícil implementarlo en una coyuntura que sacraliza la individualidad, la “libertad” y el vaya usted a lo suyo y no se meta en líos. Probablemente la toma de conciencia de que en solitario somos muy vulnerables y unidos más resistentes pueda servir para que como ciudadanos de a pie prioricemos de nuevo un tejido familiar y de barrio que nos arrope y pueda arropar a otros, más allá de las frías redes sociales industriales que nos disgregan y atontan. No creo que esta línea de actuación se pueda implantar desde arriba, aunque es cierto que hay políticas que lo pueden favorecer como que hay otras que lo dificultan. 


Como médico de familia me enfrento cada día con personas hundidas por el Malestar. Algunas por sus dolores, enfermedades crónicas o problemáticas afectivas o familiares, otras por la sobrecarga del trabajo, el cuidado de terceros o la impotencia de llevar una vida personal digna. Ofrezco escucha, que con frecuencia es exigua, y una perspectiva que trata de facilitar que el paciente entienda lo que pasa y le permita hacer los ajustes que considere. En un porcentaje de casos ofrezco medicación o psicoterapia. En la mayoría que se desahoguen solos o en compañía y se den permiso para abrir líneas de expresión narrativas que sean de su talla. A veces les mando al abogado o al sindicato. Otras les doy de baja. Es frecuente que lo haga todo a la vez. Pero mi sensación es la del que tapa agujeros en la bodega de un barco que se hunde por mil vías de agua. No damos abasto. Tampoco las unidades de salud mental con las que trabajamos. Porque la solución al problema no puede ser sanitaria. No todo el malestar es problema de salud mental. 


Y en estas estamos cuando de repente explota un volcán en la tierra media y lo llena todo de humo y cascotes. Y vamos a catástrofe por mes para aderezar una situación que ya está calentita de por si. Más ceniza de miedo y agobio para el personal y las pantallas a escupir estopa por una explosión en un puente ruso o un nuevo huracán haciendo de las suyas.


A mis pacientes les invito a apagar pantallas y salir a pasear. Dirán que soy un rancio y que mi método es muy anticuado, tendrán razón. Pero a lo mejor se relajan un poco por el bosque y se encuentran a alguien y le dan los buenos días o se ponen a hablar. Por algún sitio hay que empezar. 








