Gobernanza de datos en salud. Data Governance in Health. 健康數據治理






Es fundamental empezar a hablar de Gobernanza de Datos en salud, un término reciente que aun no tiene traducción al español en Wikipedia. Lo cierto es que todo el mundo habla de datos y muchos se frotan las manos con los enormes y riquísimos bancos de información sanitaria existentes y potenciales. Los teóricos del Big Data nos vienen cantando los beneficios que vendrán de la explotación de los mismos pero se habla bien poco de como diseñar una regulación correcta para evitar que se vuelvan contra el ciudadano.

En una época en la que nuestros desplazamientos, compras, historial de navegación e interacciones sociales virtuales quedan registradas y son usadas por diversas compañías, imaginen lo que ocurrirá cuando nuestros genomas, historiales sanitarios y datos biométricos estén en Internet.

Sería deseable que toda organización sanitaria se dote de estrategias transparentes y consensuadas de Gobernanza de Datos como que a nivel institucional y de regulación legal se implementen líneas estratégicas que velen por los derechos de los usuarios. No sería muy inteligente delegar este tema en compañías de software o tecnológicas como no lo es dejar al zorro al cuidado del gallinero.






Data Governance in Health


It is essential to start talking about Data Governance in Health, a recent term that has not yet been translated into Spanish in Wikipedia. The truth is that everyone talks about data and many rub their hands with the enormous and very rich existing and potential health information banks. Big Data theorists are singing to us about the benefits that will come from their exploitation, but there is little talk about how to design a correct regulation to prevent them from turning against the citizen.

At a time when our travel, shopping, browsing history and virtual social interactions are recorded and used by various companies, imagine what will happen when our genomes, health records and biometric data are on the Internet.

It would be desirable that all health organizations equip themselves with transparent and consensual strategies of Data Governance, such as that at the institutional and legal regulation levels strategic lines are implemented to watch over the rights of users. It would not be very intelligent to delegate this issue to software or technological companies as it is not to leave the fox in the care of the henhouse.

Una de las más famosas leyendas del Titanic es la relativa a su orquesta musical. Durante el hundimiento, los ocho miembros de la banda dirigidos por Wallace Hartley, se situaron en el salón de primera clase en un intento por hacer que los pasajeros no perdieran la calma ni la esperanza. Más tarde continuaron tocando en la parte de popa de la cubierta de botes. La banda no dejó de tocar incluso cuando ya era seguro que el buque se hundiría (cita de Wikipedia).



Esta semana la ministra de sanidad convocó una reunión urgente para abordar la problemática de la Atención Primaria española que como todos los inviernos no deja de acumular titulares de prensa. Seguramente tuvo que escuchar los datos objetivos que hablan del deterioro estructural de la misma, la sobrecarga asistencial progresiva, la falta de recursos y la delicada situación de los profesionales. Seguramente le enseñaron los números que indican como cada vez es más difícil encontrar médicos de familia que huyen de los contratos basura que les ofrecen a otros países que ofertan mejor trato. Las gráficas que indican que en pocos años se jubilará un alto porcentaje de los médicos más veteranos. Las pirámides de población que indican que cada vez habrá más pacientes crónicos complejos...

No debe ser fácil ser la responsable de un derecho constitucional como la atención al cuidado de la salud cuando la estructura de transferencias de los servicios sanitarios a las Comunidades Autónomas ha sido desde el principio un epítome de descoordinación, mercantilización política y duplicación de gasto (sistemas de información multiplicados, compras de medicamentos, tecnología y servicios externos no centralizados entre otras cosas). Y claro, lo que no han arreglado otros no lo voy a arreglar yo...

De momento el movimiento de ajedrez de la señora ministra es propiciar un nuevo grupo de trabajo para establecer un libro de directrices que es casi lo mismo que han hecho docenas de veces antes que ella. Repetimos el día de la marmota en un ejemplo de pérdida de oportunidad al que estamos tristemente acostumbrados; así no se ganará la partida jamás. ¿Qué profesional sanitario no ha leído algún libro blanco de su especialidad o rol profesional, de su estructura de servicio sea hospitalaria o ambulatoria, de la Comunidad correspondiente o de la sociedad científica o Colegio Profesional de turno? 

Cuando seguimos viendo como las instituciones fallan a la hora de gestionar un servicio público no podemos evitar pensar qué deriva llevamos dentro de unas organizaciones zombis que cada vez están más muertas que vivas sostenidas por hordas de trabajadores cada vez más agotados y desmotivados. Y es que cuando un médico de familia termina su turno tras haber atendido a sesenta personas o el personal de enfermería de un servicio de urgencia se revienta día tras día en lo que parece una batalla perpetua dan ganas de colgar la bata y meterse en política, o en cualquier actividad lejos de la asistencia.

