El peligro de que los sanitarios entren en modo supervivencia. The danger of health professionals going into survival mode. 卫生专业人员进入生存模式的危险。





Tratar con el dolor y la enfermedad ajena no es sencillo para nadie. Tampoco para los que tienen formación especializada. El sufrimiento impregna la parte invisible del alma humana, tanto del que lo padece como del que lo acompaña.

Por eso es necesario que los profesionales sanitarios tengan una formación técnica y humana completa y trabajen con medios que les permitan ofrecer un servicio de máxima calidad. Lo que vemos habitualmente es que las instituciones sanitarias conciben la calidad desde un punto de vista exclusivamente científico-técnico y se centran en proveer medios técnicos, farmacológicos e informáticos. La parte humana se obvia y, pese a estar presente en el discurso oficial, no se cuidan las condiciones laborales ni de protección de sobrecargas de los profesionales. En ocasiones los profesionales trabajan en clara situación de maltrato, como los residentes que encadenan tres turnos de trabajo seguidos en sus guardias o los médicos de familia obligados a atender a más de 60 pacientes en una jornada.

Es cierto que el entrenamiento de los profesionales permite que estos puedan asumir aumentos puntuales de demanda asistencial, pero no son de piedra. Nadie aguanta una sobrecarga permanente sin entrar en modo supervivencia. Y si sobrevivir se convierte en la prioridad, el paciente, el sistema y todo lo demás será secundario. Esto no beneficia a nadie pero cada vez es más frecuente porque son más los profesionales que se sienten sobrepasados, ninguneados, sobrecargados y abusados. La situación es global, pasa en todos los países, y tiene que ver con la dinámica social que ha convertido la salud en un bien de consumo y los sistemas sanitarios en grandes supermercados de servicios que cada vez han de producir más. Si las necesidades de salud siguen creciendo a la par que lo hacen la edad media y la complejidad de los pacientes, podrán imaginar que si los recursos no aumentan serán los profesionales quienes se carguen sobre sus hombros el aumento progresivo de trabajo.

El problema es que en modo supervivencia cada profesional se quita de encima los problemas lo más rápido que puede. Se prescriben más pruebas diagnósticas y más tratamiento, se deriva más de un profesional a otro. Se mantiene el problema en superficie sin que nadie coja el toro por los cuernos. Esto desencadena cadenas gasto inasumible, pérdidas de tiempo y de recursos, y pocos resultados.
Cuando es la Atención Primaria lo más abusado y menos cuidado el problema aumenta al ser en este nivel donde más información blanda se tiene del paciente y donde se pueden manejar el 90% de sus problemas si se dieran las condiciones adecuadas. Estrangular este nivel asistencial es condenar al fracaso la totalidad del sistema y es lo que estamos viendo año tras año con el asombro de unos, el consentimiento de otros y la dejadez o estulticia de alguno más.

Permitir que un médico de familia tenga que asumir en verano cinco consultas a la vez debería ser delito. Permitir que haya centros de salud cuyos médicos tengan que asumir las consultas no suplidas de compañeros durante meses no debería tolerarse.

Pero es lo que hay, y cada vez más profesionales quemados, con la moral por los suelos o agotados. Entramos en modo supervivencia, no pidan empatía, humanismo ni florituras. No las va a haber.

Dealing with the pain and illness of others is not easy for anyone. Nor is it easy for those with specialized training. Suffering permeates the invisible part of the human soul, both of the sufferer and of those who accompany him. 

That is why it is necessary for health professionals to have a complete technical and human formation and to work with means that allow them to offer a service of maximum quality. What we usually see is that health institutions conceive of quality from an exclusively scientific-technical point of view and focus on providing technical, pharmacological and IT means. The human side is forgotten, in spite of being present in the official discourse, the working conditions and overload protection of the professionals are not taken care of. Sometimes the professionals work in a clearly abusive situation, such as residents who work three shifts in a row on their guards or family doctors who are forced to attend to more than 60 patients in a day. 

It is true that the training of the professionals allows them to take on occasional increases in the demand for care, but they are not made of stone. No one can withstand a permanent overload without going into survival mode. And if survival becomes the priority, the patient, the system and everything else will be secondary. This does not benefit anyone but it is increasingly frequent because more and more professionals feel overloaded, unaware, overburdened and abused. The situation is global, it happens in all countries, and it has to do with the social dynamics that have turned health into a consumer good and the health systems into large service supermarkets that have to produce more and more. If health needs continue to grow at the same rate as the average age and complexity of patients, you can imagine that if resources do not increase, it will be the professionals who will have to shoulder the progressive increase in work. 

The problem is that in survival mode each professional gets rid of the problems as quickly as possible. More diagnostic tests and more treatment are prescribed, more are referred from one professional to another. The problem is kept on the surface without anyone taking the bull by the horns. This triggers chains of unconscionable expense, loss of time and resources, and poor results. 
When it is the Primary Care that is most abused and least cared for, the problem increases as it is at this level where more soft information is held on the patient and where 90% of their problems can be managed if the right conditions are given. To strangle this level of care is to condemn the whole system to failure and this is what we are seeing year after year with the astonishment of some, the consent of others and the negligence or silliness of some others. 

It should be a crime to allow a family doctor to have to take on five consultations at a time in the summer. Allowing health centres to have doctors take over unsupplied consultations from colleagues for months on end should not be tolerated. 

But there are, and more and more professionals who are burned out, morally bankrupt or exhausted. We're going into survival mode, so don't ask for empathy, humanism or frills. There won't be any.