Mental Health in Middle-earth Tolkien's imagination created a mythology and set it in a place where diverse forces and interests clashed. But if you look at it closely, behind the epic backdrop lay the eternal struggle of opposites that humans have been experiencing since the Palaeolithic era. Good against evil, light against darkness, health against disease.... Today our world is perhaps more sophisticated, but our needs, desires and passions are not very different from those experienced by the characters of Middle Earth. We face similar challenges, we all have to deal with the same dark lord. Let me call it malaise and capitalise it. Discomfort has always been linked to ordinary life. Our ancestors experienced it to the highest degree in their travels and adventures. They went hungry, suffered many diseases, cold, attacks by wild animals or other fellow creatures, as well as war and its excesses. If the ailment was small, a scrape on a foot without shoes, a chilblain, a small wound, a diarrhoea, absolutely nothing was done. If it was more important, maybe something was done, and if it was very intense, it usually ended up killing you and that was the end of the problem. There were not many complications, death was assumed as consubstantial to life and was integrated into it. People mourned the dead but they moved on, they didn't waste their time on drama, life was very hard. Today things have changed. Our society enthroned the luminous wellbeing and tries to hide the dark death, minimise illness and expatriate malaise. The insane and the dying are taken out of the way, the dead are treated with the utmost hygiene and are kept away from children and sensitive people. On the other hand, society has changed enormously from an oppressive industrial model to a self-exploitative post-industrial one, where the pressure to be more efficient is increasingly delegated to everyone, increasing working hours and work demands. Work-life balance is impossible for many and maintaining a quality private and family life is increasingly difficult. Family and personal relationships suffer under work regimes that exploit and exhaust individuals who arrive home drained of energy. One in four people who visit a family doctor do so because they feel overwhelmed, nervous, discouraged or drained. The darkness of Mordor grows relentlessly and the Malaise extends its iron grip. It is true that the Elves can weather it better thanks to their higher social status, their better living conditions and their comfortable economic situation, and perhaps the Dwarves in their mines or some in their counties do not feel the oppression much. But in the end, they are all affected by the emotional oil spill that will increase as climate change or other disasters bring new catastrophes that will globalise new waves of pandemics, famine or hardship. What we as a society seem to have forgotten is both the true meaning of malaise and its transcendence. We have engaged in an unwinnable crusade against it because the shadow never completely disappears when there is light and when there is no light it reigns in the world. What is the point of darkness? My five year old daughter asked me this week, what is discomfort for? I ask. And the answers have to do with their opposites, since both light and darkness and wellbeing and discomfort are inseparable. Discomfort helps us to know what well-being is and to be grateful for it when we enjoy it. It allows us to detect what is wrong and gives us the opportunity to do something about it. It warns us when our life is heading for a cliff or has come too close to something that burns. Every vehicle comes from the factory with visual and audible indicators that warn us if we're running low on oil, the engine has overheated, or the brakes are failing. Those little red lights or high-pitched beeps are unbearable but invaluable. They allow us to take action and avoid greater evils. Nobody would think of hitting them with a hammer when they appear to stop them from bothering us. Today we may be somewhat orphaned of ways to deal with lifestyle discomforts because that same lifestyle encourages us to ignore them and look the other way. "Put a little more distraction in your life and move on" we seem to be told. But we see that with major ailments we have no use for TV series, videos and social media nonsense, nor for the music that our screens and headphones never fail to serve us. Marta Carmona and Javier Padilla in their book Malestamos share with us the thesis that in the face of the malaise that threatens our mental health it is necessary to go beyond the individual approach (psychotherapy or trade union) and take political and social measures that have an impact on the direct causes of the various malaises and on the causes of the causes, which are usually social or structural. I agree with them, but it seems to me very difficult to implement this in a situation that sacralises individuality, "freedom" and "do your own thing and stay out of trouble". Probably the realisation that alone we are very vulnerable and united we are more resistant can help us as ordinary citizens to prioritise once again a family and neighbourhood fabric that can shelter us and can shelter others, beyond the cold industrial social networks that disintegrate and dumb us down. I do not believe that this line of action can be implemented from above, although it is true that there are policies that can favour it, just as there are others that hinder it. As a family doctor, I am confronted every day with people who are plagued by malaise. Some because of their pain, chronic illnesses or emotional or family problems, others because of work overload, caring for others or the impotence of leading a dignified personal life. I offer listening, which is often lacking, and a perspective that tries to make it easier for the patient to understand what is happening and to allow them to make adjustments as they see fit. In a percentage of cases I offer medication or psychotherapy. In the majority I let them vent alone or in company and give them permission to open up narrative lines of expression that are their own size. Sometimes I send them to a lawyer or a union. Sometimes I discharge them. Often I do it all at once. But my feeling is that of someone who plugs holes in the hold of a ship that is sinking through a thousand leaks. We can't cope. Neither can the mental health units we work with. Because the solution to the problem cannot be sanitary. Not all discomfort is a mental health problem. And this is where we are when suddenly a volcano explodes in the middle earth and fills everything with smoke and debris. And we have one catastrophe per month to add spice to a situation that is already hot in itself. More ashes of fear and anxiety for the staff, and the screens will be spitting out the smoke from an explosion on a Russian bridge or a new hurricane doing its thing. I invite my patients to turn off the screens and go for a walk. They'll say I'm old-fashioned and my method is very old-fashioned, and they'll be right. But maybe they will relax a bit in the woods and meet someone and say good morning or start talking. You have to start somewhere.