Las soluciones llevan tiempo encima de la mesa. Desde hace lustros sabemos que el camino debe ir hacia la autonomía de gestión, la motivación del personal, la flexibilización de la prestación de servicios y la educación en salud y en uso del sistema sanitario de la ciudadanía, entre otras cosas. El problema sigue siendo quién le pone el cascabel al gato dado que una profunda reforma del sistema sanitario público exige "sangre, sudor y lágrimas" de los contribuyentes y eso tiene un coste político que requiere un nivel de agallas, ética y búsqueda del bien común que hasta el momento nadie ha dado.

Dan ganas de pedir las dimisiones de aquellos con responsabilidades en el asunto. Pero nos saldría una lista inasumible por ser terriblemente extensa. Ministras, Consejeros de Sanidad, asesores, técnicos de dirección, directoras, vicedirectores y demás personal de gestión conforman un auténtico ejercito dentro de cada uno de los 17 sistemas sanitarios autonómicos. Lo cierto es que pese a que la alternancia política los cambie de vez en cuando como máximo se abordan pequeños arreglos cosméticos de chapa y pintura sin que nadie se atreva a impulsar la profunda reforma que el sistema lleva décadas pidiendo a gritos.

El escenario es de alta dificultad dado que la propia organización sanitaria es un sistema matemático complejo que fue creado para una coyuntura político social determinada que ha cambiado bastante y evoluciona a un ritmo endiabladamente rápido para la rigidez e inercia de organizaciones piramidales sólidas con mínimo margen para la innovación y el cambio. ¿Quién acometerá la labor de repensar el sistema y de implementar los cambios necesarios para conseguir la transición?  Probablemente nadie desde dentro del mismo sistema, habrá que salir con redes a pescar en las oceánicas aguas sociales y elevar lo pescado tanto a la ciudadanía como a una mesa de consenso en la que participen representantes políticos de todo color. Mucho tendremos que cambiar para dejar de politizar algo tan goloso como la sanidad que mueve un gran porcentaje de la tarta del dinero público y aprender a mirar más allá del propio beneficio para buscar el bien social. Si no se consigue el mercado será quien gane la partida y termine tomando las decisiones. Si esto ocurre la sociedad habrá perdido un tesoro que nunca llegó a entender plenamente ni a valorar en lo que vale.

Mientras tanto la orquesta a la que pertenezco sigue acometiendo partituras livianas mientras el gran navío se hunde, lento pero constante, en las frías aguas de una coyuntura económica adversa. Si nos fijamos bien hay lágrimas en muchos de los músicos, igual de saladas que el mar coyuntural que en poco tiempo los recibirá con los brazos abiertos.

This week the Minister of Health convened an urgent meeting to address the problem of Spanish Primary Care which, like every winter, does not cease to accumulate headlines. Surely she had to listen to objective data that speak of the structural deterioration of the same, the progressive overload of care, the lack of resources and the delicate situation of professionals. Surely they taught her the numbers that indicate how it is increasingly difficult to find family doctors, fleeing abroad from the garbage contracts offered. The graphs indicate that in a few years a high percentage of the most veteran doctors will retire. In addition the population pyramids indicate that there will be more and more complex chronic patients...

It should not be easy to be responsible for a constitutional right such as health care when the structure of transfers of health services to the Autonomous Communities has been from the beginning an epitome of lack of coordination, political commodification and duplication of expenditure (multiplied information systems, non-centralized purchases of medicines, technology and  external services among other things). And of course, what others have not fixed, I am not going to fix...

At the moment the chess movement of the lady minister is to propitiate a new working group to establish a guidebook that is almost the same as they have done dozens of times before her. We repeat the day of the marmot in an example of loss of opportunity to which we are sadly accustomed; this way the game will never be won. What health professional has not read a white paper of his specialty or professional role, of his service structure whether it be hospital or ambulatory, of the corresponding Community or of the scientific society or the Professional College on duty? 

When we continue to see how the institutions fail when it comes to managing a public service, we cannot help but think about what drifts we are taking within zombie organizations that are more and more dead than alive, sustained by hordes of workers who are more and more exhausted and unmotivated. The fact is that when a family doctor finishes his shift after having attended to sixty people or the nursing staff of an emergency service burst day after day in what seems like a perpetual battle, they secretly desire to hang up the robe and get involved in politics, or in any activity far from assistance.

Solutions have been on the table for some time. For years we have known that the path must go towards management autonomy, staff motivation, flexibility in the provision of services and education in self-care and in a better use of the health system by the citizenship, among other things. The problem continues to be who puts the rattle on the cat, given that a profound reform of the public health system requires "blood, sweat and tears" from taxpayers and that has a political cost that requires a level of guts, ethics and the search for the common good that no one has so far given.

This situation makes you want to call for the resignations of those with responsibilities in the matter. But we would get an unassumable list because it is terribly long. Ministers, Health Counsellors, advisors, management technicians, directors, deputy directors and other management personnel make up a real army within each of the 17 autonomous health systems. What is certain is that in spite of the fact that political alternation changes them from time to time at the most, small cosmetic arrangements of sheet metal and paint are dealt with without anyone daring to promote the profound reform that the system has been crying out for decades.

The scenario is very difficult, given that the health care organization itself is a complex mathematical system that was created for a particular political and social situation that has changed quite a bit and is evolving at a devilishly fast pace for the rigidity and inertia of solid pyramidal organizations with a minimum margin for innovation and change. Who will undertake the task of rethinking the system and implementing the changes necessary to achieve the transition?  Probably nobody from within the same system, will have to go out with nets to fish in the oceanic social waters and raise the fish both to the citizens and to a consensus table in which political representatives of all colors participate. Much will have to change to stop politicizing something as greedy as health that moves a large percentage of the pie of public money and learn to look beyond the benefit itself to seek the social good. If we don't succed the market will be the one who wins the game and ends up making the decisions. If this happens, society will have lost a treasure that it never fully understood or valued.

Meanwhile the orchestra to which I belong continues to attack light scores while the great ship sinks, slowly but steadily, into the cold waters of an adverse economic situation. If we look closely, there are tears in many of the musicians, just as salty as the conjunctural sea that will soon receive them with open arms.

Go bust


When an old bookstore closes something breaks into that invisible warp that sustains the worlds.  I have to confess that I feel a certain pain and an aftertaste of uncertainty before the sinking of an old time that does not resist the speed of the new one. This week closed a well-known bookstore in Madrid that I knew well both for having been there and for having passed by countless times. A medical bookshop full of good science books that hardly compete with digital worlds and multiplatform formats that do not need paper.

I work in a health center in a small town in the mountains of Madrid. It is increasingly difficult to find doctors who want to work in rural areas. It is almost impossible to find substitutes when one of us is absent due to illness or a leave of absence. In these moments I attend my clinic slowly with the box of tissues on the table taking care of patients with my same symptoms. It is not technically correct but I prefer it to leave without service the clinic. So do most of my colleagues in these circumstances, as things have been for too long.

Somehow I sense that many small towns will be left without medical service, will be closed health centers twinned with those bookstores that went bankrupt. And nothing will happen. We will hear complaints that will be covered by the avalanche of shouts, scandals and news that the television and social networks spit without rest. Nothing will happen.




Cuando una vieja librería echa el cierre algo se quiebra en esa invisible urdimbre que sostiene los mundos. He de confesar que noto cierto dolor y un regusto de incertidumbre ante el hundimiento de un tiempo antiguo que no resiste la velocidad del nuevo. Esta semana cerró una conocida librería de Madrid que conocía bien tanto por haber estado en ella como por haber pasado por delante infinidad de veces. Una librería médica llena de buenos libros de ciencia que cada vez lo tienen más difícil al competir con mundos digitales y formatos multiplataforma que no precisan de papel.

Trabajo en un centro de salud de un pequeño pueblo de la sierra madrileña. Cada vez es más difícil encontrar médicos que quieran trabajar en zona rural. Es casi imposible encontrar sustitutos cuando alguno de nosotros faltamos por enfermedad o algún permiso. En estos momentos paso la consulta a medio gas con la caja de pañuelos de papel encima de la mesa atendiendo a pacientes con mis mismos síntomas. No es que sea muy correcto pero lo prefiero a dejar sin servicio a mis paisanos. Lo mismo hacen la mayoría de mis compañeros en estas circunstancias, así están las cosas desde hace ya demasiado tiempo.

De alguna forma intuyo que muchos pueblos se irán quedando sin servicio médico, se irán cerrando centros de salud hermanados con esas librerías que quebraron. Y no pasará nada. Oiremos quejas que quedarán tapadas por el alud de gritos, escándalos y noticias que la televisión y las redes sociales escupen sin descanso. No pasará nada.

The double bind of Spanish Primary Health Care

We've all seen a spider web once and we know what happens when an insect falls into it.  We have witnessed its fear and its tempest of movements that have entangled him even more, clearly warning the owner of the trap that will come quickly to collect her piece.

Spanish Primary Health Care has been suffering from a double bind problem for decades. On the one hand, it is said that the national health system is one of the best in the world, that our health centres are state-of-the-art and that we have wonderful professionals. On the other hand, we face a terrible panorama of reduced resources, increased healthcare pressure and greater complexity of cases due to adverse economic and social conditions and the ageing of the population.

The result is an uncomfortable blockade of health professionals vertebrate by a deep sense of discomfort and a storm of coffee break complaints that despite their insistence do not produce the slightest change. The most that managers seem to arrive at is to design changes of all kinds that do not incur in expenditure but that do not seem to significantly improve the model either. This generates a schizophrenic situation that produces suffering in those who suffer it.

It doesn't matter if people die waiting in the emergency room, if the director of a health centre resigns or if some family doctors resign their posts. Since the exception does not move mill we continue as we are, entangled in an increasingly sticky spider web, drowned in complaints and with the great spider of the free market increasingly eager to devour the precious public health slice of which will make huge profits.

As a doctor with some experience, I know firsthand that excessive complaining is toxic and tangles and ruins the sufferer. That is why I try to dose my own even though I have the same motives as countless other colleagues in the profession.