La cuadratura del círculo sanitario. The squaring of the healthcare circle. 摆平健康圈。

La Atención Primaria madrileña sufre una situación de limitación de recursos y sobrecarga crónica de desenlace incierto. La presión asistencial es cada vez mayor en numerosos equipos que ven con desesperación como las ausencias de personal aumentan sin cesar en un horizonte sin refuerzos ni líneas de mejora creíbles mientras los pacientes esperan cada día más a ser atendidos. Los responsables de gestión reciben a su vez presiones del político de turno para reducir listas de espera y mejorar la accesibilidad pero es imposible deshacer el embrollo sin mojarse con aumentos de presupuesto o reformas estructurales que nadie se atreve a acometer. La última ocurrencia de la gerencia de Atención Primaria madrileña es crear “agendas de rebosamiento” en las que un médico asume ese día todos los pacientes “sin cita” que acudan al centro de salud. Lo que se traduce en el “más madera” de aquella película de los hermanos Marx en la que alimentaban la máquina de vapor del tren con la madera de los vagones del mismo. El problema es que nos estamos quedando sin tablas, ya no queda nada que quemar. Aumentar el número de pacientes vistos disminuyendo tiempos de visita y calidad de asistencia, destruyendo la longitudinalidad y maltratando a los profesionales tal vez sea una opción a corto plazo, pero no es mantenible. 


No entraré en la cuestión de si es necesario subir o bajar impuestos dada la alta carga demagógica del aserto y el elevado contenido emocional que suele acompañar a quien defiende una u otra opción. Más bien animaré la reflexión sobre de qué forma la sociedad puede ayudar a que su sanidad pública no colapse, los gestores y políticos a plantear líneas de actuación reales y los profesionales a mantener el tipo en lo posible. La ciudadanía es quizá la única que tiene la sartén por el mango, sin saberlo. Primero necesita conocer el verdadero diagnóstico del caso: shock hipovolémico, es decir insuficiencia circulatoria por pérdida de fluidos vitales (económicos y de recursos humanos). Segundo admitir el pronóstico: el caso tiene muy mala pinta. Tercero asumir el tratamiento y su coste: medidas de cuidados intensivos que requieren reformas severas e inversión millonaria. En caso de no asumir la factura la sanidad terminará convertida en zombi, una organización ni muerta ni viva que seguirá funcionando con cada vez menos vida en las venas. Los telediarios deberían recordarnos a diario que en 5 años un 25% de los médicos de familia de este país se jubilarán. Y no habrá renovación, los más jóvenes cada vez eligen destinos en otros países o en puestos no tan sobrecargados. Si seguimos sin hacer nada lo que seguro habrá que asumir es que cuando llegue la hora de la ancianidad o la enfermedad todos lo pasaremos peor, los que tengan menos recursos más.

GP overloading is increasing in many teams, which are desperate to see how staff absenteeism is constantly on the increase, with no reinforcements or credible lines of improvement on the horizon, while patients are waiting longer and longer to be attended to. Those in charge of management are in turn pressured by the politician on duty to reduce waiting lists and improve accessibility, but it is impossible to undo the mess without getting involved in budget increases or structural reforms that no one dares to undertake. And there will be no renewal, the younger ones are increasingly choosing postings in other countries or in jobs that are not so overburdened. If we continue to do nothing, what we will surely have to accept is that when the time comes for old age or illness, we will all have a harder time, and those with fewer resources will have a harder time.

Reflexiones sobre el fracaso en el mundo sanitario. Reflections on failure in the healthcare world. 对医疗界失败的反思




 Publico hoy en la página de la Gestió importa un artículo sobre el fracaso. Con la que está cayendo pienso que puede ser de ayuda tomar conciencia de las posibles sombras para valorar y agradecer las luces. En un mundo como el nuestro tan reacio a asumir errores y fracasos perdemos la posibilidad de aprender de ellos y ayudarnos para corregir rumbos o enmendar heridas. Espero les resulte de utilidad. 