Reforming the current model from its foundations requires a powerful professional leadership (which in our environment is practically impossible due to the atavistic separation of interests and collectives) added to a wide social and political debate and a generalized awareness of the fragility of a public service that must be cared for by all in order to survive. If health continues to be considered a consumer good, health needs will always tend to infinity. There will not be a system capable of covering them all unless the limits are well drawn and citizens are helped to be co-responsible for their own health.

Complex problems cannot be solved with simplistic solutions. Many think it is a question of lack of money, others of political ill will. It is much more. At the moment the most fragile piece of the chain is still the human one, thousands of professionals at the limit who carry out their task under very harsh conditions. We will continue to see in the news patients who die waiting for care, professionals who give up or escape to other countries, patch-like solutions. And the situation will deteriorate over time.

Solutions? after attending to several dozen patients this morning in a fragrant catarrhal state do not ask me. By avoiding getting too involved with the spider web of the complaint today I have more than enough.






Todos hemos visto alguna vez una tela de araña y sabemos lo que pasa cuando un insecto cae en ella. Hemos sido testigos de su miedo y de su tempestad de movimientos que le han enredado más si cabe avisando a las claras a la propietaria de la trampa que acudirá rauda a cobrar su pieza.

La Atención Primaria española sufre un problema de doble vínculo desde hace décadas. Por un lado se dice que la sanidad nacional es de las mejores del mundo, que nuestros centros de salud son punteros y que tenemos estupendos profesionales. Por otro nos encontramos con un terrible panorama de reducción de recursos, aumento de presión asistencial y mayor complejidad de los casos por las condiciones económico-sociales adversas y el envejecimiento poblacional.

La resultante es un bloqueo incómodo de los profesionales sanitarios vertebrado por una profunda sensación de incomodidad y una tormenta de quejas de café que pese a su insistencia no producen el más mínimo cambio. A lo más que parecen llegar los gestores es a diseñar cambios de todo tipo que no incurran en gasto pero que tampoco parece que mejoren significativamente el modelo. Se genera pues una situación esquizofrénica que produce sufrimiento y llanto en una gran mayoría de los profesionales.

Da igual que se muera la gente esperando en urgencias, que dimita el director de un centro de salud o renuncien a su puesto algunas médicas de familia. Dado que la excepción no mueve molino seguimos como estamos, enredados en una tela de araña cada vez más pegajosa, ahogados en quejas y con la gran araña del libre mercado cada vez con más ganas de devorar la preciada tajada sanitaria pública de la que conseguirá pingües beneficios.

Como médico que soy con algo de experiencia, conozco de primera mano que el exceso de queja es tóxico y enreda y malogra al que lo sufre. Por eso trato de dosificar las propias pese a que tenga motivos igual que otros incontables colegas de profesión.

Reformar el modelo actual desde sus cimientos requiere un liderazgo profesional potente (que en nuestro medio es prácticamente imposible por la atávica separación de intereses y colectivos) sumado a un amplio debate social y político y una toma de consciencia generalizada de la fragilidad de un servicio público que es necesario cuidar entre todos para que subsista. Si la salud sigue considerándose un bien de consumo, las necesidades de salud tenderán siempre al infinito. No habrá sistema capaz de cubrirlas todas salvo que se dibujen bien los límites y se ayude a la ciudadanía a ser corresponsable de su propia salud.

Los problemas complejos no se solucionan con soluciones simplistas. Muchos piensan que es una cuestión de falta de dinero, otros de mala disposición política. Es mucho más. De momento la pieza más frágil de la cadena sigue siendo la humana, miles de profesionales al límite que desarrollan su tarea en condiciones muy duras. Seguiremos viendo en las noticias pacientes que mueren, profesionales que renuncian o escapan a otros países, soluciones tipo parche. Y la situación se irá deteriorando con el tiempo.

¿Soluciones? tras atender haber atendido esta mañana varias docenas de pacientes en un fragrante estado catarral no me las pidan. Con evitar liarme demasiado con la telaraña de la queja tengo hoy más que suficiente.

Tiempo para los pacientes. Time for patients. 患者的時間



Time for patients


Winter tends to bring the annual Tsunami to Spanish health centres because of:

1. The concurrence of influenza and winter illnesses.
2. Shortfall of health professionals.
3. Overuse of the health system by a population that does not know how to manage minor health problems.



This implies:

1. Less consultation time per patient.
2. Much more stress for the health professional.
3. Increased risk of errors in consultation.
4. Increased patient waiting and therefore more stress.
5. Deficient health care due to the sum of the above factors.



If I had to make three new year wishes I would ask:

1. A dignified time to take good care of each patient.
2. A better health education of the population.
3. Better management of the health structure and human resources.







El invierno suele traernos el Tsunami de todos los años a los centros de salud españoles por tres factores:

1. La concurrencia de la gripe y las enfermedades invernales.
2. Unas plantillas de profesionales sanitarios deficitarias.
3. Un sobreuso del sistema sanitario por una población que no sabe gestionar problemas de salud menores.