Tener una residente en la consulta implica más trabajo, pero tiene ventajas. Una de ellas es estimular la reflexión y la toma de conciencia del tutor. Se parece un poco al hecho de encender la luz un día de nubes, todo parece verse mejor en la habitación. La pregunta que me surgió es cómo afrontar la docencia del manejo del fracaso en un mundo como el sanitario que no parece tolerarlo y lo invisibiliza siempre que puede. 

 

Reflexionaba esta semana sobre el concepto de fracaso que va un poco más allá del de error médico que nos es más habitual. Crear una cultura de mejora continua implica de manera imprescindible trabajar con el error puntual pero cuando hablamos de fracaso reconocemos algo más: el malogro de un proyecto personal o profesional cuya línea de tiempo no alcanza objetivos o termina sin éxito. Tanto los errores como los fracasos duelen y suscitan sentimientos adversos. Hieren la autoestima y afectan el estado de ánimo. En ocasiones pueden alterar la paz mental y afectar el desempeño laboral. Estas consecuencias no se suelen hablar y cada cual se apaña como puede dentro de un ambiente de trabajo que no ayuda a compartir estas cuestiones. 

 

Sobre el fracaso personal

 

Dentro de mis fracasos profesionales detecté un buen puñado al hacer una retrospectiva, permítanme compartirles unos cuantos. Los cuatro años de desierto laboral post-MIR en los que tuve que trabajar de todo menos de médico de familia por no encontrar un contrato digno. El circuito de pacientes sin cita que hicimos en Lavapiés en consenso con administrativos y enfermeras que duró dos semanas. El fracaso de la conciliación laboral con cuatro hijos al conseguir un turno de mañana a los 49 años. El fracaso como director de centro de salud cuando dimití por desavenencias con la dirección. El fracaso de mis proyectos de innovación (blog para pacientes, redes sociales, vídeos y salud…). Mi fracaso reivindicativo de la situación de la Atención Primaria…

 

Y ahí tenemos a los brillantes médicos MIR enfrentándose a diario con fracasos que no preveían. El posible fracaso en las urgencias, donde al principio son totalmente incapaces y después a penas se mantienen a flote. El posible fracaso con algunas rotaciones, técnicas o aprendizajes. El posible fracaso en las relaciones laborales. El posible fracaso en su esfera personal y relacional… Pero nadie dice nada de esa cara B dado que lo que se publica en las redes sociales tiene más que ver con la versión luminosa de cada cual en la que no cabe la sombra. 

 

A nivel de organización pasa lo mismo, nos cuesta hablar del fracaso. Es verdad que somos muy buenos a la hora de quejarnos o señalar fallos y errores ajenos. Y si son de categorías profesionales distintas, otras especialidades, el mundo de la gestión sanitaria en general o la política sanitaria en particular, somos verdaderos expertos. Pero reconocer el fracaso de nuestra unidad funcional, nuestra categoría profesional o especialidad es excepcional dado que esto nos mete en la ecuación y nos señala y a nadie le gusta salir en determinadas fotos. 

 

Admitir un fracaso implica un juicio. Es necesario dictaminar que un proyecto se ha malogrado. Si hablamos de un fracaso personal esto solo será posible si lo reconocemos. Para ello hace falta un ejercicio de humildad que reconozca lo que hay. No es necesario pasarnos de punitivos o de condescendientes, tan solo admitir que como humanos fallamos y fracasamos continuamente como parte de nuestra naturaleza. Y luego tener el valor de admitirlo ante los demás, algo que no suele ser sencillo pero que tiene un potente efecto de catarsis y liberación. 

 

Sobre el fracaso del sistema de salud

 

Si echamos una mirada amplia a nuestro sistema sanitario no tardaremos en ver fracasos por todas partes. Algunos pequeños, otros más grandes. Ha fracasado una Atención Primaria cada vez más ahogada, fracasan las ardientes urgencias de hospital, fracasa la atención a la salud mental, fracasa el sistema cuando sobremedica, fracasa la gestión sanitaria cuando se maltrata al personal, fracasa la sociedad cuando abusa del sistema. 