Esto implica:

1. Menos tiempo de consulta por paciente.
2. Mucho más estrés para el profesional sanitario.
3. Aumento del riesgo de cometer errores en consulta.
4. Aumento de la espera del paciente y por consiguiente más estrés.
5. Una atención sanitaria deficitaria por la suma de los anteriores factores.



Si tuviera que pedir tres deseos:

1. Un tiempo digno para atender bien a cada paciente.
2. Una mejor educación de la población.
3. Una mejor gestión de la estructura sanitaria y de los recursos humanos.

Seven Threats and Two False Promises of Health Big Data



One. Increased anxiety. Increased health concern for people who receive notifications of personal health values outside the normal limit.

Two: Constant increase in available biometric variables.

Three. Loss of privacy and intimacy.
The lack of traceability of personal data once uploaded to the cloud makes it impossible to know what use is made of them or to whom they are sold or transmitted. The possible copy and distribution that can be made with the big data will generate big benefits for those who commercialize it but not for those who generate it.

Four. Loss of economic and social rights.

Due to big data, insurance companies may vary premiums. Also the selection of Personnel and the human resources services can use these data to prime healthy profiles of a certain psychological orientation and to discard others that entail illnesses or potential problems.

There is no doubt that data are a new resource but the generation of tools and algorithms to obtain value from them will not be within everyone's reach.



Final reflection.

There are potential positive aspects but it is necessary to develop a deep reflection in order to implement a big data in health based on values and that promotes the defense of public health as well as personal health. Ensuring that big data in health does not lead to an uncontrollable lucrative business is an obligation for all social agents at the political level as well as at the level of health professionals and civil society in general. If this reflection is not carried out in depth, the market will favour the development of initiatives aimed at economic rather than social benefit. Speaking of health, this can be dangerous.

Uno. Aumento de la ansiedad. Aumento de la preocupación salud de las personas que reciban notificaciones de valores personales fuera del límite de normalidad. 


Dos. Aumento constante de las variables biométricas disponibles. 

Tres. Pérdida de privacidad e intimidad.
La falta de trazabilidad de los datos personales una vez subidos a la nube hace imposible saber que uso se hace de ellos ni a quién se venden o transmiten. El posible corta pega que se pueda hacer con el big data generará pingües beneficios para quien lo comercialice pero no para quien lo genera.

Cuatro. Pérdida de derechos económicos y sociales.

Atendiendo al big data las compañías de seguros pueden variar las primas. Asimismo la selección de Personal y los servicios de recursos humanos pueden utilizar estos datos interés sanitario para primar perfiles sanos de una determinada orientación psicológica y descartar otros que conlleven enfermedades o problemas.

Cinco. Aumento del sobre diagnóstico y sobre tratamiento.

Ante tantos datos biométricos indeterminados mucha gente se sentirá insegura. Valores fuera de los límites de normalidad pueden generar cadenas diagnósticas o terapéuticas innecesarias.

Seis. Bombardeo de publicidad de alto impacto sobre productos de salud con base en los datos. Mensajes que será difícil obviar al tocar el ámbito de nuestra salud que siempre será prioritario.

Siete. Modificación de la estructura sanitaria en búsqueda del beneficio económico y no del social o personal.



Promesas falsas:



Uno. Se mejorará la salud

No del todo claro dado que si bien pueden mejorarse aspectos de eficiencia institucional a nivel personal no está cuantificada esta mejora. Por otro lado habría que sopesar si la posible mejora en indicadores de salud compensaría el aumento de la preocupación por la salud personal o el aumento de la percepción de malestar o mala salud. 

Dos. Generará riqueza.

Si bien esto es cierto, es presumible que la explotación de big data sanitario se lleve a cabo mediante grandes corporaciones pero podemos inferir que será difícil la explotación a nivel de investigadores o de pequeñas empresas. No hay duda de que los datos son un nuevo recurso pero la generación de herramientas y algoritmos para obtener valor de ellos no estará al alcance de todos.



Reflexión final.

Hay aspectos positivos potenciales pero es necesario desarrollar una profunda reflexión para poder implementar un big data en salud basado en valores y que promueva la defensa de la salud pública tanto como de la personal. Velar para que el big data en salud no derive en lucrativo negocio es una obligación para todos los agentes sociales tanto a nivel político como de profesionales sanitarios y sociedad civil en general. Si esta reflexión no se consigue hacer en profundidad el mercado favorecerá el desarrollo de iniciativas dirigidas al beneficio económico y no al social. Hablando de salud esto puede ser peligroso.

Felices fiestas. Happy Christmas. 聖誕快樂




Happy Christmas.

This year has been moved in many ways.  I have been struck by the fact that some social networks and technological companies in which we had placed our trust have badly trafficked citizen data, provoking political, economic and social storms that are likely to increase. 

Primary Care culminated in Astana with a statement that ended decaffeinated by the enormous weight of market guidelines that seem to be much greater than those that logically defend citizens. 