 

Pero no nos podemos quedar en la penumbra, si bien es necesario admitirla, reconocerla y airearla. Creo que es sano asumir las luces y las sombras que hay en nuestras vidas y en las personas e instituciones que nos rodean. Sombras que como ya percibió Carl Jung compartimos dentro de ese gran inconsciente colectivo donde se albergan sueños y pesadillas comunes. Reflections on failure in the healthcare world


Today I am publishing an article on failure on the Gestió importa website. The question that arose in my mind is how to deal with the teaching of failure management in a world such as healthcare, which does not seem to tolerate it and makes it invisible whenever it can. 

 

This week I was reflecting on the concept of failure, which goes a little further than the more commonplace concept of medical error. Creating a culture of continuous improvement implies working with the occasional error, but when we talk about failure we recognise something else: the failure of a personal or professional project whose timeline does not achieve its objectives or ends unsuccessfully. Both mistakes and failures hurt and arouse adverse feelings. They hurt self-esteem and affect mood. They can sometimes disturb peace of mind and affect job performance. These consequences are often not talked about and everyone copes as best they can in a work environment that is not conducive to sharing these issues. 

 

On personal failure

 

Among my professional failures I detected a handful in retrospect, let me share a few with you. The four years of post-MIR labour desert in which I had to work as anything but a family doctor because I could not find a decent contract. The circuit of patients without appointments that we made in Lavapiés in consensus with administrative staff and nurses that lasted two weeks. The failure to reconcile work and family life with four children by getting a morning shift at the age of 49. The failure as director of a health centre when I resigned due to disagreements with the management. The failure of my innovation projects (blog for patients, social networks, videos and health...). My failure to make demands about the situation of primary care...

 

And there we have the brilliant MIR doctors facing daily failures that they did not foresee. The possible failure in the emergency department, where at first they are totally incapable and then they can barely stay afloat. The possible failure with some rotations, techniques or training. Possible failure in working relationships. The possible failure in their personal and relational sphere... But nobody says anything about this B-side, given that what is published on social networks has more to do with the luminous version of each person in which there is no room for the shadow. 

 

At the organisational level it's the same thing, we find it difficult to talk about failure. It is true that we are very good at complaining or pointing out other people's failures and mistakes. And if they are of different professional categories, other specialties, the world of health management in general or health policy in particular, we are real experts. But acknowledging the failure of our functional unit, our professional category or speciality is exceptional because it brings us into the equation and points the finger at us, and nobody likes to be in certain photos. 

 

Admitting failure implies a judgement. It is necessary to rule that a project has failed. If we are talking about a personal failure, this will only be possible if we acknowledge it. This requires an exercise in humility that recognises what is there. It is not necessary to be overly punitive or condescending, just to admit that as humans we fail and fail continuously as part of our nature. And then have the courage to admit it to others, which is often not easy but has a powerful cathartic and liberating effect. 

 

On the failure of the health system

 

If we take a broad look at our health care system, it is not long before we see failures everywhere. Some small, some larger. Primary care has failed when it is increasingly suffocated, hospital emergency departments have failed, mental health care has failed, the system has failed when it overmedicates, health management has failed when it mistreats staff, society has failed when it abuses the system. 

But we cannot remain in the shadows, although it is necessary to admit them, to recognise them and to air them. I believe it is healthy to assume the lights and shadows in our lives and in the people and institutions that surround us. Shadows that, as Carl Jung already perceived, we share within that great collective unconscious where common dreams and nightmares are harboured. Juan Gérvas describes family doctors as long term losers in an expression that has to do with what we are talking about and which contains a seed of perseverance and hope. It is all right if we lose things along the way if we know where we want to go and what values we want to defend.