Dr. Veronica Casado was elected best international family doctor, which has pleased many. The quality level of Spanish health professionals remains high despite the pressures to which the public system subjects us. To continue working to do things as best as possible is an enormous challenge ahead of us. 

For my part, I have changed the family medicine practice from a city like Collado Villalba to a mountain village like Miraflores. I regret the loss of longitudinality of my previous patients that on the other hand is forcing me to update and improve in many aspects given the greater complexity and age of my current patients. 

As you can see I still keep this blog alive though the pace of publications is quieter and the format has become trilingual. The same happens with the social networks that I maintain, mainly Twitter, where my level of participation is much more relaxed. Have a good time at these parties, I wish you all the best for the year that begins and if you wish we continue to see each other through these pages.




Este año ha sido movido en muchos sentidos. Me ha llamado mucho la atención como unas redes sociales y unas empresas tecnológicas en las que habíamos confiado han traficado malamente con los datos de la ciudadanía provocando tormentas políticas, económicas y sociales que probablemente vayan a más. 

La Atención Primaria tuvo su culmen en Astana con una declaración que terminó descafeinada por el enorme peso de las directrices del mercado que parecen ser muchos mayores que las que en buena lógica defienden a la ciudadanía. 

La doctora Verónica Casado fue elegida mejor médica de familia internacional lo que no ha alegrado a muchos. El nivel de los profesionales sanitarios españoles sigue siendo alto pese a las presiones a las que el sistema público nos somete. Seguir trabajando para hacer las cosas lo mejor posible es un enorme reto que tenemos por delante. 

Por mi parte he cambiado la consulta de medicina de familia de una ciudad como Collado Villalba a un pueblo serrano como Miraflores. Lamento la pérdida de longitudinalidad de mis anteriores pacientes que por otra parte me está obligando a actualizarme y mejorar en muchos aspecto dada la mayor complejidad y edad de mis pacientes actuales. 

Como ven sigo manteniendo vivo este blog aunque el ritmo de publicaciones es más tranquilo y el formato ha pasado a ser trilingüe. Lo mismo pasa con las redes sociales que mantengo, fundamentalmente Twitter, donde mi nivel de participación es mucho más relajado. Pásenlo bien en estas fiestas, les deseo todo lo mejor para el año que empieza y si lo desean nos seguimos viendo por estas páginas.

¿Es posible regular la biometría? Is it possible to regulate biometrics? 是否有可能調節生物識別技術?








Is it possible to regulate biometrics?



Hand in hand with advances in mobile telephony we are getting used to get more and more features in our personal communication devices.  This implies that the hardware we carry is going to have a greater capacity to measure vital parameters. The mobile knows the steps we take, the speed of these and the rhythm of movement at the same time that can detect the time of rest or sleep. There are telephones capable of measuring heart beats, knowing the rhythm, even doing an electrocardiogram.

To the extent that these advances condition an increase in biometrics, it is logical to think that they will detect conditions or patterns that they consider abnormal. When the measuring subject is confronted with them, he will have no way of knowing whether they are normal or not. This will condition an increase in the number of consultations with health professionals to whom the measurements obtained will be presented in order to try to find an interpretation. 

At present, wearables, bracelets, smart phones, smart rings, etc. are predominantly designed for a young and athletic profile. These are used to improve or maintain the physical condition. As soon as they become universal, a growing number of citizens will use them, including people with different degrees of disease or risk factors for it.

Biometrics is undoubtedly attractive because it offers a quantification of one's own being and allows us to measure it, giving us a feeling of control that we can direct towards achieving better health. The dark side of this over-measurement has to do with the inevitable concern that will arise from measurements that are interpreted as insufficient, unwanted or simply threatening.

Given that it can be inferred that the use of these devices and algorithms is going to condition, in a subsidiary way, an increase of the consultations to the health system, it would be interesting to program some type of compensation with the objective of improving these systems, avoiding unnecessary worries, and avoiding an excess of use of the health system by healthy people who objectively do not need it.

A simple proposal would be to bill the corresponding biometric application for the expenses derived from health consultations originated by it. This measure would encourage each application to construct its own filters and decision algorithms according to the results obtained, in which it should be clearly stipulated that consultation with the health professional should only be carried out when there are objective criteria that indicate this.





De la mano de los avances en telefonía móvil vamos acostumbrándonos a que nuestros aparatos de comunicación personal vayan incorporando cada vez más prestaciones. Esto implica que el hardware que portamos va a disponer de una mayor capacidad de medición de parámetros vitales. El móvil conoce los pasos que damos, la velocidad de estos, el ritmo de movimiento a la par que puede detectar el tiempo de reposo o sueño. Existen teléfonos capaces de medir las pulsaciones cardiacas, conocer el ritmo, incluso hacer un electrocardiograma.

En la medida que estos avances condicionen un aumento de la biometría, es lógico pensar que detectarán condiciones o patrones que consideren anormales. Cuando el sujeto de medición se enfrente con los mismos no tendrá forma de saber si son normales o no. Esto condicionará un aumento de las consultas a profesionales sanitarios a los que se presentarán las mediciones obtenidas para tratar de encontrar una interpretación. 

En el momento actual los wearables, pulseras, teléfonos inteligentes, anillos inteligentes, etc… están diseñados predominantemente para un perfil joven y deportista. Éstos los utilizan para mejorar su condición física o mantenerla. En el momento en que se universalicen una cantidad creciente de ciudadanos los usarán y entre ellos personas con diferentes grados de enfermedad o de factores de riesgo para la misma.

La biometría es indudablemente atrayente por cuanto ofrece una cuantificación del propio ser y nos permite una medición del mismo dándonos una sensación de control que podemos dirigir hacia la consecución de una mejor salud. El lado oscuro de este exceso de medición tiene que ver con la inevitable preocupación que surgirá frente a mediciones Que se interpreten como insuficientes, no deseadas o simplemente amenazantes.

Dado que se puede inferir que el uso de estos dispositivos y algoritmos va a condicionar de forma subsidiaria subsidiaria un aumento de las consultas al sistema de salud, sería interesante programar algún tipo de compensación con la objetivo de mejorar estos sistemas, evitar preocupaciones innecesarias, y evitar un exceso de uso del sistema sanitario por personas sanas que objetivamente no lo necesitan.

Una propuesta sencilla sería facturar a la aplicación biométrica correspondiente los gastos derivados de consultas sanitarias originadas por la misma. Esta medida estimularía para que cada aplicación construya sus propios filtros y algoritmos de decisiones según los resultados obtenidos en los que debería estar claramente estipulado que la consulta al profesional sanitario deba hacerse únicamente cuando existan criterios objetivos que así lo indiquen.

¿Existen los médicos nivel dios? Do god level doctors exist? 上帝级医生存在吗?




Recently there was some controversy on Twitter from a translation of mine of a commentary by psychiatrist Allen Frances that questioned the statistical literacy of doctors. Not managing the available statistical information well would be, according to him, the cause of overdiagnosis and overtreatment.

One of the later comments expressed his discomfort with the previous assessment as if the translator of the same was a doctor level god and the rest a panda of illiterate statisticians. I apologized and tried to clarify the mistake that I apparently got when my apologies were accepted.
However I was left with a question to carry, if the practice of medicine is currently so extremely complex who would be today's god level doctor?

Some will answer that Dr. House, the one capable of solving any question helped by all the technology possible, others will answer with the names of famous super-specialists or some media doctor of those who appear on television. These weeks maybe some of you will remember Dr. Veronica Casado, recently awarded as the best family doctor in the world.

And it is really difficult to say what technical and human skills are essential given that regardless of what the curricula say patients seem to want different skills that do not always coincide with what the academic training says or the preferences of the doctor.

I am fortunate to know good doctors who stand out for different qualities. There are those of easy verb and stupendous communication skills, others with privileged and encyclopaedic minds, there are also those who are versatile and resolute, skilled with the scalpel or with the ability to see much more than is apparent. I know some with a fabulous clinical eye, with powerful statistical skills or with a deep reflective or analytical capacity.

However, it would be difficult for me to say who is the most complete or the only canon to imitate. In fact when I have had students or residents in my charge I have tried to offer them a good handful of examples so that they themselves could choose which referents could be of greater help to them.

What causes me the most pain is knowing that many excellent colleagues cannot do a better job because they are subjected to nonsensical pressures or because they belong to services or units that do not recognize their worth and condemn them to a charitable assistance activity that does not allow for research, sufficient training or minimum professional promotion.

What is clear to me is that the god-level doctor is not necessarily the most successful in the media, financially or professionally. As a village doctor I know firsthand how important it is to try to be diligent and worry as much as possible about the issues that come up every day. It may not be possible for us to be up to date in all matters or to have a complementary private activity that gives us abundant income, but we have at least the possibility of maintaining a dignified position and offering a conscientious service. Allow me to try, it is not in my objectives to become a god level doctor, but to be able to be a little more human, to attend the best I can to my patients and to help and inspire my colleagues as much as possible.









Hace poco hubo cierta polémica en Twitter a partir de una traducción mía de un comentario del psiquiatra Allen Frances que cuestionaba la alfabetización estadística de los médicos. No manejar bien la información estadística disponible sería según él causa de sobrediagnóstico y sobretratamiento.

Uno de los comentarios posteriores expresaba su malestar con la anterior valoración como si el traductor de la misma fuera un médico nivel dios y el resto una panda de iletrados estadísticos. Me disculpé y traté de aclarar el entuerto cosa que al parecer conseguí siendo mis disculpas aceptadas.
Sin embargo me quedé con una pregunta a cuestas, si el ejercicio de la medicina es actualmente tan sumamente complejo ¿quién sería hoy el médico nivel dios?

Alguno contestará que el doctor House, aquel capaz de resolver cualquier cuestión ayudado de toda la tecnología posible, otros contestarán con los nombres de superespecialistas célebres o  algún médico mediático de los que salen en televisión. Estas semanas quizá alguno se acuerde de la doctora Verónica Casado recientemente premiada como la mejor médica de familia del mundo.

Y es que es verdaderamente difícil decir qué competencias técnicas y humanas son imprescindibles dado que independientemente de lo que digan los planes de estudio los pacientes parecen desear habilidades diferentes que no siempre coinciden con lo que la formación académica dice o las preferencias del galeno.

Tengo la suerte de conocer buenos médicos que destacan por cualidades diferentes. Los hay de verbo fácil y estupendas dotes de comunicación, otros con privilegiadas y enciclopédicas mentes, también los hay polivalentes y resolutivos, hábiles con el bisturí o con la habilidad de ver mucho más de lo aparente. Conozco algunos con un ojo clínico fabuloso, con destrezas estadísticas potentes o con una profunda capacidad reflexiva o analítica.

Sin embargo me costaría decir quién es el más completo o el único canon a imitar. De hecho cuando he tenido estudiantes o residentes a mi cargo he intentado ofrecerles un buen puñado de ejemplos para que ellos mismos eligieran qué referentes les pudieran servir de mayor ayuda.

Lo que más dolor me produce es saber que muchos excelentes compañeros no pueden desarrollar una mejor labor al estar sometidos a presiones asistenciales disparatadas o pertenecer a servicios o unidades que no reconocen su valía y les condenan a una actividad asistencial de beneficencia que no permite investigación, formación suficiente o promoción profesional mínima.

Lo que tengo claro es que el médico nivel dios no es necesariamente el que tiene más éxito mediático, económico o profesional. Como médico de pueblo conozco de primera mano lo importante que es tratar de ser diligente y preocuparse todo lo posible por las cuestiones que cada día llegan a consulta. Quizá  a los médicos de a pie no nos saquen jamás por televisión ni nos den galardones de prestigio, quizá no nos sea posible estar actualizados en todas las materias ni tener una actividad privada complementaria que nos dé ingresos abundantes pero tenemos al menos la posibilidad de mantener una posición digna y ofrecer un servicio esmerado. Permítanme intentarlo, no está en mis objetivos llegar a ser un médico nivel dios, pero sí llegar a ser capaz de ser un poco más humano, atender lo mejor que pueda a mis pacientes y ayudar e inspirar en lo posible a mis colegas.





上帝级医生存在吗?

最近在Twitter上出现了一些争议,这些争议来自于精神病学家艾伦·弗朗西斯(Allen Frances)的一篇评论的翻译,他质疑医生的统计素养。他认为,不能很好地管理现有的统计信息是过度诊断和过度治疗的原因。

后来的一篇评论表达了他对先前的评估的不满,就好像同一篇评论的译者是博士级的上帝,而其余的则是一群不识字的统计学家。我道歉,试图澄清当我的道歉被接受时我显然犯的错误。
不过我还是有一个问题要提,如果现在的医学实践如此复杂,谁会是当今的上帝级医生呢?

有些人会回答豪斯博士,一个能够解决所有可能的技术帮助的任何问题的博士,其他人会回答那些出现在电视上的著名超级专家或媒体医生的名字。这几周也许你们中的一些人会记得Veronica Casado博士,她最近被评为世界上最好的家庭医生。

而且很难说哪些技术和人的技能是必不可少的,因为无论课程说什么,病人似乎想要不同的技能,而这些技能并不总是与学术培训所说的或者医生的偏好相一致。

我很幸运能认识到优秀的医生。有那些简单的动词和惊人的沟通技巧,其他有特权和百科全书的头脑,也有那些是多才多艺和果断,熟练的手术刀或能力看到远远超过明显。我认识一些人,他们具有惊人的临床眼光,具有强大的统计能力,或者具有深刻的反思或分析能力。

然而,对我来说,很难说谁是最完整的,或者说谁是唯一可以仿效的经典。事实上,当我有学生或居民负责时,我试着给他们举几个例子,让他们自己选择哪些参考资料对他们更有帮助。

最让我痛心的是知道许多优秀的同事不能做得更好,因为他们受到无意义的压力,或者因为他们属于不承认自己价值的服务机构或单位,并让他们参加不允许fo的慈善援助活动。研究、充分培训或最低专业晋升。

我清楚的是,在媒体上,无论是在财务上还是在专业上,神职医生不一定是最成功的。作为一名乡村医生,我亲身体会到,对于每天出现的问题,尽可能地勤奋和担心是多么的重要。我们可能不可能在所有事情上都做到最新,也不可能有一个能给我们带来丰厚收入的互补性私人活动,但我们至少有可能保持一个有尊严的职位,并提供尽职尽责的服务。请允许我尝试一下,我的目标不是成为一名神圣的医生,而是能够变得更加人性化,能够尽我所能地照顾我的病人,并尽可能地帮助和激励我的同事。
